De-Rash Plus is a topical antifungal-barrier preparation indicated for the treatment and prevention of diaper rash (nappy rash) that is complicated by, or at risk of, cutaneous candidiasis (yeast infection) in infants and, where labeled, in adults with incontinence-associated dermatitis.

• Established use: Treatment of diaper dermatitis with confirmed or clinically suspected superimposed Candida infection, combining an antifungal with a protective occlusive barrier.
• Preventive/adjunct use: Prevention of recurrence of fungal diaper rash in infants prone to repeated episodes, used as part of routine napkin-area skin care alongside frequent diaper changes and hygiene measures.
• Guideline-supported use: As a barrier and mild antifungal measure in intertrigo of skin folds where friction, moisture, and candidal overgrowth coexist.

De-Rash Plus is not intended for the treatment of extensive, deep, or systemic fungal infections, which require systemic antifungal therapy.

Each gram of the topical preparation contains:

• Miconazole Nitrate ... an imidazole antifungal agent
• Zinc Oxide ... a mild astringent and skin protectant
• White Soft Paraffin ... an occlusive emollient base

Together these form Miconazole Nitrate + Zinc Oxide + White Soft Paraffin, formulated as a cream or ointment for topical application to the diaper area or affected skin folds.

De-Rash Plus is a combination topical preparation that pairs an antifungal agent with a physical skin-protectant barrier. Miconazole nitrate provides antifungal activity against Candida albicans and other yeasts/dermatophytes commonly implicated in diaper rash, while zinc oxide and white soft paraffin form a soothing, water-repellent barrier over irritated skin, reducing further contact with moisture, urine, and stool.

This dual action makes De-Rash Plus suitable for diaper rash that either shows signs of fungal involvement (satellite lesions, beefy-red erythema in skin folds) or is at high risk of developing it.

De-Rash Plus belongs to the class of topical antifungal-plus-skin-protectant combination products, used in dermatology and pediatric skin care for diaper dermatitis management.

Miconazole Nitrate + Zinc Oxide + White Soft Paraffin combines three components with complementary mechanisms:

• Miconazole nitrate is an imidazole derivative that inhibits fungal cytochrome P450-dependent 14-alpha-demethylase, blocking conversion of lanosterol to ergosterol. Depletion of ergosterol disrupts fungal cell membrane structure and function, producing fungistatic and, at higher concentrations, fungicidal activity against Candida albicans and other yeasts and dermatophytes.
• Zinc oxide acts as a mild astringent and skin protectant; it has weak antiseptic properties, helps dry weepy lesions, and reduces friction and irritation on the skin surface.
• White soft paraffin is an inert occlusive emollient that forms a hydrophobic film over the skin, minimizing contact with urine, faeces, and moisture, and reducing transepidermal water loss to support barrier repair.

Systemic absorption of miconazole through intact or mildly inflamed skin is minimal when Miconazole Nitrate + Zinc Oxide + White Soft Paraffin is applied as directed to the diaper area.

For external (topical) use only. Clean and gently dry the affected area before application. Apply De-Rash Plus in a thin layer and gently rub in until absorbed. Wash hands before and after application, unless hands are being treated. Avoid application to broken skin or open wounds unless directed by a physician. Do not apply to the eyes, inside the nose or mouth, or on mucous membranes.

Clinically significant interactions with De-Rash Plus are uncommon due to its minimal systemic absorption when used topically as directed.

• Concurrent use of other topical products (talcum powder, other creams/ointments) on the same skin area may reduce adhesion or effectiveness of De-Rash Plus and is generally not recommended unless advised by a physician.
• Miconazole applied topically over large areas of broken skin, especially in neonates, could theoretically be absorbed enough to interact with oral anticoagulants (e.g., warfarin), potentiating bleeding risk; caution is advised if De-Rash Plus is used on caregivers who take warfarin and apply it with bare hands over large areas.

De-Rash Plus is generally well tolerated when used as directed. Reported adverse effects are usually mild and local:

• Mild burning, stinging, or itching sensation at the application site
• Local skin irritation or redness
• Rarely, contact dermatitis or allergic skin reaction

If severe irritation, blistering, or signs of an allergic reaction (rash spreading beyond the application site, swelling, difficulty breathing) occur, discontinue De-Rash Plus and seek medical attention.

De-Rash Plus is applied topically to the infant's or patient's skin and is not typically used systemically; however, if applied by a pregnant or breastfeeding caregiver, only a small amount contacts the caregiver's skin. Systemic absorption of miconazole from topical use is minimal. As a general precaution, De-Rash Plus should be used during pregnancy and lactation only if clearly needed, and the potential benefit justifies any potential risk; a physician or pharmacist should be consulted before use in pregnant or breastfeeding individuals, and care should be taken to avoid application to the breast area if breastfeeding.

For external use only. De-Rash Plus is not for oral, ophthalmic, or intravaginal use.

• Avoid contact with the eyes; if accidental contact occurs, rinse thoroughly with water.
• Do not apply to open wounds, deep cuts, or severely broken skin unless directed by a physician.
• Discontinue use and consult a physician if the rash worsens, does not improve within about 7 days, or if signs of secondary bacterial infection (increasing redness, warmth, pus, fever) develop.
• Complete the full recommended course of treatment as directed by the physician, even if the rash appears to improve early, to reduce the risk of fungal recurrence; do not stop, extend, or share this medicine with others without medical advice.
• Keep the diaper area as dry and well-ventilated as possible; change diapers frequently.
• Not intended for long-term continuous use without medical supervision.

Topical overdose of De-Rash Plus is unlikely to cause systemic toxicity given its minimal absorption; however, excessive or prolonged application may increase the risk of local irritation. If De-Rash Plus is accidentally ingested (e.g., by a child), or if signs of a severe allergic reaction occur, seek immediate medical attention or contact a poison control center/emergency services rather than attempting home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children. Keep the container tightly closed after use.

Infants and Neonates: De-Rash Plus is commonly used in infants for diaper rash; use the minimum effective amount and avoid application over large areas of broken skin in very young or premature infants due to a theoretically greater potential for systemic absorption.

Elderly: De-Rash Plus may be used for incontinence-associated dermatitis in elderly patients under medical supervision; no specific dose adjustment is required for topical use.

Renal/Hepatic Impairment: No dose adjustment is established as systemic exposure from topical use is minimal.

Immunocompromised patients: Persistent or extensive fungal skin infections in immunocompromised individuals may require evaluation for systemic antifungal therapy in addition to, or instead of, De-Rash Plus; consult a physician.

De-Rash Plus is typically used until the rash has fully resolved, generally within 1-2 weeks of consistent use. If there is no improvement within 7 days, or if the condition worsens, treatment should be reassessed by a physician. For prevention of recurrence, De-Rash Plus may be used intermittently as part of routine diaper-area care, as advised by a physician.

Miconazole Nitrate + Zinc Oxide + White Soft Paraffin is classified as a topical antifungal (imidazole) combined with a skin protectant/barrier preparation.
