Gemifloxacin is a fluoroquinolone antibacterial approved for the treatment of specific bacterial infections in adults caused by susceptible strains of designated microorganisms. Because fluoroquinolones as a class carry a risk of serious, potentially irreversible adverse effects, Gemifloxacin should be reserved for patients who have no other appropriate treatment options for the following indications.
Gemifloxacin is not indicated for uncomplicated infections that typically respond to narrower-spectrum agents; culture and susceptibility data should guide therapy whenever possible.
| Indication | Dose | Duration |
|---|---|---|
| Acute bacterial exacerbation of chronic bronchitis | 320 mg orally once daily | 5 days |
| Community-acquired pneumonia (susceptible pathogens) | 320 mg orally once daily | 5 days |
| Community-acquired pneumonia (multi-drug-resistant S. pneumoniae, K. pneumoniae, or M. catarrhalis) | 320 mg orally once daily | 7 days |
| Creatinine clearance | Recommended dose |
|---|---|
| > 40 mL/min | No adjustment needed (usual dose) |
| ≤ 40 mL/min (including hemodialysis or CAPD) | 160 mg once daily |
No dosage adjustment is required for hepatic impairment or in elderly patients based on age alone.
Gemifloxacin tablets may be taken with or without food and should be swallowed whole with a liberal amount of liquid. Antacids, sucralfate, and multivitamins/mineral supplements containing aluminum, magnesium, iron, or zinc should not be taken at the same time (see Interactions).
Take Gemifloxacin exactly as prescribed by your physician; do not stop, extend, skip doses, or share this medicine with others without medical advice. Completing the full prescribed course, even if symptoms improve early, helps ensure the infection is fully treated and reduces the risk of antibiotic resistance.
The most commonly reported adverse effects of Gemifloxacin (occurring in ≥2% of patients) are generally mild to moderate and include:
Elevated liver enzymes, taste disturbance, insomnia, constipation, dyspepsia, and mild changes in blood counts have also been reported. Serious effects — including tendon injury, peripheral neuropathy, central nervous system effects, QT prolongation, and hypersensitivity reactions — are addressed in detail under Precautions and Warnings, as they require prompt recognition and discontinuation of Gemifloxacin rather than routine management.
Gemifloxacin is contraindicated in:
