Spotclen Plus

Spotclen Plus is a topical combination product used for the gradual lightening of localized areas of hyperpigmented skin, combined with sunscreen agents to help protect treated skin from further UV-induced darkening.

Established uses

• Melasma (chloasma) associated with pregnancy, oral contraceptive use, or hormone therapy
• Freckles (ephelides) and senile/solar lentigines
• Post-inflammatory hyperpigmentation and other forms of superficial epidermal melasma/hyperpigmentation

Adjunct role of the sunscreen components

Octyldimethyl P-aminobenzoate, dioxybenzone, and oxybenzone in this combination are not intended as a primary sunscreen for general sun protection; they are included specifically to reduce re-pigmentation of the treated area during hydroquinone therapy. Strict, separate broad-spectrum sun protection is still recommended.

This is a cosmetic/dermatologic combination and is not indicated for deep dermal pigmentation, nevi, or malignant lesions.

Each application of Hydroquinone + Octyldimethyl P-aminobenzoate + Dioxybenzone + Oxybenzone typically contains hydroquinone (a melanin-synthesis inhibitor) together with the sunscreen actives octyldimethyl PABA (padimate O), dioxybenzone, and oxybenzone, formulated as a topical cream or lotion for external use only.

Spotclen Plus is a fixed-dose topical combination used in dermatology for the management of hyperpigmentary skin disorders. Hydroquinone is a well-established depigmenting agent that lightens the skin by inhibiting melanin production, while octyldimethyl PABA, dioxybenzone, and oxybenzone are UV-filtering (sunscreen) agents included to limit sun-induced repigmentation and photodamage in the treated area during the course of therapy.

It is applied directly to the affected areas of skin and is not intended for oral, ophthalmic, or mucosal use.

Spotclen Plus belongs to the class of topical depigmenting (skin-lightening) agents combined with sunscreen/UV-filtering agents.

Hydroquinone + Octyldimethyl P-aminobenzoate + Dioxybenzone + Oxybenzone combines a depigmenting agent with UV-absorbing sunscreen actives.

Hydroquinone

Hydroquinone is thought to inhibit the enzyme tyrosinase, reducing conversion of tyrosine to melanin, and may also cause selective damage to melanocytes, reducing melanin production and existing pigment over several weeks of use. Effects are gradual and reversible on discontinuation, with pigmentation typically returning if sun exposure is not controlled.

Octyldimethyl PABA, dioxybenzone, and oxybenzone

These are organic UV filters that absorb ultraviolet radiation (UVB and, for dioxybenzone/oxybenzone, portions of UVA), reducing UV-triggered melanogenesis and helping prevent repigmentation of skin being treated with hydroquinone.

Spotclen Plus is for external (topical) use only. Wash and dry the affected area before application. Apply a thin layer and gently massage in; avoid contact with the eyes, lips, and mucous membranes. Wash hands after application unless the hands are the treated area. Use a separate broad-spectrum sunscreen on treated and surrounding skin during the day, in addition to this product, and limit sun exposure.

Concurrent use of other topical products containing peroxide, resorcinol, retinoids/tretinoin, or other exfoliating/irritant agents on the same area as Spotclen Plus may increase local irritation and should be spaced apart or avoided unless directed by a physician. There are no well-established clinically significant systemic drug interactions with this topical combination due to its low systemic absorption when used as directed.

Local skin reactions are the most commonly reported effects of Spotclen Plus, including:

• Mild stinging, burning, or itching at the application site
• Erythema (redness) and dryness
• Contact dermatitis or allergic sensitization

With prolonged, high-concentration, or unsupervised use, hydroquinone has been associated with exogenous ochronosis (a paradoxical blue-black darkening of the skin) and, rarely, temporary or permanent depigmentation of treated skin. Report persistent irritation or unexpected skin color changes to a physician promptly.

Systemic absorption of hydroquinone with normal topical use is generally low, but data on the sunscreen components' systemic absorption and fetal/infant effects are limited. Spotclen Plus should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus; a physician should be consulted before use. It is not known whether the components are excreted in breast milk in clinically significant amounts; caution is advised, and application to the breast/nipple area should be avoided during breastfeeding. Consult a physician before use while breastfeeding.

Use Spotclen Plus only on intact, non-irritated skin; avoid use on sunburned, broken, or eczematous skin. Perform a patch test before first use if sensitive skin is a concern. Strict sun avoidance and use of an additional broad-spectrum sunscreen are essential during treatment, as sun exposure can worsen hyperpigmentation and reduce treatment effectiveness. Prolonged use, use of high concentrations, or use without physician supervision increases the risk of exogenous ochronosis (paradoxical darkening) and should be avoided; limit duration of therapy to that recommended by the prescribing physician or product labeling. Avoid contact with the eyes, eyelids, lips, and mucous membranes. Discontinue and consult a physician if severe irritation, blistering, or unexpected pigment changes occur. Not intended for use on children.

Topical overdose most commonly presents as increased local irritation, redness, or chemical burn-like reactions from excessive or overly frequent application of Spotclen Plus. If accidentally ingested, or if signs of a significant allergic reaction (such as swelling of the face/throat or difficulty breathing) occur, seek immediate medical attention or contact a poison control center/emergency services rather than attempting home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Pediatric use

Spotclen Plus is not recommended for use in children; safety and efficacy in the pediatric population have not been established.

Elderly

No specific dose adjustment is established for elderly patients; use with the same precautions regarding skin sensitivity and sun protection as in other adults.

Renal/hepatic impairment

No specific adjustment is required for topical use given minimal systemic absorption when used as directed.

Duration of treatment with Spotclen Plus should be determined by a physician based on response, typically evaluated after 4–8 weeks of use. Continuous unsupervised use beyond the period recommended by the physician or product labeling should be avoided due to the risk of exogenous ochronosis and other adverse skin effects.

Topical depigmenting (skin-lightening) agent with sunscreen/UV-filtering agents

Hydroquinone + Octyldimethyl P-aminobenzoate + Dioxybenzone + Oxybenzone lightens hyperpigmented skin through hydroquinone's inhibition of tyrosinase and reduction of melanin synthesis, while the co-formulated octyldimethyl PABA, dioxybenzone, and oxybenzone absorb ultraviolet radiation to reduce UV-triggered melanocyte stimulation and repigmentation of the treated area during therapy.

Spotclen Plus is not recommended for use in children, as safety and efficacy have not been established in this population. Use in pediatric patients should be avoided unless specifically directed by a physician.
