About Flunarizine

Indications of Flunarizine

Flunarizine is a selective calcium channel blocker with well-established (guideline-supported) uses in:

  • Prophylaxis of migraine (with or without aura) in patients with frequent and/or severe attacks who have not responded adequately to other measures. It is not intended for treating an acute migraine attack.
  • Symptomatic treatment of vertigo of vestibular origin (e.g., Meniere's disease-related vertigo, vestibular dysfunction).

It has also been used, with more limited or regional evidence, for motion sickness prophylaxis and as adjunctive therapy in peripheral vascular disease and refractory epilepsy; such uses should follow specific prescriber guidance.

Dosage & Administration of Flunarizine

Dosage of Flunarizine should be individualized by the prescriber. It is typically given as a single dose at night due to its sedative effect. Treatment is generally given in defined cycles rather than continuously (see Duration of Treatment).

  • Migraine prophylaxis (adults under 65 years): Usually 10 mg once daily at bedtime.
  • Migraine prophylaxis (adults 65 years and older): Usually 5 mg once daily at bedtime, due to higher sensitivity to side effects.
  • Vertigo of vestibular origin: Typically 10 mg once daily at bedtime, adjusted per response.

Some regimens include two medicine-free days per week during the maintenance phase; this should follow the prescriber's specific instructions.

Side Effects of Flunarizine

Common side effects of Flunarizine include:

  • Drowsiness/sedation and fatigue, particularly at the start of treatment.
  • Increased appetite and weight gain.
  • Gastrointestinal upset (nausea, heartburn, dry mouth).
  • Headache, insomnia, or anxiety in some patients.

Less common but clinically important effects include depression and extrapyramidal symptoms (tremor, rigidity, bradykinesia, akathisia, orofacial dyskinesia) — see Precautions and Warnings and Duration of Treatment for details, as these are linked to prolonged use and are more frequent in elderly patients. Rarely, galactorrhea, muscle aches, or skin rash may occur.

Contraindications of Flunarizine

Flunarizine is contraindicated in patients with:

  • Known hypersensitivity to Flunarizine or any component of the formulation.
  • A history of depressive illness.
  • Parkinson's disease or other pre-existing extrapyramidal disorders.

Frequently Asked Questions about Flunarizine

What is Flunarizine used for?

Flunarizine is mainly used to prevent migraine attacks in people who have frequent or severe migraines, and to treat vertigo (dizziness) caused by inner ear/vestibular problems. It is a preventive medicine, not a treatment for an attack already in progress.

How long can I take Flunarizine?

Flunarizine is usually taken in defined courses, not continuously without a break. Your doctor will typically review your response after a few months and may limit continuous use to around 6 months at a time before reassessing whether you still need it, because long-term use raises the risk of side effects like depression and movement problems.

Can Flunarizine cause depression or movement problems?

Yes. Flunarizine can occasionally cause or worsen depression and can cause extrapyramidal symptoms such as tremor, stiffness, and slowed movement, particularly with longer use and in older adults. Tell your doctor immediately if you notice low mood, unusual movements, or stiffness while taking Flunarizine.

Is Flunarizine safe for elderly patients?

Flunarizine is generally not recommended for elderly patients unless clearly needed, because they have a higher risk of sedation and movement-related side effects. If prescribed, a lower dose and close monitoring are typically used.

Can I drink alcohol while taking Flunarizine?

It is best to avoid alcohol while taking Flunarizine, since both can cause drowsiness, and combining them can increase sedation. Use caution when driving or operating machinery, especially when starting treatment.

Is Flunarizine safe during pregnancy or breastfeeding?

Flunarizine is generally not recommended during pregnancy or breastfeeding because safety has not been well established and the drug passes into breast milk. It should only be used in pregnancy or lactation if your doctor decides the benefit clearly outweighs the risk.
