Ciprocort is an otic (ear) suspension indicated for the treatment of specific bacterial infections of the ear where inflammation is also present.

Established / FDA-approved use

• Acute otitis externa ("swimmer's ear") in adults and pediatric patients, caused by susceptible strains of Pseudomonas aeruginosa, Staphylococcus aureus, and Proteus mirabilis.

Ciprocort is not indicated for viral, fungal, or otherwise non-bacterial ear infections, and is not a substitute for systemic antibiotic therapy when deep or invasive infection is suspected.

Ciprofloxacin + Hydrocortisone Acetate otic suspension typically contains:

• Ciprofloxacin (as ciprofloxacin hydrochloride) 0.2%–0.3% w/v
• Hydrocortisone (as hydrocortisone acetate) 1% w/v

Exact strength varies slightly by manufacturer and marketed brand; always check the product label for the precise concentration dispensed.

Ciprocort is a fixed-dose otic combination of a fluoroquinolone antibiotic (ciprofloxacin) and a corticosteroid (hydrocortisone acetate). It is formulated as a sterile suspension for instillation into the ear canal, combining antibacterial activity against common ear-canal pathogens with local anti-inflammatory action to relieve the pain, redness, swelling, and itching that accompany bacterial otitis externa.

Otic antibiotic-corticosteroid combination (Ciprocort) – Fluoroquinolone antibiotic combined with a corticosteroid, for topical ear use.

Ciprofloxacin + Hydrocortisone Acetate combines two complementary mechanisms of action for local ear-canal therapy:

• Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes required for bacterial DNA replication, transcription, and repair. This action is bactericidal against susceptible Gram-positive and Gram-negative organisms, including Pseudomonas aeruginosa, Staphylococcus aureus, and Proteus mirabilis.
• Hydrocortisone acetate is a corticosteroid that suppresses local inflammatory mediators (reducing capillary permeability, leukocyte migration, and release of inflammatory cytokines), thereby reducing the pain, edema, erythema, and pruritus associated with otitis externa.

Following otic administration of Ciprofloxacin + Hydrocortisone Acetate, systemic absorption of both components is minimal, limiting systemic corticosteroid or antibiotic exposure.

Ciprocort is for otic (ear) use only. It must never be given by mouth, injected, or placed in the eyes. Warm the bottle before use, shake well, instill drops with the ear facing upward, and keep the position for 30–60 seconds after instillation. Because Ciprocort is applied topically to the ear canal with minimal systemic absorption, clinically significant systemic drug interactions are unlikely. However:

• Avoid concurrent use of other ototopical (ear) products unless directed by a physician, as this may alter local drug concentration or increase the risk of local irritation.
• Inform the physician of all other ear preparations, including other antibiotic or steroid ear drops, being used at the same time.

No clinically significant systemic drug-drug interactions have been well established for otic Ciprocort in prescribing information.

Most reported side effects of Ciprocort are local and mild to moderate in severity:

• Ear discomfort, pain, or itching at the application site
• Headache
• Ear canal redness or congestion
• Temporary changes in hearing or a feeling of ear fullness
• Rash or urticaria (uncommon)

Fungal overgrowth in the ear canal can occur with prolonged use of the antibiotic component; see Precautions and Warnings. Discontinue and seek medical advice if a rash or other sign of hypersensitivity develops.

Pregnancy: There are no adequate and well-controlled studies of Ciprocort in pregnant women. Because systemic absorption from otic use is minimal, risk to the fetus is expected to be low, but Ciprocort should be used during pregnancy only if clearly needed and the potential benefit justifies the potential risk; consult a physician before use.

Lactation: It is not known whether topically applied Ciprocort is excreted in human milk in clinically significant amounts. Systemically administered ciprofloxacin does pass into breast milk. Caution is advised, and a physician should be consulted before use while breastfeeding.

Ciprocort is for otic use only — it must not be used in the eyes or given by injection or by mouth.

• Discontinue Ciprocort at the first sign of a rash or any other sign of hypersensitivity and consult a physician.
• Use of the specific product depends on whether the tympanic membrane is intact or perforated (including in patients with tympanostomy tubes); follow the exact labeling of the dispensed brand and the physician's instructions regarding use with a perforated eardrum or ear tubes.
• Prolonged or repeated use of the corticosteroid component may lead to local effects such as skin/ear-canal thinning or delayed healing; use only for the prescribed duration.
• As with other antibiotic preparations, prolonged use of Ciprocort may result in overgrowth of non-susceptible organisms, including fungi; if infection worsens or does not improve, seek medical re-evaluation and possible culture.
• The dropper cap of some brands may contain natural rubber (latex), which may cause allergic reactions in latex-sensitive individuals.
• Take Ciprocort exactly as prescribed by your physician; do not stop, extend, skip doses, or share this medicine with others without medical advice.

Overdose with otic Ciprocort is unlikely to cause serious systemic toxicity because absorption from the ear canal is minimal. If a much larger amount than prescribed is accidentally instilled, or if the suspension is accidentally swallowed or gets into the eyes, flush the affected area with water and seek immediate medical attention or contact a poison control center. Do not attempt to manage a suspected overdose at home without professional guidance.

Store at room temperature (below 30°C), protected from light and moisture. Do not freeze. Shake well before each use. Discard any unused suspension after completing the prescribed course. Keep out of reach of children.

Pediatric patients: Ciprocort is approved for use in children from the age specified on the dispensed product's label (commonly 1 to 2 years of age and older, depending on the brand); safety and efficacy in younger infants have not been established.

Elderly: No specific dose adjustment is required in elderly patients for otic use of Ciprocort.

Renal/hepatic impairment: Because systemic absorption after otic administration is minimal, dose adjustment for renal or hepatic impairment is not typically required for Ciprocort; consult a physician if impairment is significant.

Perforated tympanic membrane / tympanostomy tubes: Suitability of a specific Ciprocort brand for use with a perforated eardrum or ear tubes depends on the individual product's approved labeling; follow the physician's guidance and the specific package insert.

Ciprocort is typically used for 7 days for acute otitis externa, or as otherwise directed by the physician. Treatment should be completed in full even if symptoms improve earlier, and should not be continued beyond the prescribed duration without medical review, given the risk of local corticosteroid effects and fungal overgrowth with prolonged antibiotic use.

Fluoroquinolone antibiotic + Corticosteroid (otic combination) – Ciprofloxacin + Hydrocortisone Acetate

Ciprofloxacin + Hydrocortisone Acetate acts through two mechanisms at the site of infection: ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, blocking bacterial DNA replication and causing bactericidal killing of susceptible organisms; hydrocortisone acetate, a corticosteroid, reduces local inflammation by suppressing the release of inflammatory mediators, decreasing capillary permeability and leukocyte infiltration. Together these actions clear the causative bacterial infection while relieving the associated ear pain, swelling, and itching.

Ciprocort is approved for use in pediatric patients from the age stated on the specific product's label (commonly 1 year of age and older for some formulations, 2 years and older for others) for acute otitis externa due to susceptible bacteria. The dosing regimen (drops and frequency) is the same as for adults unless the physician directs otherwise. Safety and efficacy of Ciprocort in infants below the approved age range have not been established, and it should not be used in this population without specific medical advice.
