  • Common: Dry, persistent, non-productive cough; taste disturbance (metallic taste or loss of taste); skin rash; dizziness; headache; mild increase in serum potassium.
  • Less common but important: First-dose hypotension, especially in volume-depleted patients; proteinuria.
  • Rare but serious: Angioedema (swelling of face, lips, tongue, or throat — requires immediate discontinuation and medical attention); neutropenia or agranulocytosis (more common with renal impairment or collagen vascular disease); acute renal failure (especially with bilateral renal artery stenosis); cholestatic jaundice progressing rarely to hepatic necrosis.

Boxed Warning: Fetal Toxicity

Acetor can cause injury and death to the developing fetus when used during the second and third trimesters of pregnancy. Acetor must be discontinued as soon as pregnancy is detected, and an alternative agent should be used if continued antihypertensive therapy is needed. Acetor is contraindicated in pregnancy.

Lactation

Acetor is excreted into breast milk in small amounts. Because of the potential for adverse effects in a nursing infant, Acetor should be used during breastfeeding only if clearly needed and the potential benefit justifies the potential risk; a physician should be consulted, and the infant should be monitored for signs of hypotension.

  • Renal impairment / bilateral renal artery stenosis: Acetor can cause acute renal failure in patients with bilateral renal artery stenosis or stenosis of the artery to a solitary kidney; renal function should be monitored, especially in patients with pre-existing kidney disease.
  • Hyperkalemia: Risk is increased with renal impairment, diabetes, or concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes (see Interactions).
  • Neutropenia/agranulocytosis: Rare, but risk is higher in patients with renal impairment, especially those with collagen vascular disease (e.g., systemic lupus erythematosus, scleroderma); periodic monitoring of white blood cell counts may be advised by the physician in these patients.
  • Angioedema: Can occur at any time during treatment; if swelling of the face, lips, tongue, or throat occurs, discontinue Acetor immediately and seek emergency care.
  • Cough: A persistent dry cough is a known class effect of Acetor and other ACE inhibitors; it is not dangerous but is a common reason patients discontinue therapy — discuss with a physician rather than stopping abruptly on your own.
