Medicine overview
Indications of Erian
Erian is a topical anorectal (rectal) combination preparation. Its established/labeled uses include:
- Established use: symptomatic relief of pain, itching (pruritus ani), burning and inflammation associated with internal and external hemorrhoids (piles).
- Established use: symptomatic treatment of anal fissures and proctitis where secondary bacterial infection is present or likely.
- Adjunct use: short-term relief following anorectal surgical procedures (e.g. post-hemorrhoidectomy) as an adjunct to standard post-operative care, on medical advice.
Erian is intended for short-term symptomatic use only and does not treat the underlying cause of hemorrhoidal disease; it is not a substitute for evaluation of unexplained rectal bleeding.
Composition
Cinchocaine + Hydrocortisone + Framycetin + Esculin is a fixed-dose combination containing four active components, each contributing a distinct action:
- Cinchocaine (dibucaine) – an amide-type local anesthetic.
- Hydrocortisone – a mild-to-moderate potency topical corticosteroid.
- Framycetin – an aminoglycoside antibiotic active against common perianal bacterial pathogens.
- Esculin – a plant-derived (horse chestnut, Aesculus hippocastanum) coumarin glycoside with vasoprotective and anti-inflammatory (capillary-protective) properties.
Description
Erian is a multi-component topical preparation formulated as an ointment/cream and/or suppository for anorectal use. It combines a local anesthetic, a corticosteroid, an antibiotic, and a vasoprotective agent to address the pain, inflammation, infection risk, and vascular fragility that commonly occur together in hemorrhoidal and other anorectal conditions. Erian is intended for short courses of symptomatic relief under medical guidance rather than for long-term or indiscriminate use.
Therapeutic Class
Anorectal preparation: combination local anesthetic, corticosteroid, aminoglycoside antibiotic, and vasoprotective agent used topically for hemorrhoids and related anorectal conditions.
Pharmacology
Cinchocaine + Hydrocortisone + Framycetin + Esculin acts through the combined, complementary mechanisms of its four components:
- Cinchocaine blocks voltage-gated sodium channels in sensory nerve fibers, reversibly inhibiting nerve impulse conduction and producing local anesthesia that relieves pain, burning and itching.
- Hydrocortisone reduces local inflammation by suppressing the release of inflammatory mediators, decreasing capillary permeability, and relieving swelling, redness and itching.
- Framycetin binds the bacterial 30S ribosomal subunit, inhibiting protein synthesis and providing bactericidal activity against common perianal Gram-positive and Gram-negative organisms, reducing the risk of secondary infection.
- Esculin reduces capillary fragility and permeability and has mild anti-inflammatory and anti-edematous (vasoprotective) activity, supporting the venous/vascular component of hemorrhoidal disease.
Dosage & Administration of Erian
Adults
| Indication | Dosage form | Typical regimen |
|---|---|---|
| External hemorrhoids, anal fissure, pruritus ani | Ointment/cream | Apply a small amount to the affected area 2–3 times daily and after each bowel motion, for a short course (usually not exceeding 5–7 days unless a physician directs otherwise) |
| Internal hemorrhoids, proctitis | Suppository (where available) | Insert one suppository rectally 1–2 times daily and after defecation, for a short course as above |
Erian should be used exactly as directed by a physician; do not use for longer than the recommended duration, do not use more frequently than directed, and do not share this medicine with others without medical advice.
Administration of Erian
Wash and dry the affected area gently before use. For external application, apply a thin layer of ointment/cream to the perianal skin or, using the applicator provided, just inside the anal canal. For suppositories, insert the suppository rectally, pointed end first, preferably after a bowel movement. Wash hands before and after application. Avoid contact with the eyes. Erian is for external/rectal use only and must not be taken orally.
Interaction of Erian
Systemic absorption from short-term topical anorectal use of Erian is normally low, so clinically significant drug interactions are uncommon; however:
- Concurrent use of other topical or systemic corticosteroids on the same area may increase the risk of local corticosteroid adverse effects (see Precautions).
- Concurrent use of other topical or systemic aminoglycoside antibiotics, or use over large denuded/broken skin areas, may increase systemic absorption of framycetin and the theoretical risk of ototoxicity/nephrotoxicity, particularly with prolonged or extensive use.
- Avoid applying other anorectal medicines to the same area at the same time unless advised by a physician, to reduce the risk of additive local irritation.
Contraindications
Cinchocaine + Hydrocortisone + Framycetin + Esculin is contraindicated in:
- Known hypersensitivity to cinchocaine, hydrocortisone, framycetin, esculin, other aminoglycosides, or any excipient of the formulation.
- Viral (e.g. herpes simplex, varicella, vaccinia), fungal, or tuberculous anorectal/perianal lesions, because the corticosteroid component can mask signs of infection and permit its spread.
Side Effects of Erian
Most adverse effects of Erian are local and mild, particularly with short-term use:
- Common: local burning, stinging, itching, or irritation at the application site.
- Less common: allergic contact dermatitis or hypersensitivity reaction to any component.
- With prolonged/excessive use: local skin atrophy, striae, telangiectasia, or delayed wound healing (corticosteroid effect); secondary fungal or resistant bacterial overgrowth; sensitization to framycetin; rarely, systemic aminoglycoside effects (ototoxicity, nephrotoxicity) with extensive or prolonged application.
Pregnancy & Lactation
Data on the use of Erian in pregnancy and lactation are limited. As with other topical corticosteroid/antibiotic combinations, it should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus, and only for the shortest effective duration. It is not known whether topically applied components are excreted in clinically significant amounts in breast milk; caution is advised, and a physician should be consulted before use during breastfeeding, particularly for prolonged or extensive application.
Precautions & Warnings
Erian is intended for short-term use only; prolonged or extensive application increases the risk of local corticosteroid effects (skin atrophy, striae) and of sensitization or systemic absorption of the aminoglycoside component. Do not use for longer than the period recommended by a physician.
- Do not use if there is active or unexplained anorectal bleeding without first consulting a physician to rule out more serious underlying pathology.
- To help limit antibiotic resistance, use the antibiotic (framycetin) component only as directed; do not use indiscriminately, do not stop, extend, skip doses, or share this medicine with others without medical advice.
- Avoid contact with the eyes and avoid application to broken or extensively denuded skin unless directed by a physician.
- Discontinue and consult a physician if signs of hypersensitivity, worsening irritation, or infection develop during use.
Overdose Effects of Erian
Overdose with topical anorectal use of Erian is unlikely but excessive or prolonged use may increase the risk of local and, rarely, systemic adverse effects described above. If the product is accidentally ingested, or if signs of a serious reaction (e.g. severe allergic reaction, hearing changes, or reduced urine output) occur, seek immediate medical attention or contact a poison control center; do not attempt specific home treatment.
Storage Conditions
Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.
Use In Special Populations
Elderly: no specific dose adjustment is established; use the shortest effective course as in other adults, with attention to skin fragility.
Renal impairment: limited data; because systemic absorption is normally low with short-term topical use, no specific dose adjustment is established, but prolonged/extensive use should be avoided due to the theoretical risk of aminoglycoside accumulation.
Hepatic impairment: no specific dose adjustment is established with short-term topical use.
Pediatric patients: see Pediatric Uses.
Duration Of Treatment
Treatment with Erian is generally limited to short courses, typically not exceeding 5–7 days, unless a physician directs a longer course after reassessment. If symptoms do not improve within this period, or worsen, medical review is required before continuing.
Drug Classes
Local anesthetic (amide type) + topical corticosteroid + aminoglycoside antibiotic + vasoprotective/venotonic agent, combined for anorectal (rectal/perianal) topical use.
Mode Of Action
Cinchocaine + Hydrocortisone + Framycetin + Esculin combines four complementary actions at the anorectal site: local anesthesia (sodium channel blockade by cinchocaine) to relieve pain and itching, anti-inflammatory activity (hydrocortisone) to reduce swelling and inflammation, antibacterial activity (framycetin, via inhibition of bacterial protein synthesis) to control or prevent secondary infection, and vasoprotection (esculin) to reduce capillary fragility and permeability contributing to hemorrhoidal symptoms.
Pediatric Uses
Safety and efficacy of Erian in children have not been well established. It is generally not recommended for use in young children and infants due to a lack of adequate data and the theoretical risk of increased systemic absorption of the corticosteroid and aminoglycoside components through more permeable skin/mucosa. Use in children should only occur on the specific advice of a physician, at the lowest effective dose for the shortest possible duration.
Frequently Asked Questions
