Torel is a topical rubefacient/counterirritant combination indicated for the temporary relief of minor aches and pain of muscles and joints associated with the following, based on long-standing OTC external analgesic monograph use and common clinical practice:

• Established (OTC monograph) uses: Simple backache, arthritis-related joint pain, muscle strains and sprains, and general muscular soreness.
• Commonly used for: Bruises, minor sports-related muscle fatigue, and stiffness/soreness after physical exertion.
• Adjunct use: As a topical adjunct alongside oral analgesics or physiotherapy for localized musculoskeletal pain, not as a substitute for systemic treatment of underlying disease.

This product is intended for external use only and provides symptomatic relief; it does not treat the underlying cause of pain.

Each application of Menthol + Camphor + Methyl Salicylate + Oleoresin Capsicum topical preparation typically contains approximately: Menthol 8-11% w/w, Camphor 8-11% w/w, Methyl Salicylate 10-15% w/w, and Oleoresin Capsicum (standardized to capsaicin content) approximately 0.025-0.1% w/w, in a suitable ointment, cream, gel, or balm base. Exact strengths vary by brand/dosage form; refer to the specific product label for the precise composition.

Torel is a fixed-dose combination topical counterirritant (rubefacient) preparation used for temporary relief of minor musculoskeletal pain. Menthol and camphor produce a cooling then warming sensation by acting on cutaneous sensory receptors (including TRPM8 and TRPV3 channels), methyl salicylate provides mild anti-inflammatory and counterirritant activity related to its salicylate structure, and oleoresin capsicum (containing capsaicin) acts on TRPV1 receptors to initially cause warmth/burning followed by a period of reduced pain sensitivity. Together these ingredients distract from and modulate the perception of deep musculoskeletal pain when applied to intact skin over the affected area.

Torel belongs to the therapeutic class of topical analgesics, rubefacients, and counterirritants (external analgesic combination products).

Menthol + Camphor + Methyl Salicylate + Oleoresin Capsicum works through several complementary mechanisms:

• Menthol activates cold-sensing TRPM8 receptors in the skin, producing a cooling sensation that counteracts pain signals via the gate-control mechanism.
• Camphor stimulates cutaneous nerve endings, producing local anesthetic and mild counterirritant effects, and enhances local blood flow.
• Methyl salicylate is absorbed through the skin and is thought to contribute mild local anti-inflammatory action through inhibition of local prostaglandin synthesis, in addition to a counterirritant/warming effect.
• Oleoresin capsicum (capsaicin) acts on TRPV1 receptors on sensory nerve fibers, initially causing a burning sensation followed by depletion of substance P from nerve terminals, which reduces transmission of pain signals with repeated use.

The combined effect is a counterirritant action that distracts the central nervous system from deeper pain and provides localized symptomatic relief.

For topical (external) use only. Clean and dry the skin before application. Apply a small amount and rub in gently until the product is absorbed. Avoid application to broken, irritated, or sunburned skin, and avoid the eyes, mouth, nose, and genital area. Do not cover the treated area with a tight bandage or apply external heat.

Formal drug-drug interaction studies with Torel are limited, as systemic absorption from topical use is generally low when used as directed. However, the following points are clinically relevant:

• Concomitant use with oral anticoagulants (e.g., warfarin) should be approached with caution, as methyl salicylate can be absorbed transdermally and, particularly with prolonged use over large areas, may theoretically enhance anticoagulant effect.
• Do not use together with other topical or oral salicylate-containing products on the same area, as this may increase the risk of salicylate toxicity.
• Avoid concurrent use of other topical counterirritants, heating devices, or occlusive dressings on the same site, which can increase absorption and risk of skin injury.

Most side effects of Torel are local and related to the application site:

• Common: Transient burning, stinging, warmth, or tingling sensation at the application site (expected pharmacological effect, especially from the capsaicin component).
• Less common: Local skin redness (erythema), itching, or mild irritation.
• Rare but serious: Allergic contact dermatitis, blistering, or chemical burns (risk increases significantly if used with heat sources or occlusive dressings). Systemic salicylate toxicity (salicylism) if used excessively over large areas or under occlusion, especially in children.

Discontinue use and seek medical advice if severe irritation, blistering, or signs of an allergic reaction occur.

Data on the use of Torel during pregnancy and lactation are limited. Systemic absorption of methyl salicylate through intact skin is generally low with normal use, but caution is warranted because salicylates can cross the placenta and are excreted in breast milk. This product should be used during pregnancy and breastfeeding only if clearly needed, in the smallest effective amount and for the shortest duration, and only after consulting a physician. Avoid use over large body surface areas during pregnancy and lactation.

Observe the following precautions when using Torel:

• For external use only; avoid contact with eyes, mucous membranes, and genital areas.
• Do not apply to broken, irritated, or sunburned skin (see Contraindications).
• Do not use with heating pads, hot water bottles, or occlusive/tight bandages, as this significantly increases the risk of severe skin burns.
• Wash hands thoroughly with soap and water immediately after application to avoid transferring the product to the eyes or other sensitive areas.
• Avoid applying over large areas of the body, as this increases systemic absorption of methyl salicylate and the risk of salicylate-related adverse effects, particularly relevant in children and the elderly.
• Discontinue use and consult a physician if excessive skin irritation, redness lasting more than a few days, or signs of infection develop.
• Keep out of reach of children; accidental ingestion can be dangerous (see Overdose).

Overdose of Torel may occur from excessive topical application (especially under occlusion or over large areas) or accidental oral ingestion, particularly in children. Signs of salicylate/camphor toxicity may include nausea, vomiting, ringing in the ears (tinnitus), rapid breathing, confusion, dizziness, or seizures. Accidental ingestion, especially of camphor-containing products, can be life-threatening in young children. If overdose or accidental ingestion is suspected, seek immediate medical attention or contact emergency services/a poison control center right away; do not wait for symptoms to appear.

Store at room temperature (below 30°C), away from light and moisture. Keep the container tightly closed and out of reach of children.

Children: Use of Torel in children under 12 years should only be under medical supervision, as children are at higher risk of systemic absorption and toxicity due to a higher skin surface-area-to-body-weight ratio; not recommended for children under 2 years.

Elderly: Generally well tolerated for external use, but elderly patients with thinner or more fragile skin may be more prone to local irritation; use the smallest effective amount.

Renal/hepatic impairment: No specific dose adjustment is established for topical use, but caution is advised with extensive application given potential systemic salicylate absorption.

Use Torel for the shortest duration needed to control symptoms, typically not more than 7 days for a given episode of pain. If pain persists, worsens, or recurs after treatment, or is accompanied by fever, redness, or swelling, discontinue use and consult a physician, as this may indicate a condition requiring further medical evaluation.

Safety and efficacy of Torel in children under 2 years have not been established, and use in this age group is not recommended. For children aged 2 to under 12 years, use only under the advice and supervision of a physician, using the smallest effective amount and avoiding large-area application, occlusive dressings, or heat sources, due to the increased risk of systemic salicylate absorption and skin irritation in this age group.
