Pioglitazone + Glimepiride is a fixed-dose combination antidiabetic medicine used in the management of type 2 diabetes mellitus and more.
Pioglitazone + Glimepiride is a fixed-dose combination antidiabetic medicine used in the management of type 2 diabetes mellitus, indicated as an adjunct to diet and exercise to improve glycemic control.
The dosage of Pioglitazone + Glimepiride must be individualized based on the patient's current antidiabetic regimen, glycemic response, and tolerability, and should be started at the lowest appropriate dose.
| Clinical scenario | Typical starting approach |
|---|---|
| Inadequate control on sulfonylurea alone | Start with a low-dose pioglitazone/glimepiride combination and titrate based on glycemic response, generally not exceeding pioglitazone 45 mg and glimepiride 8 mg per day. |
| Already on pioglitazone plus glimepiride separately | Pioglitazone + Glimepiride may be substituted at the corresponding component doses already being taken. |
| Initial response to pioglitazone alone, needing added control | Add the sulfonylurea component at a low starting dose, then titrate. |
See Dosage and Administration sections below for further detail, and Use in Special Populations for renal/hepatic adjustment.
Side effects of Pioglitazone + Glimepiride may arise from either component:
| Common | Less common / Serious |
|---|---|
| Hypoglycemia, weight gain, headache, upper respiratory tract infection, mild edema | Fluid retention/heart failure, macular edema, bone fractures (especially in women), hepatic enzyme elevation, hemolytic anemia (in G6PD-deficient patients) |
