About Iloperidone

Indications of Iloperidone

Iloperidone is an atypical (second-generation) antipsychotic used in the treatment of psychiatric disorders.

Established / FDA-approved use

  • Acute treatment of schizophrenia in adults. Iloperidone is indicated for the management of the acute symptoms of schizophrenia in adult patients, based on controlled clinical trials.

Uses not established / not approved

  • Iloperidone is not approved for the treatment of dementia-related psychosis in elderly patients; use in this population carries a boxed warning of increased mortality (see Precautions and Warnings).
  • Iloperidone is not approved for use in children or adolescents (below 18 years); safety and efficacy have not been established in this age group.

Iloperidone should only be used for indications supported by adequate evidence and physician judgment; it should not be used off-label without careful risk-benefit assessment by a qualified physician.

Dosage & Administration of Iloperidone

Adult Dose (Schizophrenia)

Iloperidone must be started with a gradual dose titration to reduce the risk of orthostatic hypotension. Do not initiate at the target maintenance dose.

Titration DayTotal Daily Dose
Day 11 mg twice daily (2 mg/day)
Day 22 mg twice daily (4 mg/day)
Day 34 mg twice daily (8 mg/day)
Day 46 mg twice daily (12 mg/day)
Day 58 mg twice daily (16 mg/day)
Day 610 mg twice daily (20 mg/day)
Day 712 mg twice daily (24 mg/day)

The recommended target maintenance dose range is 6 mg to 12 mg twice daily (12 mg to 24 mg per day), individualized according to response and tolerability. The maximum recommended dose is 24 mg/day.

Dose Adjustment for Drug Interactions / Poor Metabolizers

When Iloperidone is co-administered with a strong CYP2D6 inhibitor, a strong CYP3A4 inhibitor, or in patients known to be poor CYP2D6 metabolizers, the Iloperidone dose should be reduced to half of the otherwise indicated dose (see Interactions and Contraindications).

Renal and Hepatic Impairment

No specific dose adjustment is established for mild-to-moderate renal impairment; Iloperidone has not been systematically studied in severe renal or hepatic impairment, so it should be used with caution and close monitoring in these patients.

Pediatric Dose

Safety and efficacy of Iloperidone have not been established in patients under 18 years; therefore no pediatric dose is established.

Side Effects of Iloperidone

Common Side Effects

  • Dizziness and orthostatic hypotension (most common at treatment initiation/titration)
  • Dry mouth
  • Fatigue and somnolence
  • Tachycardia (reflex, related to orthostatic hypotension)
  • Nasal congestion
  • Weight gain

Serious but Less Common Side Effects

  • QT interval prolongation, with risk of serious arrhythmia in susceptible patients
  • Neuroleptic malignant syndrome (rare, potentially fatal)
  • Tardive dyskinesia (may be irreversible)
  • Metabolic changes: hyperglycemia, dyslipidemia, and weight gain requiring periodic monitoring
  • Leukopenia, neutropenia, or agranulocytosis (rare, class effect of antipsychotics)

Contraindications of Iloperidone

  • Known hypersensitivity to Iloperidone or any component of the formulation.
  • Congenital long QT syndrome, due to the risk of Iloperidone-associated QT prolongation.
  • Concomitant use with other drugs that prolong the QT interval.
  • Concomitant use with a strong CYP2D6 or CYP3A4 inhibitor at standard (non-reduced) Iloperidone dosing, since this combination markedly raises Iloperidone exposure and QT-prolongation risk unless the dose is halved as directed.

Frequently Asked Questions about Iloperidone

What is Iloperidone used for?

Iloperidone is an atypical antipsychotic approved for the acute treatment of schizophrenia in adults.

Why does Iloperidone need to be started at a low dose and increased gradually?

Iloperidone is titrated slowly over about a week because starting at a high dose significantly increases the risk of orthostatic hypotension (a sudden drop in blood pressure on standing). Following the titration schedule your physician gives you helps reduce this risk.

Can Iloperidone affect my heart?

Yes. Iloperidone can prolong the QT interval of the heart's electrical cycle in a dose-dependent manner, which can increase the risk of a serious irregular heartbeat. It should not be combined with other QT-prolonging medicines or used in people with congenital long QT syndrome.

Is Iloperidone safe during pregnancy?

Iloperidone should only be used during pregnancy if clearly needed and if your physician determines the benefit outweighs the potential risk to the baby. Antipsychotics used late in pregnancy, including Iloperidone, may cause temporary movement or withdrawal symptoms in the newborn, so any exposed infant should be monitored.

What should I do if I miss doses or stop Iloperidone for a few days?

If Iloperidone is interrupted for more than 3 days, it must be re-titrated from the starting dose rather than resumed at the previous maintenance dose, to avoid the risk of orthostatic hypotension. Contact your physician before restarting.

What should I do if someone takes too much Iloperidone?

Overdose can cause drowsiness, rapid heartbeat, low blood pressure, and dangerous heart rhythm changes. Seek immediate medical attention or contact emergency services/poison control right away; do not attempt to treat an overdose at home.
