Indications of Expecto 4 mg/5 ml

Expecto 4 mg/5 ml is a mucolytic agent indicated for the management of respiratory tract disorders associated with excessive or viscous mucus and productive (chesty) cough. By thinning and loosening thick bronchial secretions, it helps clear the airways and makes coughing more effective. It is commonly prescribed for the following conditions:

  • Tracheobronchitis – inflammation of the trachea and bronchi with mucus build-up
  • Bronchitis with emphysema – chronic bronchitis accompanied by damaged air sacs
  • Bronchiectasis – abnormal widening of the airways with recurrent mucus accumulation
  • Bronchitis with bronchospasm – airway inflammation with narrowing of the bronchial tubes
  • Chronic inflammatory pulmonary conditions – long-standing lung inflammation producing sticky sputum
  • Pneumoconiosis – occupational lung disease caused by inhaled dust particles

Pharmacology

Expecto 4 mg/5 ml is an orally active mucolytic agent that promotes the clearance of mucus from the respiratory tract. It works by depolymerising and breaking down the acid mucopolysaccharide fibres within mucoid sputum, thereby reducing the viscosity (thickness) of the mucus. This produces thinner, less sticky secretions that are far easier to expectorate (cough up), improving airway clearance and easing breathing.

Following oral administration, Bromhexine is rapidly and almost completely absorbed from the gastrointestinal tract, with plasma concentrations reached within about one hour. It is extensively distributed throughout body tissues and undergoes significant first-pass metabolism in the liver, where it is partly converted to the active metabolite ambroxol. The drug and its metabolites are primarily eliminated through the kidneys via urine.

Dosage & Administration of Expecto 4 mg/5 ml

The recommended doses of Expecto 4 mg/5 ml for adults and children are outlined below. It is best taken after meals to minimise gastric irritation, and patients are encouraged to maintain adequate fluid intake to support the mucus-clearing effect.

Note: Expecto 4 mg/5 ml is not recommended for children under 2 years of age without medical advice. For tablet formulations, adults are generally given 8 mg three times daily (equivalent to 10 ml of the 4 mg/5 ml syrup). Do not exceed the prescribed dose or duration of treatment.

Interaction of Expecto 4 mg/5 ml

There are no known clinically significant interactions between Expecto 4 mg/5 ml and other medicines. However, because Bromhexine liquefies bronchial secretions, combining it with antitussives (cough suppressants) is generally discouraged, as suppressing the cough reflex may cause loosened mucus to accumulate in the airways. Bromhexine may also enhance the concentration of certain antibiotics (such as amoxicillin and erythromycin) in bronchopulmonary secretions. To be safe, inform your physician about all prescription drugs, over-the-counter products and herbal supplements you are taking.

Contraindications

Expecto 4 mg/5 ml is contraindicated in patients with known hypersensitivity or allergy to Bromhexine or to any of the excipients contained in the formulation. It should not be used where a previous allergic reaction to the drug has occurred.

Expecto 4 mg/5 ml is generally well tolerated at recommended doses. When adverse effects do occur, they are usually mild and transient. Reported side effects include:

  • Gastrointestinal disturbances such as nausea, indigestion or stomach upset (occasional)
  • A transient rise in serum aminotransferase (liver enzyme) values
  • Headache
  • Vertigo or dizziness
  • Allergic reactions, including skin rash

Rarely, severe hypersensitivity or skin reactions may occur. If you experience swelling of the face, lips or throat, difficulty breathing, or a spreading skin rash, stop the medication and seek immediate medical attention.

Pregnancy & Lactation

Use in Pregnancy: Special care is recommended when Expecto 4 mg/5 ml is used during pregnancy, particularly in the first trimester. The potential benefits to the mother must be carefully weighed against any possible risk to the developing child. It should only be used during pregnancy when clearly needed and under the supervision of a physician.

Use in Lactation: Bromhexine is expected to pass into breast milk. For this reason, its use is not recommended in breastfeeding mothers unless the benefit is judged to outweigh the potential risk to the infant, and only on medical advice.

Precautions & Warnings

Because mucolytic agents may disrupt the protective gastric mucosal barrier, Expecto 4 mg/5 ml should be used with caution in patients with a current or previous history of peptic ulcer disease. Additional care is advised in the following situations:

  • Asthmatic patients – mucolytic therapy should be monitored, as loosened secretions may initially increase airway irritation.
  • Hepatic impairment – clearance of Bromhexine and its metabolites may be reduced in patients with severe liver dysfunction.
  • Renal impairment – reduced kidney function may impair elimination of the drug and its metabolites; dose adjustment may be required.

Patients should consult a doctor if the cough persists beyond a few days, worsens, or is accompanied by high fever, chest pain or difficulty breathing.

Storage Conditions

Store Expecto 4 mg/5 ml in a cool, dry place, protected from direct light and heat. Keep the bottle tightly closed when not in use. Keep this and all medicines out of the reach and sight of children. Do not use after the expiry date printed on the pack.
