Indications of Betadine 15 gm

Betadine 15 gm is a broad-spectrum topical antiseptic used across multiple dosage forms — cream, ointment, solution, gargle, surgical scrub, powder, and ophthalmic solution — to prevent and treat microbial infections of the skin, mucous membranes, and eyes.

Cream or Ointment

Used as an adjunct to systemic therapy for:

  • Primary or secondary topical infections
  • Infected surgical incisions
  • Infected decubitus (bedsore) or stasis ulcers
  • Pyodermas and secondarily infected dermatoses
  • Infected traumatic lesions

It is also used prophylactically to prevent microbial contamination in burns, incisions, and other topical lesions; to degerm the skin during hyperalimentation, around the umbilical area, or before circumcision; and to reduce the risk of infection in minor abrasions, cuts, and wounds while supporting healing.

Solution

  • Prevention and treatment of surface infections, and degerming of the skin, mucous membranes, and hyperalimentation sites

Gargle & Mouthwash

A pleasantly flavoured oral rinse used for the treatment of acute mucosal infections of the mouth and pharynx, and for maintaining oral hygiene before, during, and after dental or oral surgery.

Surgical Scrub

A golden surfactant solution that forms a rich lather, used as an antiseptic skin cleanser for preoperative and postoperative scrubbing by surgeons and operating room staff, and for preoperative preparation of the patient's skin.

Powder

Applied topically for superficial wounds, minor cuts, burns, abrasions, and lacerations, and for the treatment and prevention of infection in these areas.

Ophthalmic Solution

Used for symptomatic relief of dry eye conditions, including keratoconjunctivitis sicca, and as a substitute for tear fluid where the tear film is unstable or eye surface moistening is insufficient.

Pharmacology

Betadine 15 gm is a stable chemical complex of elemental iodine bound to the water-soluble polymer povidone (polyvinylpyrrolidone). This complexation acts as a reservoir that releases free iodine slowly and continuously, giving it a distinct advantage over plain elemental iodine: comparable antimicrobial potency with markedly lower tissue irritancy and toxicity.

Once released, free iodine penetrates microbial cell walls rapidly and oxidizes key cellular components — proteins, nucleotides, and fatty acids — disrupting cell structure and metabolism. This mechanism gives Betadine 15 gm a rapid, broad-spectrum microbicidal effect against Gram-positive and Gram-negative bacteria, protozoa, viruses, fungi, and yeasts. It is also sporicidal, a property few other topical antiseptics share, making it one of the few agents effective across essentially the entire spectrum of clinically relevant microorganisms.

Betadine 15 gm is non-staining to most fabric and skin after washing, provides prolonged germicidal activity following a single application, and importantly remains active even in the presence of organic material such as blood, serum, pus, and mucosal secretions — conditions that inactivate many other antiseptics.

Dosage & Administration of Betadine 15 gm

Betadine 15 gm dosing depends on the formulation and the clinical indication. General guidance for each dosage form is summarized below.

There are no specific dosage adjustments required for children or elderly patients using topical formulations, though age restrictions apply to certain forms (see Precautions and Special Populations). Interaction of Betadine 15 gm

Betadine 15 gm can chemically react with strong alkalis, sodium thiosulphate, sodium metabisulphite, and thiomersal, which may reduce its antiseptic activity — these substances should not be combined with it on the same application site.

Concurrent use with lithium therapy has been associated with an increased risk of hypothyroidism, since both substances can independently affect thyroid function. Patients on long-term lithium treatment should use Betadine 15 gm only under medical supervision and generally avoid regular or prolonged application.

Contraindications

  • Known hypersensitivity to iodine, povidone, or any component of the formulation
  • Patients with thyroid disorders — particularly nodular colloid goitre, endemic goitre, and Hashimoto's thyroiditis — should avoid regular use
  • Patients on concurrent lithium therapy should avoid regular use
  • Severely burnt patients require monitoring of serum iodide levels due to the risk of hepatic and renal impairment from systemic iodine absorption
  • Betadine 15 gm powder should not be used in deep body cavities or in children under 2 years of age

Side Effects of Betadine 15 gm

Betadine 15 gm is generally well tolerated with topical use, but the following effects have been reported:

  • Hypersensitivity reactions and local irritation of the skin or mucous membranes
  • When applied to severe burns or large areas of denuded skin, systemic absorption of iodine may cause metabolic acidosis, hypernatraemia, and impaired renal function
  • Interference with thyroid function test results due to iodine absorption

Pregnancy & Lactation

Betadine 15 gm cream, solution, and powder are not recommended during pregnancy, as sufficient iodine may be absorbed to affect fetal thyroid function. Regular use of the gargle/mouthwash and surgical scrub formulations should also be avoided during pregnancy and lactation.

For the ophthalmic solution, there is insufficient safety data in human pregnancy or lactation. Its use during these periods is therefore not recommended except where clearly necessary, and only under medical guidance.

Precautions & Warnings

Cream or Ointment

Consult a Registered Physician for deep or puncture wounds, or serious burns. Discontinue use if redness, irritation, swelling, or pain persists or worsens, or if infection develops.

Solution

During preoperative prepping, avoid pooling of the solution beneath the patient, as prolonged contact with wet solution may cause irritation or, rarely, severe skin reactions. Seek medical advice for deep wounds or serious burns.

Gargle & Mouthwash

Avoid regular, prolonged use, as this may lead to significant iodine absorption. Do not use for more than 14 consecutive days. Seek dental or medical advice if mouth sores or ulcers fail to heal within 14 days. Not recommended for children under 6 years, and regular use should be avoided in patients on lithium therapy.

Surgical Scrub

Use with caution for repeated application to broken skin in patients with pre-existing renal insufficiency. Avoid regular use in patients on lithium therapy. This formulation can permanently discolor white gold jewellery, which should be removed before use.

Powder

Use with care in patients with known iodine sensitivity, although true reactions to Betadine 15 gm are uncommon. Excess powder can be washed off with warm water. Apply directly from the container and do not administer internally.

Ophthalmic Solution

Patients who experience blurred vision after instillation should not drive or operate machinery until vision clears. Contact lenses must be removed before use, and reinsertion should be delayed for at least 30 minutes after instillation.

Overdose Effects of Betadine 15 gm

Excessive or prolonged use of Betadine 15 gm, particularly over large areas of broken skin, deep wounds, or severe burns, can lead to significant systemic absorption of iodine. This may present as metabolic acidosis, hypernatraemia, and impaired renal function, and can disturb thyroid function.

If accidental ingestion or signs of systemic iodine toxicity occur — such as burning in the mouth and throat, abdominal pain, or symptoms of thyroid dysfunction — seek immediate medical attention. Management is supportive and should be carried out under medical supervision; there is no specific antidote.

Use In Special Populations

  • Children: Betadine 15 gm powder should not be used in children under 2 years of age. The gargle/mouthwash formulation is not recommended for children under 6 years. No specific dose adjustment is needed for other topical forms in children, but regular or prolonged use should be avoided due to the risk of iodine absorption.
  • Elderly: No specific dosage adjustment is required for elderly patients using topical formulations.
  • Renal or Hepatic Impairment: Use with caution, particularly in severely burnt patients or those with pre-existing renal insufficiency undergoing repeated surgical scrub application, as systemic iodine absorption can affect renal and hepatic function.

Mode Of Action

Betadine 15 gm is a complex of iodine and an organic polymer, povidone. This polymerization makes Betadine 15 gm superior to ordinary elemental iodine. It prolongs the germicidal activity of iodine by liberating elemental iodine slowly. Consequently it has a lower toxicity than elemental iodine. It gives rapid microbicidal activity against both Gram-positive and Gram-negative bacteria, protozoa, viruses and fungi/yeasts. It is also sporicidal. It is the only microbicide with this broad spectrum of activity. It is non-staining, exerts prolong germicidal action and is also active in the presence of soap, blood, serum, pus, mucosal secretions and water.

Pregnancy

Betadine 15 gm Cream, Solution & Powder are not recommended for use during pregnancy because of the possibility of absorption of sufficient iodine to affect the fetal thyroid. American Academy of Pediatrics considers that the use of Povidone-Iodine is usually compatible with breast feeding. Consult Registered Physician. Regular use of Gargle & Mouthwash and Surgical Scrub should also be avoided in pregnant and lactating women.There is no experience regarding the safety of the Povidone eye drops in human pregnancy or lactation. Administration during pregnancy and lactation is therefore not recommended, except for compelling reasons.
