About Pimecrolimus

Indications of Pimecrolimus

Pimecrolimus is a topical calcineurin inhibitor indicated for the treatment of mild-to-moderate atopic dermatitis (eczema).

Established (FDA-approved) use

  • Second-line, short-term treatment of mild-to-moderate atopic dermatitis in non-immunocompromised patients 2 years of age and older who have not responded adequately to, or who cannot tolerate, other treatments (such as topical corticosteroids).
  • Non-continuous long-term (intermittent) treatment to prevent disease flares, under physician supervision.

Particularly useful for

  • Sensitive skin areas such as the face, eyelids, neck, and skin folds (axillae, groin), where prolonged topical corticosteroid use carries a risk of skin thinning (atrophy).

Pimecrolimus is not a first-line treatment and is not indicated for other skin conditions unless specifically advised by a physician.

Dosage & Administration of Pimecrolimus

Adults and children 2 years of age and older

IndicationDose
Mild-to-moderate atopic dermatitisApply a thin layer of Pimecrolimus 1% cream to the affected skin areas twice daily. Rub in gently and completely.

Children under 2 years of age

Safety and efficacy have not been established; use in this age group is not recommended (see Pediatric Use).

General administration notes

  • Pimecrolimus may be used on all affected skin surfaces, including the face, neck, and skin folds, but not inside the mouth, eyes, or vagina.
  • Treatment should continue until signs and symptoms have resolved. If no improvement is seen within 6 weeks, the patient should be re-examined to confirm the diagnosis.
  • For intermittent long-term use to prevent flares, apply at the earliest signs/symptoms and continue under physician guidance.
  • Do not use with occlusive dressings, as this has not been studied and may increase systemic absorption.

Side Effects of Pimecrolimus

The most common adverse reaction associated with Pimecrolimus is a mild-to-moderate sensation at the application site.

Very common

  • Application-site burning, stinging, or warmth (usually mild and decreases with continued use)

Common

  • Application-site irritation, erythema, or pruritus
  • Headache
  • Nasopharyngitis (common cold symptoms)
  • Influenza-like illness
  • Cough

Uncommon/rare

  • Folliculitis or worsening of atopic dermatitis
  • Skin papilloma/warts
  • Hypersensitivity reactions (rash, swelling)

Patients should notify their physician if application-site reactions are severe, persistent, or if signs of skin infection develop.

Contraindications of Pimecrolimus

Pimecrolimus is contraindicated in patients with:

  • Known hypersensitivity to Pimecrolimus or to any component of the cream formulation.

Pimecrolimus should not be applied to areas of active viral skin infections (e.g., herpes simplex, chickenpox, shingles) or to malignant or premalignant skin lesions (see Precautions and Warnings).

Frequently Asked Questions about Pimecrolimus

What is Pimecrolimus used for?

Pimecrolimus is a topical cream used to treat mild-to-moderate atopic dermatitis (eczema), especially on sensitive areas like the face and skin folds, in patients who cannot use or have not responded well to other treatments such as steroid creams.

Is Pimecrolimus a steroid?

No. Pimecrolimus is a non-steroidal topical calcineurin inhibitor. It works differently from corticosteroids and does not cause the skin thinning (atrophy) associated with long-term steroid use.

Is it normal for my skin to burn or sting after applying Pimecrolimus?

Yes, a temporary burning, stinging, or warm sensation at the application site is the most common reaction, especially in the first few days of treatment, and it usually decreases with continued use. If it is severe or persistent, consult your physician.

Can Pimecrolimus cause cancer?

Pimecrolimus carries a boxed warning because rare cases of skin cancer and lymphoma have been reported in patients using topical calcineurin inhibitors, although a direct cause-and-effect relationship has not been proven in humans. As a precaution, use the lowest effective amount for the shortest necessary time, avoid continuous long-term use, and avoid excessive sun exposure while using this medicine.

Can Pimecrolimus be used during pregnancy or breastfeeding?

Pimecrolimus has minimal absorption into the bloodstream after skin application, but safety data in pregnancy and breastfeeding are limited. It should be used only if clearly needed, after consulting a physician, and breastfeeding mothers should avoid applying it to the breast/nipple area.

Can Pimecrolimus be used in young children?

Pimecrolimus is approved for children 2 years of age and older. It is not recommended for infants and children under 2 years because safety and efficacy have not been established in this age group.
