Indications of Risperdex 2 mg

Risperdex 2 mg is an atypical antipsychotic medicine prescribed for the management of a range of psychiatric conditions. It works by restoring the balance of natural chemical messengers (neurotransmitters) in the brain, helping to reduce disturbed thoughts, mood swings, and behavioural symptoms. Risperdex 2 mg is indicated for the treatment of:

  • Acute and chronic psychoses — conditions marked by a loss of contact with reality, including hallucinations and delusions.
  • Mania — episodes of elevated mood, overactivity, and irritability, commonly seen in bipolar disorder.
  • Schizophrenia — a long-term mental health disorder affecting thoughts, emotions, and behaviour, including both positive symptoms (such as hallucinations) and negative symptoms (such as social withdrawal).

Theropeutic Class

Risperdex 2 mg belongs to the class of atypical neuroleptic (antipsychotic) drugs.

Pharmacology

Risperdex 2 mg is a selective monoaminergic antagonist with a high affinity for serotonergic 5-HT2 and dopaminergic D2 receptors. It also binds to alpha1-adrenergic receptors and, with lower affinity, to H1 histaminergic and alpha2-adrenergic receptors. Risperdex 2 mg has no affinity for cholinergic receptors.

As a potent D2 antagonist, Risperdex 2 mg improves the positive symptoms of schizophrenia (such as hallucinations and delusions) while causing comparatively less suppression of motor activity than conventional antipsychotics. Its balanced antagonism of central serotonin and dopamine is thought to reduce the likelihood of extrapyramidal side effects and to extend therapeutic benefit to the negative and affective symptoms of schizophrenia, such as blunted emotion, social withdrawal, and depressed mood.

Dosage & Administration of Risperdex 2 mg

The dose of Risperdex 2 mg should be individualised according to the patient's condition, age, and tolerance. The following are general guidelines; the exact regimen must be determined by a qualified physician.

Psychoses

  • Start with 2 mg daily in 1–2 divided doses on day 1, then 4 mg in 1–2 divided doses on day 2. Slower titration is appropriate in some patients.
  • Usual dose range: 4–6 mg daily. Doses above 10 mg daily should be used only when the expected benefit outweighs the risk (maximum 16 mg daily).
  • Elderly patients, or those with hepatic or renal impairment: initially 1 mg daily in two divided doses, increased in steps of 1–2 mg twice daily.
  • Not recommended for children under 15 years.

Mania

  • Initially 2 mg once daily, increased if necessary in steps of 1 mg daily. Usual dose range: 1–6 mg daily.
  • Elderly patients, or those with hepatic or renal impairment: initially 1 mg daily in two divided doses, increased in steps of 1–2 mg twice daily.

Schizophrenia

  • Generally start at 1 mg twice daily (BID), increasing in increments of 1 mg BID on the second and third days, as tolerated, to a target dose of 3 mg BID by the third day.
  • Further dose adjustments, when indicated, should generally occur at intervals of not less than 1 week. Small increments or decrements of 1–2 mg are recommended.

Interaction of Risperdex 2 mg

Risperdex 2 mg may interact with several other medicines, altering their effects or its own. Notable interactions include:

  • Levodopa and dopamine agonists: Risperdex 2 mg may antagonise (reduce) their effects.
  • Carbamazepine: Long-term (chronic) co-administration reduces the plasma clearance of Risperdex 2 mg, which may affect its levels.
  • Clozapine: Chronic co-administration may decrease the clearance of Risperdex 2 mg.
  • Antihypertensives: Risperdex 2 mg may enhance the blood-pressure-lowering effects of certain antihypertensive medicines.

Contraindications

  • Hyperprolactinaemia (raised prolactin levels) and urinary incontinence
  • Tachycardia (rapid heartbeat), hypertension (raised blood pressure), and oedema (swelling)
  • Skin rash and rhinitis (runny or blocked nose)
  • Cerebrovascular accident (stroke)
  • Neutropenia and thrombocytopenia (reductions in certain blood cells)

Pregnancy & Lactation

Pregnancy: In experimental animals, Risperdex 2 mg did not show direct reproductive toxicity, though some indirect, prolactin- and CNS-mediated effects were observed. No teratogenic (birth-defect-causing) effect was noted in any study. However, the safety of Risperdex 2 mg during human pregnancy has not been established, so it should be used only if the potential benefit justifies the potential risk to the foetus.

Lactation: In animal studies, Risperdex 2 mg and its active metabolite 9-hydroxyRisperdex 2 mg are excreted in milk, and both have also been demonstrated to be excreted in human breast milk. Therefore, women receiving Risperdex 2 mg should not breastfeed.

Precautions & Warnings

Special caution is required when using Risperdex 2 mg in the following situations:

  • Pre-existing cardiovascular (heart and blood vessel) diseases.
  • Discontinue use if signs and symptoms of tardive dyskinesia (involuntary, repetitive body movements) occur.
  • Renal (kidney) and hepatic (liver) impairment.
  • Elderly patients.

  • Parkinson's disease.
  • Pregnancy.

Patients and caregivers should be advised to report any unusual movements, mood changes, or other concerning symptoms to a physician promptly.

Overdose Effects of Risperdex 2 mg

Drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms may occur. In case of severe extrapyramidal symptoms, anticholinergic medication should be administered. Close medical supervision and monitoring should continue until the patient recovers. There is no specific antidote to Risperdex 2 mg.

Storage Conditions

Do not store above 30°C. Keep Risperdex 2 mg away from light and out of the reach of children. Do not use the medicine after its expiry date.
