Fixal Plus is a fixed-dose combination of a non-sedating (second-generation) antihistamine, fexofenadine, and a sympathomimetic nasal decongestant, pseudoephedrine. It is indicated for the relief of symptoms associated with seasonal allergic rhinitis (hay fever) in adults and adolescents.

Established / FDA-approved uses

• Seasonal allergic rhinitis with nasal congestion: relief of nasal congestion together with sneezing, rhinorrhea (runny nose), itchy/watery eyes, and itching of the nose or throat caused by seasonal allergies (e.g., pollen).

Important limitations

Fixal Plus is intended only for patients who have both allergic symptoms (histamine-mediated) AND nasal congestion. It is not indicated for the common cold, for allergic rhinitis without congestion (fexofenadine alone is preferred in that case), or for chronic idiopathic urticaria (fexofenadine alone is used for that indication). Fixal Plus is not an antibiotic and does not treat sinus infections or bacterial illness.

Each extended-release tablet of Fexofenadine + Pseudoephedrine typically contains Fexofenadine Hydrochloride 60 mg and Pseudoephedrine Hydrochloride 120 mg (12-hour formulation); some brands market a once-daily 180 mg/240 mg extended-release combination. Exact strengths, release profile, and inactive ingredients vary by manufacturer/brand; refer to the specific product label.

Fixal Plus is an oral fixed-dose combination product. Fexofenadine is a peripherally selective, non-sedating H1-receptor antagonist (the active carboxylic acid metabolite of terfenadine) that relieves allergic symptoms without significant central nervous system penetration at recommended doses. Pseudoephedrine is an orally active sympathomimetic amine that constricts swollen nasal blood vessels, relieving nasal and sinus congestion.

Fixal Plus is typically formulated as a bilayer or matrix extended-release tablet, combining an immediate/extended-release antihistamine layer with an extended-release decongestant layer, and is intended for oral use only.

Fexofenadine + Pseudoephedrine combines two agents with complementary mechanisms for allergic rhinitis:

• Fexofenadine selectively and competitively blocks peripheral histamine H1-receptors, inhibiting histamine-mediated symptoms such as sneezing, itching, and rhinorrhea. It has low affinity for CNS receptors and minimal ability to cross the blood-brain barrier, which accounts for its low sedation potential compared with first-generation antihistamines.
• Pseudoephedrine is an indirect/direct-acting sympathomimetic amine that stimulates alpha-adrenergic receptors in the nasal mucosa, causing vasoconstriction of dilated blood vessels, reduced mucosal edema, and improved nasal airflow. It also has mild beta-adrenergic activity, which underlies its cardiovascular and CNS-stimulant effects.

Following oral administration of Fexofenadine + Pseudoephedrine, fexofenadine reaches peak plasma concentration in about 1-3 hours and pseudoephedrine somewhat later from the extended-release layer, providing combined relief of both allergic and congestive symptoms over the labeled dosing interval (commonly 12 hours). Fexofenadine undergoes minimal hepatic metabolism and is eliminated largely unchanged in feces and urine; pseudoephedrine is partly metabolized and partly excreted unchanged in urine, with a pH-dependent renal elimination.

• Take Fixal Plus tablets whole with a full glass of water; do not crush, split, or chew extended-release tablets, as this can alter the controlled-release profile.
• Take on an empty stomach or as directed; avoid taking with fruit juices (apple, orange, grapefruit), which can reduce absorption of the fexofenadine component — take with plain water instead.
• Avoid taking aluminum- or magnesium-containing antacids at the same time as Fixal Plus; separate dosing by at least 2 hours if both are needed.
• Take the evening/second dose several hours before bedtime if insomnia or nervousness occurs, since the pseudoephedrine component can be stimulating.
• Do not exceed the dose or frequency stated on the product label or by your physician.

Clinically significant interactions with Fixal Plus include:

• Monoamine oxidase inhibitors (MAOIs): concurrent use, or use within 14 days of stopping an MAOI, can precipitate a hypertensive crisis due to the pseudoephedrine component; this combination is contraindicated (see Contraindications).
• Other sympathomimetics/decongestants/stimulants (e.g., other cold/flu products, appetite suppressants, ADHD stimulants): additive risk of hypertension, tachycardia, and CNS stimulation; avoid combining with other pseudoephedrine- or phenylephrine-containing products.
• Antihypertensive agents (e.g., beta-blockers, methyldopa, guanethidine-type agents): pseudoephedrine's sympathomimetic action may reduce the antihypertensive effect and increase blood pressure.
• Fruit juices (apple, orange, grapefruit): can reduce absorption/bioavailability of fexofenadine by inhibiting intestinal drug-uptake transporters; take Fixal Plus with water rather than fruit juice.
• Aluminum/magnesium-containing antacids: may reduce fexofenadine absorption; separate administration by at least 2 hours.
• Ketoconazole or erythromycin: can increase plasma concentrations of fexofenadine via transporter-mediated interactions; clinical significance is usually limited given fexofenadine's wide safety margin, but caution is still advised.

Report all other medicines, herbal products, and supplements to your physician or pharmacist before starting Fixal Plus.

Side effects reported with Fixal Plus reflect the combined effects of both components:

Common

• Insomnia, nervousness, restlessness, or anxiety (pseudoephedrine)
• Headache
• Dizziness
• Dry mouth or throat irritation
• Nausea
• Palpitations or a sensation of rapid heartbeat

Less common but clinically important

• Significant elevation of blood pressure or heart rate, especially in predisposed patients
• Tremor
• Difficulty urinating/urinary retention, particularly in men with prostatic enlargement
• Hypersensitivity reactions, including rash, urticaria, and rarely angioedema
• Rare cardiac arrhythmias or seizures with excessive intake

Seek prompt medical advice for signs of an allergic reaction, chest pain, marked palpitations, severe dizziness, or difficulty breathing (see Precautions and Warnings).

Pregnancy: Adequate, well-controlled studies of Fixal Plus in pregnant women are limited. Pseudoephedrine has been associated in some observational studies with a small increased risk of rare fetal abnormalities (e.g., gastroschisis) with first-trimester use, and sympathomimetic vasoconstriction could theoretically reduce uterine blood flow. Fixal Plus should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus; consult a physician before use, especially in the first trimester.

Lactation: Both fexofenadine and pseudoephedrine pass into breast milk in small amounts; pseudoephedrine may also reduce milk supply. Fixal Plus should be used with caution while breastfeeding, and a physician should be consulted before use, particularly with repeated dosing.

The following precautions apply to the use of Fixal Plus:

• Cardiovascular disease: use with caution in patients with hypertension, coronary artery disease, or arrhythmias, since the pseudoephedrine component can raise blood pressure and heart rate (severe forms of these conditions are contraindications; see Contraindications).
• Hyperthyroidism: pseudoephedrine's sympathomimetic effects may be exaggerated in thyrotoxicosis; use with caution.
• Diabetes mellitus: use with caution, as sympathomimetic agents can affect glycemic control.
• Prostatic hypertrophy/urinary retention: pseudoephedrine can worsen urinary obstruction symptoms in men with benign prostatic hyperplasia; use with caution and seek medical advice if urination becomes difficult.
• Narrow-angle glaucoma: contraindicated (see Contraindications); use pseudoephedrine-containing products with caution even in other forms of glaucoma.
• CNS stimulation: may cause insomnia, nervousness, restlessness, tremor, or anxiety; avoid dosing close to bedtime and use caution when driving or operating machinery until individual response is known.
• Renal impairment: both components are renally cleared to some degree; dose or frequency adjustment, or use of the individual components, may be needed (see Use in Special Populations).
• Duration of use: do not use for more than 7 days without medical advice; worsening or new symptoms (fever, persistent cough, or symptoms lasting beyond a week) warrant physician evaluation.
• Not an antibiotic: Fixal Plus does not treat bacterial or viral infections and should not be substituted for prescribed antimicrobial therapy when one is indicated.

Overdose of Fixal Plus is likely to be dominated by the pseudoephedrine component and may cause central nervous system stimulation (restlessness, tremor, insomnia, anxiety, hallucinations, or seizures in severe cases), hypertension, tachycardia, palpitations, and other cardiac arrhythmias. Fexofenadine overdose has generally produced mild effects such as dizziness, drowsiness, or dry mouth. In case of suspected overdose, seek immediate medical attention or contact a poison control center/emergency services promptly; do not attempt unsupervised specific home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Pediatric use

Fixal Plus extended-release combination tablets are generally not recommended for children under 12 years of age; safety and efficacy in this age group have not been established for the fixed-dose combination (see Pediatric Uses).

Elderly

Elderly patients may be more sensitive to the sympathomimetic effects of pseudoephedrine (blood pressure elevation, urinary retention in men with prostatic enlargement, CNS stimulation) and may have age-related decreases in renal function; use with caution and physician guidance.

Renal impairment

Clearance of both fexofenadine and pseudoephedrine may be reduced in renal impairment, increasing systemic exposure and the risk of side effects. Use with caution; a physician may recommend dose/frequency adjustment or use of the individual components instead of the fixed-dose combination in significant renal impairment.

Hepatic impairment

Fexofenadine undergoes minimal hepatic metabolism, so hepatic impairment is not expected to significantly affect its levels; specific dose adjustment data for the combination in hepatic impairment are limited, so use with caution and medical advice.

Fixal Plus is intended for short-term, symptomatic relief of seasonal allergic rhinitis, typically used during periods of active allergen exposure (allergy season). Treatment should generally not exceed 7 days without consulting a physician, since prolonged use of the decongestant component can be associated with tachyphylaxis, rebound nasal congestion (on stopping), and sustained cardiovascular/CNS stimulant exposure. If symptoms persist beyond a week, worsen, or are accompanied by fever, medical reassessment is advised.

Fexofenadine + Pseudoephedrine belongs to the antihistamine-decongestant combination class: fexofenadine (a second-generation, non-sedating H1-antihistamine) combined with pseudoephedrine (a sympathomimetic amine/nasal decongestant).

Fixal Plus fixed-dose extended-release combination tablets are generally indicated only for patients 12 years of age and older; safety and efficacy in children under 12 years have not been established for this combination product, and it should not be given to younger children. In children and adolescents where treatment is appropriate, use should follow the specific product's approved age range and be supervised by a physician, with attention to signs of CNS stimulation (insomnia, nervousness) or cardiovascular effects, which children may be more sensitive to. Fexofenadine alone (without pseudoephedrine) has separate pediatric labeling and dosing for younger children with allergic rhinitis or urticaria, but this does not apply to the fixed-dose combination.
