About Testosterone Decanoate

Indications of Testosterone Decanoate

Testosterone Decanoate is a long-acting testosterone ester used as part of injectable androgen replacement therapy. It is typically formulated together with other testosterone esters (propionate, phenylpropionate, isocaproate) in a single intramuscular injection to provide both rapid and sustained release of testosterone.

Established/Approved Uses

  • Male hypogonadism: Testosterone replacement therapy in adult males with primary hypogonadism (congenital or acquired testicular failure, e.g. cryptorchidism, orchitis, orchidectomy, Klinefelter syndrome) or hypogonadotropic hypogonadism (idiopathic gonadotropin/LH-RH deficiency, or pituitary-hypothalamic injury from tumours, trauma, or radiation), when the diagnosis has been confirmed by clinical features and biochemical (laboratory) testing.

Guideline-Supported / Adjunct Uses

  • Gender-affirming hormone therapy (masculinization) in female-to-male transgender individuals, under specialist endocrinology supervision.
  • Delayed puberty in adolescent males due to confirmed hypogonadism, under paediatric endocrinology supervision (short courses only).

Not Recommended / Off-Label Caution

  • Testosterone Decanoate is not indicated for age-related decline in testosterone ("low T") without confirmed pathological hypogonadism, and is not approved for enhancement of athletic performance or physique, which carries serious cardiovascular and psychiatric risks (see Precautions and Warnings).

Dosage & Administration of Testosterone Decanoate

Route

Testosterone Decanoate (as part of a mixed testosterone ester injection) is given by deep intramuscular injection only - never intravenously. It must not be self-administered without proper training and physician oversight.

Adult Male Hypogonadism

IndicationTypical DoseFrequency
Confirmed male hypogonadismOne ampoule (per product strength, e.g. 1 mL) by deep IM injectionEvery 3 weeks, adjusted according to clinical response and serum testosterone levels

Gender-Affirming Therapy (Female-to-Male)

Typically one injection every 2-4 weeks, individualized by a specialist according to clinical and hormonal response.

Pediatric / Delayed Puberty

Used only under paediatric endocrinology supervision, with lower doses and shorter treatment duration; safety and efficacy for routine use in children have not been fully established (see Use in Special Populations).

Renal/Hepatic Impairment

No formal dose-adjustment guidelines are established; however, Testosterone Decanoate should be used with caution and close monitoring in patients with renal or hepatic impairment, and is contraindicated in severe hepatic or renal disease (see Contraindications).

Monitoring

Periodic monitoring of serum testosterone, haematocrit/haemoglobin, prostate-specific antigen (PSA) and digital rectal examination (in appropriate age groups), lipid profile, and liver function is recommended during treatment with Testosterone Decanoate.

Side Effects of Testosterone Decanoate

Side effects of Testosterone Decanoate are generally dose-related and reflect excess androgenic activity.

Common

  • Acne, oily skin
  • Injection-site pain, induration, or reaction
  • Increased haematocrit/haemoglobin (polycythaemia)
  • Weight gain, fluid and sodium retention (oedema)
  • Increased libido; in some cases, increased frequency of erections
  • Gynaecomastia (breast enlargement/tenderness), due to peripheral conversion to oestrogen

Less Common / Serious

  • Hepatic dysfunction (abnormal liver enzymes, rarely jaundice)
  • Priapism (prolonged, painful erection) - requires urgent medical attention
  • Oligospermia/azoospermia and reduced testicular size with prolonged use, due to suppression of endogenous gonadotropins
  • Worsening of sleep apnoea
  • Mood changes, aggression, or psychiatric disturbance
  • Rare pulmonary oil microembolism reaction (cough, dyspnoea, dizziness) immediately after injection of oily testosterone formulations
  • In women (if inadvertently exposed or used off-label): virilization - deepening of voice, hirsutism, clitoromegaly, menstrual irregularities (partly irreversible)
  • Prepubertal boys: precocious sexual development, phallic enlargement, premature epiphyseal closure with resultant compromised final adult height

Contraindications of Testosterone Decanoate

Testosterone Decanoate is contraindicated in:

  • Known hypersensitivity to Testosterone Decanoate or any excipient of the product (including peanut/soya-derived oils used as vehicle in some formulations).
  • Known or suspected carcinoma of the prostate.
  • Known or suspected carcinoma of the male breast.
  • Pregnancy (see Pregnancy and Lactation) - Testosterone Decanoate is not indicated for use in women and pregnant patients.

  • Severe renal impairment.

Frequently Asked Questions about Testosterone Decanoate

What is Testosterone Decanoate used for?

Testosterone Decanoate is used as part of injectable testosterone replacement therapy for adult men with confirmed hypogonadism (low testosterone due to testicular or pituitary disease), and in some cases for gender-affirming hormone therapy or delayed puberty under specialist supervision.

How often is Testosterone Decanoate injected?

Testosterone Decanoate, usually given as part of a mixed-ester formulation, is typically injected deep into a muscle about once every 3 weeks for hypogonadism, though your physician may adjust the frequency based on your symptoms and blood testosterone levels.

Can Testosterone Decanoate be used for bodybuilding or athletic performance?

No. Testosterone Decanoate is not approved for physique or athletic enhancement. Misuse for these purposes carries serious risks of heart attack, stroke, and psychiatric problems, and is illegal without a valid medical prescription in many countries.

Is Testosterone Decanoate safe during pregnancy?

No. Testosterone Decanoate is contraindicated in pregnancy because it can cause virilization (masculinizing changes) of a female fetus. It is not indicated for use in women, and is also not recommended during breastfeeding.

What are the main side effects of Testosterone Decanoate?

Common side effects of Testosterone Decanoate include acne, injection-site reactions, increased red blood cell count, fluid retention, and gynaecomastia. More serious effects can include prostate changes, worsened sleep apnoea, priapism, liver enzyme changes, and cardiovascular events, especially with misuse or high doses.

Who should not use Testosterone Decanoate?

Testosterone Decanoate should not be used by men with known or suspected prostate or breast cancer, by anyone with a known hypersensitivity to the product, in severe liver or kidney disease, or in pregnant women. It requires physician supervision and periodic monitoring throughout treatment.
