About Betrixaban

Indications of Betrixaban

Betrixaban is a direct-acting oral factor Xa inhibitor (anticoagulant). Its indications, classified by strength of evidence, are as follows:

Established / FDA-approved use

  • Prophylaxis of venous thromboembolism (VTE) in adult patients hospitalized for an acute medical illness who are at risk of thromboembolic complications because of moderate or severe restricted mobility and other risk factors for VTE, given for an extended duration (typically 35 to 42 days) that continues beyond the usual hospital stay.

Not established / off-label

Betrixaban is not approved and does not have guideline-supported evidence for stroke prevention in non-valvular atrial fibrillation, treatment of acute deep vein thrombosis (DVT) or pulmonary embolism (PE), use after hip or knee replacement surgery, or use in patients with mechanical/prosthetic heart valves. Use of Betrixaban for any of these purposes would be off-label and is not recommended outside specialist clinical judgement.

Dosage & Administration of Betrixaban

Dosing of Betrixaban is indication-specific and must be individualized based on renal function and concomitant medications.

Indication / PopulationRecommended Dosage
Standard VTE prophylaxis in acutely ill hospitalized adultsInitial single dose of 160 mg on Day 1, then 80 mg once daily, for 35 to 42 days total, started on the day of hospital admission.
Severe renal impairment (CrCl 15-30 mL/min)Initial single dose of 80 mg on Day 1, then 40 mg once daily.
Concomitant use with a P-glycoprotein (P-gp) inhibitorInitial single dose of 80 mg on Day 1, then 40 mg once daily (see Interactions).
Severe hepatic impairmentUse is contraindicated (see Contraindications).

See Dosage and Administration sections below for additional details, and Use in Special Populations for renal/hepatic guidance.

Frequently Asked Questions about Betrixaban

What is Betrixaban used for?

Betrixaban is used to prevent venous thromboembolism (blood clots) in adults who are hospitalized for an acute medical illness and have restricted mobility or other clot risk factors, continued for an extended period after admission.

How should I take Betrixaban?

Betrixaban should be taken once daily with food, at about the same time each day, exactly as prescribed, usually starting with a higher first dose followed by a lower daily dose, for the full course of 35 to 42 days.

What are the main side effects of Betrixaban?

The most common side effect of Betrixaban is bleeding, which may range from minor bruising to serious internal bleeding. Nausea and anemia have also been reported. Seek urgent medical care for signs of serious bleeding.

Can Betrixaban be taken during pregnancy or breastfeeding?

Betrixaban should be used in pregnancy only if clearly necessary, as data are limited and the potential benefit must outweigh potential risk to the fetus. It is not known if Betrixaban passes into breast milk, so a physician should help decide whether to continue breastfeeding or continue the medicine.

Who should not take Betrixaban?

Betrixaban should not be used by people with active abnormal bleeding, severe liver impairment, known hypersensitivity to Betrixaban, or those with severe kidney impairment who are also taking a strong P-glycoprotein inhibitor medicine.

What happens if I stop taking Betrixaban early or miss a dose?

Stopping Betrixaban before completing the prescribed course increases the risk of blood clots forming. If a dose is missed or early discontinuation is being considered, contact a physician promptly rather than stopping on your own.
