Indications of Rimstar

Rimstar is a fixed-dose combination (FDC) of four first-line antituberculardrugs used together for the treatment of tuberculosis (TB).

Established / Guideline-Supported Uses

  • Intensive-phase treatment of active pulmonary and extrapulmonary tuberculosis intreatment-naive (new) adult and pediatric patients, as the standard first-line four-drug regimenrecommended by the World Health Organization (WHO), typically given daily for 2 months, ideallyunder directly observed therapy (DOT) where available.
  • Used as part of a complete anti-TB treatment course, usually followed by a continuation phasewith a reduced-drug combination (e.g., rifampicin plus isoniazid) for a further 4 months, asdetermined by the treating physician based on national/WHO TB guidelines.

Not Indicated For

  • Latent TB infection prophylaxis (single-agent isoniazid preventive therapy is used instead).
  • Drug-resistant TB (multidrug-resistant/rifampicin-resistant TB) without modification of theregimen by a TB specialist, since Rimstar contains rifampicin and isoniazid to which resistancemay exist.

Rimstar must only be used under the supervision of a physician experienced in TB management.

Composition

Rifampicin + Isoniazid + Pyrazinamide + Ethambutol is available as a fixed-dose combination tablet. A typical adult-strength tabletcontains:

Mode of Action of Each Component

  • Isoniazid inhibits synthesis of mycolic acids, essential components of themycobacterial cell wall; it is highly bactericidal against rapidly dividing bacilli.
  • Pyrazinamide is converted to its active form (pyrazinoic acid) inside thebacterium and is particularly active against semi-dormant bacilli residing in the acidicenvironment of caseous necrotic lesions and macrophages, contributing significantly tosterilizing activity.
  • Ethambutol inhibits arabinosyl transferases involved in arabinogalactansynthesis in the mycobacterial cell wall, and is included mainly to help prevent the emergenceof resistance to the other components.

Pharmacokinetically, absorption of rifampicin is reduced by food, so Rifampicin + Isoniazid + Pyrazinamide + Ethambutol is generally besttaken on an empty stomach; rifampicin is also a potent inducer of hepatic cytochrome P450 enzymes(see Interactions). How to Take

  • Take the full dose at one time, once daily, preferably on an empty stomach (at least 1 hourbefore or 2 hours after food) with a full glass of water, as food can reduce absorption ofrifampicin.
  • Ideally taken under directly observed therapy (DOT) as recommended by national TB programs.

Administration of Rimstar

Rimstar should be taken by mouth, once daily, as a single dose, preferably on an empty stomach(at least 1 hour before or 2 hours after a meal) to optimize absorption of rifampicin. Tabletsshould be swallowed with a full glass of water. Dispersible pediatric formulations may bedissolved in a small amount of water immediately before administration, if labeled for such use.Doses are best supervised (directly observed therapy) to support adherence and completion ofthe full course.

Interaction of Rimstar

Rimstar contains rifampicin, a potent inducer of hepatic cytochrome P450 enzymes, which isresponsible for most of the clinically significant interactions with this combination. Allconcurrent medications should be reviewed by a physician or pharmacist before and duringtreatment.

Clinically Significant Interactions

  • Hormonal contraceptives (oral, implant, or other): rifampicin induces theirmetabolism, markedly reducing contraceptive efficacy; an alternative, non-hormonal method ofcontraception should be used during and for some time after treatment.
  • Antiretroviral drugs (e.g., protease inhibitors, some non-nucleoside reversetranscriptase inhibitors): rifampicin can substantially reduce plasma levels of manyantiretrovirals; regimens must be co-managed by an HIV/TB specialist.
  • Warfarin and other oral anticoagulants: rifampicin reduces anticoagulanteffect; more frequent INR monitoring and dose adjustment are needed.
  • Azole antifungals, corticosteroids, oral hypoglycemics (sulfonylureas), andcyclosporine/tacrolimus: rifampicin reduces plasma concentrations and efficacy of thesedrugs through enzyme induction.
  • Phenytoin: isoniazid can increase phenytoin levels (risk of toxicity), whilerifampicin can lower them; phenytoin levels should be monitored.
  • Alcohol: increases the risk of hepatotoxicity with both isoniazid andrifampicin; alcohol should be avoided during treatment.
  • Antacids (aluminum/magnesium-containing): can reduce absorption of isoniazidand ethambutol; separate dosing by at least 1–2 hours.
  • Other hepatotoxic drugs (e.g., paracetamol in high doses): additive risk ofliver injury; use with caution and monitor.

Because rifampicin affects so many drug classes, any new medication started during treatmentwith Rimstar should be checked for interactions.

Contraindications

Rifampicin + Isoniazid + Pyrazinamide + Ethambutol is contraindicated in patients with:

  • Known hypersensitivity to rifampicin, isoniazid, pyrazinamide, ethambutol,other rifamycins, or any component of the formulation.
  • Severe hepatic impairment or active severe liver disease, due to thesignificant hepatotoxic potential of the combination.
  • Acute gout, because pyrazinamide markedly raises serum uric acid and canprecipitate or worsen an acute attack.

Side Effects of Rimstar

Because Rimstar combines four active drugs, side effects may arise from any single component.Report any new or worsening symptom to your physician promptly.

Common

  • Loss of appetite, nausea, vomiting, abdominal discomfort
  • Orange-red discoloration of urine, sweat, tears, and saliva (harmless effectof rifampicin, but can permanently stain soft contact lenses)
  • Joint pain/arthralgia (related to pyrazinamide-induced hyperuricemia)
  • Headache, dizziness

Less Common but Clinically Important

Rare but Serious

  • Severe hepatic failure, thrombocytopenia, hemolytic anemia, acute kidney injury,Stevens-Johnson syndrome/toxic epidermal necrolysis (all rare).

Pregnancy & Lactation

Pregnancy: Untreated active tuberculosis poses a greater risk to both motherand fetus than treatment, so Rimstar is generally continued in pregnant women with active TBunder close specialist supervision. Ethambutol is considered to have a relatively favorablesafety profile among the four components in pregnancy, but the combination as a whole should beused only if the potential benefit justifies the potential risk, and only under the direction ofa physician experienced in TB management; pyridoxine (vitamin B6) supplementation is generallyrecommended during pregnancy to reduce the risk of isoniazid-associated neuropathy. Liverfunction should be monitored closely, as pregnancy may increase susceptibility tohepatotoxicity. Do not start or stop Rimstar in pregnancy without consulting a physician.

Lactation: The components of Rimstar are excreted into breast milk in smallamounts. Breastfeeding is generally considered compatible with maternal treatment using thisregimen, and treatment should not be delayed or withheld on this basis, but a physician should beconsulted, and the infant should be monitored; breast milk levels are not a substitute for directinfant TB prophylaxis/treatment if the infant requires it.

Precautions & Warnings

Hepatotoxicity

Rimstar carries a significant risk of drug-induced liver injury, particularly from rifampicin,isoniazid, and pyrazinamide in combination. Baseline liver function tests are recommended beforestarting treatment, with periodic monitoring during treatment, especially in patients withpre-existing liver disease, heavy alcohol use, older age, or malnutrition. Patients should seekimmediate medical attention for jaundice (yellowing of skin/eyes), dark urine, persistent nauseaor vomiting, unusual fatigue, or abdominal pain.

Ethambutol-Associated Optic Neuropathy

Ethambutol can cause dose-related optic neuritis, which may be irreversible if not detectedearly. Baseline visual acuity and color vision testing is recommended before starting treatment,with periodic monitoring, particularly for treatment courses longer than 2 months or higherdoses. Any change in vision, blurred vision, or difficulty distinguishing red from green must bereported immediately and the medicine discontinued promptly on medical advice.

Peripheral Neuropathy

Isoniazid-associated peripheral neuropathy is more likely in patients who are malnourished,diabetic, pregnant, have renal failure, or have alcohol use disorder or HIV infection.Co-administration of pyridoxine (vitamin B6) is often recommended, particularly in thesehigher-risk patients.

Hyperuricemia and Gout

Pyrazinamide commonly raises serum uric acid, which can cause arthralgia or precipitate gout;see Contraindications regarding acute gout.

Harmless Discoloration

Orange-red discoloration of urine, sweat, tears, and saliva is an expected and harmless effectof rifampicin; patients should be counseled about this in advance, including that soft contactlenses may become permanently stained.

Drug Interactions

Rifampicin is a potent inducer of hepatic enzymes with numerous significant interactions(see Interactions), including reduced efficacy of hormonal contraceptives; alternativecontraception should be discussed.

Overdose Effects of Rimstar

Overdose of Rimstar is a medical emergency. Features may include severe nausea and vomiting,dizziness, visual disturbances, and, in more severe isoniazid overdose, seizures, alteredconsciousness, and severe metabolic (lactic) acidosis; rifampicin overdose can cause pronouncedskin/urine discoloration, hypotension, and liver injury.

If an overdose is suspected, seek immediate medical attention or contact a poison controlcenter/emergency services right away. Do not attempt to treat an overdose at home. Hospital-basedsupportive care (and, for significant isoniazid overdose, specific antidotal treatmentadministered by medical professionals) may be required.

Storage Conditions

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Use In Special Populations

Renal Impairment

Ethambutol and pyrazinamide (and their metabolites) are renally eliminated and can accumulatein renal impairment, increasing the risk of ocular and other toxicity. Dose adjustment orextended dosing intervals, and closer monitoring, are generally required in moderate-to-severerenal impairment; a fixed-dose combination may not allow the flexibility needed and single-drugcomponents dosed separately may be preferred by the specialist in this situation.

Hepatic Impairment

Use with caution and close monitoring in pre-existing liver disease; contraindicated in severehepatic impairment/active severe liver disease (see Contraindications).

Elderly

Older patients may be at increased risk of hepatotoxicity and peripheral neuropathy; use withappropriate monitoring.

HIV Co-infection

Patients with HIV/TB co-infection require careful coordination of Rimstar with antiretroviraltherapy because of significant drug interactions with rifampicin (see Interactions); managed byan HIV/TB specialist.

Diabetes Mellitus

Patients with diabetes are at higher risk of peripheral neuropathy and may need closer glucosemonitoring, as rifampicin can affect oral hypoglycemic drug levels.

Duration Of Treatment
