About Atorvastatin + Ezetimibe

Indications of Atorvastatin + Ezetimibe

Atorvastatin + Ezetimibe combines a statin (atorvastatin) with a cholesterol absorption inhibitor (ezetimibe) and is used for the following indications:

Established / Guideline-Supported Uses

  • As an adjunct to diet, for treatment of primary hyperlipidemia (heterozygous familial and non-familial) or mixed dyslipidemia, to reduce elevated total cholesterol, LDL-cholesterol, apolipoprotein B, triglycerides, and non-HDL-cholesterol, in patients whose response to statin monotherapy is inadequate, or who are already being treated with both agents.
  • To reduce the risk of major cardiovascular events (myocardial infarction, stroke, revascularization) in patients with established atherosclerotic cardiovascular disease or at high cardiovascular risk, as part of an overall lipid-lowering strategy.

Adjunct / Combination-Only Use

  • As adjunctive therapy to other lipid-lowering treatments (e.g. LDL apheresis) in homozygous familial hypercholesterolemia, when a greater degree of LDL-cholesterol reduction is needed than diet and a statin alone provide.

Atorvastatin + Ezetimibe is not indicated for patients whose predominant abnormality is elevated chylomicrons (elevated triglycerides only, e.g. Fredrickson Type I, III, IV, and V dyslipidemia).

Dosage & Administration of Atorvastatin + Ezetimibe

Atorvastatin + Ezetimibe is taken orally, once daily, generally in the evening, with or without food. The dose is individualized based on baseline LDL-cholesterol level, treatment goal, and current statin/ezetimibe therapy, and should be determined by the prescribing physician.

Dosing by Indication

IndicationTypical Adult Dose
Primary hyperlipidemia / mixed dyslipidemiaStarting dose commonly atorvastatin 10 mg/ezetimibe 10 mg once daily; may be adjusted up to atorvastatin 80 mg/ezetimibe 10 mg once daily depending on response and goal, at intervals of no less than 2 weeks between titrations.
Patients requiring >55% LDL-C reductionMay be started at a higher combined strength (e.g. atorvastatin 40 mg/ezetimibe 10 mg) under physician supervision.
Homozygous familial hypercholesterolemia (adjunct)Atorvastatin 10–80 mg with ezetimibe 10 mg once daily, as adjunct to other lipid-lowering treatments (e.g. LDL apheresis), under specialist supervision.
Concomitant strong CYP3A4 inhibitors or specific interacting drugsLower atorvastatin-component doses or avoidance may be required (see Interactions).

Renal Impairment

No dose adjustment of the atorvastatin component is generally required for renal impairment; ezetimibe also generally requires no adjustment in mild-to-moderate renal impairment, but Atorvastatin + Ezetimibe should be used with caution and under medical supervision in patients with significant renal disease.

Hepatic Impairment

Atorvastatin + Ezetimibe is contraindicated in active liver disease (see Contraindications). Liver function should be assessed before starting and monitored during treatment.

Side Effects of Atorvastatin + Ezetimibe

Most patients tolerate Atorvastatin + Ezetimibe well; when side effects occur, they are usually mild. Reported side effects include:

Common

  • Muscle aches/pain (myalgia)
  • Diarrhea and other GI upset (more frequent with the ezetimibe component)
  • Nasopharyngitis, joint pain (arthralgia)
  • Elevated liver transaminases
  • Headache

Less Common / Serious

  • Myopathy, and rarely rhabdomyolysis (severe muscle breakdown with risk of kidney injury) — seek urgent medical attention for unexplained muscle pain, tenderness, or weakness, especially with fever or dark urine.
  • Hepatotoxicity, including rare reports of liver failure.
  • Hypersensitivity reactions, including rare angioedema.
  • Rare reports of thrombocytopenia, pancreatitis, and gallstones/cholecystitis (associated with ezetimibe).

Contraindications of Atorvastatin + Ezetimibe

Atorvastatin + Ezetimibe is contraindicated in the following situations:

  • Known hypersensitivity to atorvastatin, ezetimibe, or any component of the formulation.
  • Active liver disease, or unexplained persistent elevations of serum transaminases.
  • Pregnancy — statins can cause fetal harm and are contraindicated in pregnant women.
  • Breastfeeding — contraindicated due to potential for serious adverse effects in nursing infants.
  • Concomitant use with certain strong CYP3A4 inhibitors (e.g. certain HIV protease inhibitor combinations, glecaprevir/pibrentasvir) where combined use with atorvastatin is specifically contraindicated per product labeling.

Frequently Asked Questions about Atorvastatin + Ezetimibe

What is Atorvastatin + Ezetimibe used for?

Atorvastatin + Ezetimibe is used to lower high cholesterol and treat mixed dyslipidemia, as an adjunct to diet, especially when a statin alone has not adequately controlled cholesterol, and to help reduce the risk of heart attack and stroke in people with or at high risk of cardiovascular disease.

How should I take Atorvastatin + Ezetimibe?

Atorvastatin + Ezetimibe is usually taken once daily, generally in the evening, with or without food, exactly as prescribed by your physician. Swallow the tablet whole with water. Do not change the dose or stop taking it without consulting your doctor.

Can Atorvastatin + Ezetimibe be used during pregnancy or breastfeeding?

No. Atorvastatin + Ezetimibe is contraindicated in pregnancy and during breastfeeding because the statin component can harm the developing fetus and may pass into breast milk. If you become pregnant while taking Atorvastatin + Ezetimibe, stop the medicine and contact your doctor immediately.

What side effects should I watch for with Atorvastatin + Ezetimibe?

Common side effects of Atorvastatin + Ezetimibe include muscle aches, diarrhea, joint pain, and mildly elevated liver enzymes. Seek urgent medical attention for unexplained muscle pain, tenderness or weakness (possible sign of myopathy or rhabdomyolysis), or signs of liver problems such as yellowing of the skin/eyes, dark urine, or unusual fatigue.

Can I drink grapefruit juice or alcohol while taking Atorvastatin + Ezetimibe?

Large amounts of grapefruit juice (more than about 1.2 liters a day) can increase levels of the atorvastatin component of Atorvastatin + Ezetimibe and should be avoided. Alcohol should be used in moderation, as heavy alcohol use increases the risk of liver problems while taking Atorvastatin + Ezetimibe.

What should I do if I miss a dose or take too much Atorvastatin + Ezetimibe?

If you miss a dose of Atorvastatin + Ezetimibe, take it as soon as you remember unless it is almost time for the next dose — in that case, skip the missed dose and do not double up. If you suspect an overdose, seek immediate medical attention or contact a poison control center right away.
