About Valganciclovir

Indications of Valganciclovir

Valganciclovir is an antiviral medicine used against cytomegalovirus (CMV) infection. Its approved and evidence-based uses are classified below by strength of evidence.

Established / FDA-approved uses

  • Treatment of CMV retinitis in adults with acquired immunodeficiency syndrome (AIDS) and other immunocompromised patients at risk of vision-threatening CMV eye disease.
  • Prevention (prophylaxis) of CMV disease in adult and certain pediatric (4 months to 16 years) kidney, heart, and kidney-pancreas transplant recipients who are at high risk of developing CMV infection after transplantation.

Guideline-supported / commonly extended uses

  • Prevention of CMV disease in other solid organ transplant recipients (e.g. liver, lung) at high risk, based on transplant-society guidelines, even where formal product labeling is transplant-type specific.
  • Pre-emptive therapy for asymptomatic CMV viremia detected on surveillance testing in transplant recipients, guided by a specialist.

Off-label uses (specialist-directed only)

  • Management of CMV disease in hematopoietic stem cell transplant recipients.
  • Treatment of certain other CMV end-organ diseases (e.g. CMV colitis, CMV pneumonitis) in immunocompromised patients, under infectious disease specialist supervision.

Valganciclovir is not effective against, and should not be used for, infections caused by organisms other than CMV and related herpesviruses.

Dosage & Administration of Valganciclovir

Dosing by indication

IndicationPopulationTypical dose
CMV retinitis, inductionAdults with normal renal function900 mg orally twice daily with food for 21 days
CMV retinitis, maintenanceAdults with normal renal function900 mg orally once daily with food; may resume induction dosing if disease progresses
CMV prevention after transplantAdults with normal renal function900 mg orally once daily with food, started within 10 days of transplantation and continued for the duration recommended for the specific transplant type
CMV prevention after kidney or heart transplantPediatric patients 4 months to 16 yearsOnce-daily dose individualized using a body-surface-area and creatinine-clearance based formula, calculated and prescribed by the treating specialist

Renal impairment

Dose and/or dosing frequency of Valganciclovir must be reduced according to creatinine clearance; a physician or pharmacist should calculate the adjusted regimen for each patient, as unadjusted dosing in renal impairment significantly increases the risk of hematologic toxicity (see Precautions and Warnings).

Hepatic impairment

Valganciclovir has not been formally studied in hepatic impairment; no specific dose adjustment has been established, and it should be used with caution under medical supervision in patients with significant liver disease.

Side Effects of Valganciclovir

The most commonly reported side effects of Valganciclovir include:

  • Diarrhea, nausea, vomiting, abdominal pain
  • Headache, insomnia, peripheral neuropathy, tremor
  • Fever (pyrexia), fatigue
  • Neutropenia, anemia, and thrombocytopenia (see Precautions and Warnings for full detail on blood-count monitoring)

Less common but serious effects can include severe bone marrow suppression, renal function changes, and central nervous system effects such as confusion or seizures (see Precautions and Warnings). Patients should promptly report unusual bruising or bleeding, signs of infection, or significant mood/mental changes to their physician.

Contraindications of Valganciclovir

Valganciclovir is contraindicated in:

  • Patients with known hypersensitivity to Valganciclovir, ganciclovir, or valacyclovir (due to shared/related chemical structure and cross-sensitivity potential).
  • Patients with CMV retinitis who have a baseline absolute neutrophil count below 500/microliter, a platelet count below 25,000/microliter, or a hemoglobin level below 8 g/dL, until these values have been corrected.

Frequently Asked Questions about Valganciclovir

What is Valganciclovir used for?

Valganciclovir is an antiviral medicine used to treat cytomegalovirus (CMV) retinitis in people with weakened immune systems (such as AIDS patients) and to prevent CMV disease in certain high-risk organ transplant recipients. It is not used for common viral infections like colds or flu.

How should I take Valganciclovir?

Valganciclovir should be taken exactly as prescribed, generally with food to improve absorption. Tablets should be swallowed whole and not crushed or chewed. Never stop, skip, extend, or share your doses without consulting your physician, as this can reduce effectiveness and increase the risk of side effects.

What are the most serious risks of Valganciclovir?

Valganciclovir carries boxed warnings for severe blood-count suppression (low white cells, red cells, and platelets), potential harm to a developing fetus and possible cancer risk based on animal studies, and possible effects on fertility in both men and women. Regular blood tests are needed during treatment, and effective contraception is required during and for a period after treatment.

Can Valganciclovir be used during pregnancy or breastfeeding?

Valganciclovir should be used in pregnancy only if the physician judges the benefit clearly outweighs the risk to the fetus, because animal studies suggest it may harm a developing baby. Breastfeeding is not recommended during treatment because Valganciclovir may pass into breast milk and harm a nursing infant; discuss safer alternatives with your physician.

Can Valganciclovir be given to children?

Valganciclovir is approved only for preventing CMV disease in certain kidney or heart transplant patients aged 4 months to 16 years, with the dose carefully calculated by a specialist. It has not been established as safe and effective for other pediatric uses, including CMV retinitis treatment.

What should I do if I miss a dose or take too much Valganciclovir?

If you miss a dose, take it as soon as you remember unless it is close to the next dose; do not double up. If you or someone else has taken more Valganciclovir than prescribed, seek immediate medical attention or contact a poison control center right away, since overdose can worsen blood-count and kidney problems.
