Clomiphene citrate is used for the treatment of ovulatory dysfunction in women desiring pregnancy who have demonstrated adequate endogenous oestrogen levels. It is considered a first-line treatment for anovulatory infertility, including in women with polycystic ovary syndrome (PCOS), and is used to induce ovulation in normally oestrogenized, anovulatory women. It is also used off-label to induce spermatogenesis in certain cases of male infertility, under specialist guidance.

Clomiphene citrate is an oral fertility medicine used to stimulate ovulation in women who are not ovulating regularly, most commonly due to polycystic ovary syndrome or other causes of ovulatory dysfunction. It works by blocking oestrogen's usual feedback signal to the brain, which tricks the body into producing more of the hormones that stimulate the ovary to develop and release an egg. Clomiphene citrate is capable of interacting with oestrogen receptor-containing tissues, including the hypothalamus, pituitary, ovary, endometrium, vagina, and cervix, acting as an oestrogen antagonist in some tissues while retaining some weak oestrogenic activity in others. Clomiphene citrate is taken orally, once daily, for 5 days per treatment cycle, typically starting on day 5 of the menstrual cycle (whether spontaneous or induced by prior treatment). Dosing starts at the lowest effective level and may be increased in subsequent cycles if ovulation does not occur.
The starting dose is 50 mg daily for 5 days, beginning on day 5 of the cycle. If ovulation does not occur, the dose may be increased to 100 mg daily for 5 days in the next cycle. Treatment is generally not continued beyond 100 mg/day, and courses are typically limited to a maximum of 6 cycles, with reassessment if 3 ovulatory cycles occur without pregnancy or if ovulation does not occur after 3 courses.
A lower dose, commonly 25 mg daily, for an extended period (such as 25 days on, 5 days off), as directed by the treating specialist.
Patients must be evaluated to exclude pregnancy, ovarian enlargement, or ovarian cyst formation between each treatment cycle before starting a further course.
