About Paracetamol + Caffeine

Indications of Paracetamol + Caffeine

Paracetamol + Caffeine is an over-the-counter analgesic-antipyretic combination widely used for the short-term relief of mild-to-moderate pain and fever.

Established uses

  • Headache, including tension-type headache
  • Toothache
  • Backache and minor musculoskeletal aches and pains
  • Period pain (dysmenorrhea)
  • Sore throat pain
  • Fever and generalized aches associated with the common cold and influenza

Adjunct use

Paracetamol + Caffeine is also used as an adjunct in the symptomatic relief of migraine headache; caffeine is added to enhance the analgesic effect of paracetamol and to help counter the fatigue and drowsiness that often accompany headache and migraine, rather than to treat migraine on its own.

Dosage & Administration of Paracetamol + Caffeine

IndicationPopulationDose
Headache, fever, minor painAdults and children 12 years and older1–2 tablets every 4–6 hours as needed, not to exceed 8 tablets (or the manufacturer's stated maximum) in 24 hours
Headache, fever, minor painChildren under 12 yearsNot recommended; use a pediatrician-directed, weight-based paracetamol-only product instead (see Pediatric Uses)

The total daily dose of paracetamol from Paracetamol + Caffeine and from any other paracetamol-containing product combined should never exceed the maximum recommended daily dose of paracetamol (generally 4 g/day in healthy adults; lower limits apply in the elderly, in low-body-weight adults, and in those with liver disease — see Precautions and Warnings).

Paracetamol + Caffeine should not be used continuously for more than 10 days for pain or 3 days for fever without medical advice (see Duration of Treatment).

Side Effects of Paracetamol + Caffeine

Paracetamol + Caffeine is generally well tolerated at recommended doses. Reported side effects include:

  • Nausea or mild stomach upset (uncommon)
  • Skin rash, itching, or urticaria (uncommon; discontinue if a skin reaction occurs)
  • Nervousness, restlessness, insomnia, or palpitations, related to the caffeine component, particularly with higher intake or in caffeine-sensitive individuals
  • Rare but serious hematological reactions (e.g., thrombocytopenia, leukopenia, agranulocytosis) have been reported very rarely with long-term paracetamol use
  • Serious liver injury with overdose or chronic excessive use (see Overdose and Precautions)

Contraindications of Paracetamol + Caffeine

Paracetamol + Caffeine is contraindicated in:

  • Patients with known hypersensitivity to paracetamol, caffeine, or any excipient of the product.
  • Patients with severe hepatic impairment or severe active liver disease.

Frequently Asked Questions about Paracetamol + Caffeine

What is Paracetamol + Caffeine used for?

Paracetamol + Caffeine is used for the short-term relief of mild-to-moderate pain such as headache, migraine-associated headache, toothache, backache, and period pain, and to reduce fever, such as that associated with colds and flu.

Why does Paracetamol + Caffeine contain caffeine?

Caffeine is added at a low, sub-stimulant dose in Paracetamol + Caffeine to enhance the pain-relieving effect of paracetamol and to counter the tiredness or drowsiness that often accompanies headache and pain, not to treat pain on its own.

How much Paracetamol + Caffeine can I take in a day?

Adults and children 12 years and older can generally take 1–2 tablets of Paracetamol + Caffeine every 4–6 hours, up to a maximum stated on the product label (commonly 8 tablets in 24 hours). You must never exceed the maximum recommended daily dose of paracetamol, including from other paracetamol-containing products taken at the same time.

Can I take Paracetamol + Caffeine if I am pregnant or breastfeeding?

Paracetamol + Caffeine should be used in pregnancy only if clearly needed and after consulting a physician, since it contains caffeine in addition to paracetamol, and total caffeine intake from all sources should be limited in pregnancy. It is generally considered compatible with breastfeeding at recommended doses, but a physician or pharmacist should be consulted first.

Is Paracetamol + Caffeine safe for children?

Paracetamol + Caffeine is generally not recommended for children under 12 years of age because safety and efficacy in this age group have not been established; a paracetamol-only, weight-based product directed by a physician is usually preferred for children.

What should I do if I take too much Paracetamol + Caffeine?

Overdose of Paracetamol + Caffeine is a medical emergency because of the paracetamol component, which can cause serious, potentially fatal liver damage that may not cause noticeable symptoms for the first day. Seek immediate medical attention or contact a poison control center right away, even if the person seems fine.
