Composition

Dosage of Nospot 0.01%+4%+0.05%

Apply a thin film of cream to the hyperpigmented areas of melasma, including about half an inch (1-1.5 cm) of normal-appearing skin surrounding each patch, once daily at least 30 minutes before bedtime. Gently wash the face and neck with a mild cleanser, rinse, and pat dry before application. Treatment is typically continued for a defined course of about 8 weeks, as directed by the prescribing doctor, and should be discontinued once adequate control of the melasma is achieved - it is not intended for ongoing maintenance use. Interaction of Nospot 0.01%+4%+0.05%

The following should be considered when using this combination:

  • Other topical products applied to the same skin area (such as other retinoids, exfoliants, medicated soaps, or products containing benzoyl peroxide): can increase skin irritation and dryness when combined; avoid using additional irritant skin products on treated areas.
  • Photosensitizing medicines (such as certain antibiotics like tetracyclines, or other photosensitizing drugs): tretinoin already increases skin sensitivity to sunlight, and combining with other photosensitizing medicines may further increase this risk.
  • Other topical corticosteroids used on the same or nearby skin areas: may increase the total corticosteroid exposure and associated risk of skin thinning or systemic absorption.

Contraindications

  • Known history of hypersensitivity to this product or any of its components (fluocinolone acetonide, hydroquinone, tretinoin, or other ingredients including sodium metabisulfite)

Common side effects

  • Signs of hypothalamic-pituitary-adrenal axis suppression from systemic corticosteroid absorption (rare, mainly with extensive or prolonged use)
  • Signs of a severe allergic reaction, including asthmatic symptoms in sulfite-sensitive patients: wheezing, difficulty breathing, widespread rash

Pregnancy & Lactation

Pregnancy

Tretinoin is considered highly teratogenic when given systemically, and although absorption from topical use is generally low, adequate reproductive safety data for this specific combination are not available; corticosteroids have also shown teratogenic effects in animal studies with systemic and, in some cases, dermal application. This product should not be used during pregnancy unless a doctor determines it is clearly necessary, and melasma commonly improves after delivery in any case, which may support deferring treatment.

Breastfeeding

It is not known whether topical application of this combination results in sufficient systemic absorption to produce detectable amounts of any of its three active ingredients in human milk.
