About Bimatoprost + Timolol

Indications of Bimatoprost + Timolol

Bimatoprost + Timolol is a fixed-dose combination ophthalmic solution (bimatoprost, a prostaglandin analog, plus timolol, a non-selective beta-blocker) indicated for the following:

Established/FDA-approved use

  • Reduction of elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension who require additional IOP lowering and are insufficiently responsive to a topical beta-blocker or another single IOP-lowering agent used alone.

Place in therapy

Bimatoprost + Timolol is intended for patients who need more IOP reduction than a single agent (either bimatoprost alone or timolol alone) provides; it is not typically used as first-line monotherapy when a single agent would be adequate, since combination therapy carries a broader side-effect profile from both components.

Dosage & Administration of Bimatoprost + Timolol

Adult Dose

One drop of Bimatoprost + Timolol in the affected eye(s) once daily, in the morning. Do not exceed the recommended once-daily dosing frequency; more frequent dosing may actually reduce the IOP-lowering effect of the bimatoprost component.

Pediatric Dose

Safety and efficacy of Bimatoprost + Timolol have not been established in pediatric patients; it is not recommended for use in children (see Use in Special Populations).

Renal/Hepatic Impairment

No specific dose adjustment of Bimatoprost + Timolol has been established for renal or hepatic impairment; use with appropriate caution regarding the systemic beta-blocker effects of the timolol component in patients with significant organ impairment, under physician guidance.

Administration Technique

  • Instill one drop of Bimatoprost + Timolol into the conjunctival sac of the affected eye(s).
  • Apply gentle pressure over the tear duct (nasolacrimal occlusion) or close the eyelids gently for about 2 minutes after instillation to reduce systemic drug absorption.
  • If using other topical ophthalmic products, space administration by at least 5 minutes between products.
  • Remove contact lenses before instillation; lenses may be reinserted after the interval stated on the product labeling (commonly around 15 minutes), since the solution contains a preservative that can be absorbed by soft contact lenses.
  • If a dose is missed, continue with the next scheduled dose the following day; do not instill an extra drop to make up for a missed dose.

Side Effects of Bimatoprost + Timolol

Common Side Effects

  • Conjunctival hyperemia (eye redness) - the most commonly reported effect
  • Growth of eyelashes (increased length, thickness, number, and pigmentation)
  • Eye pruritus (itching)
  • Ocular irritation, burning, or stinging on instillation
  • Dry eye
  • Increased iris pigmentation (see Precautions and Warnings)
  • Headache

Less Common but Notable

  • Periorbital and eyelid skin pigmentation changes (darkening)
  • Blurred vision, tearing, or photophobia
  • Systemic effects from the timolol component (uncommon with topical use but possible): bradycardia, hypotension, fatigue, bronchospasm/dyspnea in susceptible patients
  • Superficial punctate keratitis

Contraindications of Bimatoprost + Timolol

Bimatoprost + Timolol is contraindicated in patients with:

  • Reactive airway disease, including bronchial asthma, a history of bronchial asthma, or severe chronic obstructive pulmonary disease - the systemic beta-blockade from the timolol component can precipitate bronchospasm even with topical ophthalmic use.
  • Sinus bradycardia.
  • Second- or third-degree atrioventricular (AV) block.
  • Overt cardiac failure.
  • Cardiogenic shock.
  • Known hypersensitivity to bimatoprost, timolol, or any other component of Bimatoprost + Timolol.

Frequently Asked Questions about Bimatoprost + Timolol

What is Bimatoprost + Timolol used for?

Bimatoprost + Timolol is an eye drop that combines two medicines to lower elevated eye pressure in people with open-angle glaucoma or ocular hypertension who need more pressure reduction than one medicine alone provides.

How should I use Bimatoprost + Timolol?

Instill one drop of Bimatoprost + Timolol into the affected eye(s) once daily in the morning, exactly as prescribed. Do not use it more than once a day, as this can actually reduce its effectiveness. Gently press on the inner corner of the eye near the nose, or close your eyes, for about 2 minutes after each dose to reduce absorption into the body.

Can Bimatoprost + Timolol change the color of my eyes?

Yes. Bimatoprost + Timolol can gradually increase brown pigmentation in the iris (the colored part of the eye), and this change may be permanent even after stopping the medicine. It can also cause eyelashes to grow longer, thicker, and darker, and can darken the skin around the eyes; these skin and lash changes are usually reversible. Tell your doctor if you notice any of these changes.

Is Bimatoprost + Timolol safe for people with asthma or heart problems?

No, not for many. Bimatoprost + Timolol contains timolol, a beta-blocker, and is contraindicated in people with asthma, a history of asthma, severe COPD, slow heart rate (sinus bradycardia), certain heart block conditions, overt heart failure, or cardiogenic shock, because enough of the medicine can be absorbed into the bloodstream to affect the heart and lungs even though it is used in the eye. Tell your doctor about any history of asthma, lung disease, or heart problems before starting.

Can Bimatoprost + Timolol be used during pregnancy or breastfeeding?

Data on Bimatoprost + Timolol in pregnancy are limited, so it should be used only if the potential benefit clearly outweighs the potential risk to the fetus, under a physician's supervision. Timolol passes into breast milk, so breastfeeding mothers should discuss the risks and benefits with their doctor, who may recommend discontinuing breastfeeding or the medicine.

What should I do if I accidentally use too much Bimatoprost + Timolol or someone swallows it?

If too much Bimatoprost + Timolol is used in the eye or the solution is accidentally swallowed, symptoms such as a slow heartbeat, low blood pressure, or difficulty breathing could occur from the beta-blocker component. Seek immediate medical attention or contact emergency services/a poison control center right away rather than trying to manage it at home.
