Divastin Plus is a fixed-dose combination indicated in patients for whom treatment with both a calcium channel blocker (amlodipine) and an HMG-CoA reductase inhibitor/statin (atorvastatin) is considered appropriate.
Divastin Plus is only appropriate when a patient independently needs both an antihypertensive calcium channel blocker and a statin; it is not intended for patients who need only one of the two components.
Divastin Plus should always be used alongside diet, exercise, and other lifestyle modifications as advised by a physician.
Amlodipine + Atorvastatin is a fixed-dose combination tablet containing:
Amlodipine + Atorvastatin tablets are available in multiple strength combinations of amlodipine and atorvastatin (for example, amlodipine 5 mg or 10 mg combined with atorvastatin 10 mg, 20 mg, 40 mg, or 80 mg), allowing dose individualization based on a patient's blood pressure and lipid targets.
Divastin Plus combines two medicines that act on two different cardiovascular risk factors in a single tablet.
Amlodipine is a long-acting dihydropyridine calcium channel blocker that relaxes vascular smooth muscle, dilates peripheral arteries, and lowers blood pressure. Atorvastatin is a statin that inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis, thereby lowering LDL-cholesterol and triglycerides while raising HDL-cholesterol.
By combining an antihypertensive with a lipid-lowering agent, Divastin Plus simplifies treatment for patients who need both types of therapy for overall cardiovascular risk reduction.
Divastin Plus belongs to a combination therapeutic class comprising:
Divastin Plus is therefore classified as an antihypertensive–antihyperlipidemic fixed-dose combination.
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle by blocking L-type calcium channels. This produces relaxation of vascular smooth muscle (vasodilation), reducing peripheral vascular resistance and blood pressure, with a gradual onset of action and a long half-life allowing once-daily dosing.
Atorvastatin selectively and competitively inhibits HMG-CoA reductase, the enzyme that converts HMG-CoA to mevalonate, an early and rate-limiting step in cholesterol biosynthesis. This reduces hepatic cholesterol content, upregulates hepatic LDL-receptor expression, and increases clearance of LDL from the bloodstream, resulting in reductions in LDL-cholesterol, total cholesterol, apolipoprotein B, and triglycerides, and modest increases in HDL-cholesterol.
Amlodipine + Atorvastatin combines these two complementary mechanisms of action in one tablet, addressing blood pressure and lipid abnormalities together.
The dose of Divastin Plus must be individualized based on the patient's blood pressure control needs and lipid goals, taking into account the effective and tolerated doses of each component when previously used separately.
| Indication | Typical starting dose | Maximum dose |
|---|---|---|
| Hypertension (amlodipine component) | 5 mg once daily | 10 mg once daily |
| Hypercholesterolemia/dyslipidemia (atorvastatin component) | 10–20 mg once daily (up to 40 mg if a large LDL reduction is needed) | 80 mg once daily |
Divastin Plus tablets are typically available in combinations ranging from amlodipine 5 mg/atorvastatin 10 mg up to amlodipine 10 mg/atorvastatin 80 mg, allowing selection of the combination that matches the patient's individually titrated doses of each component.
When titration of either component is required, allow at least 7 to 14 days between dose adjustments to assess response, unless more rapid titration is clinically indicated (for example, in an inpatient setting with close monitoring).
Take Divastin Plus once daily, at approximately the same time each day, with or without food. Swallow the tablet whole with a glass of water.
No dosage adjustment of Divastin Plus is required for renal impairment, as neither amlodipine nor atorvastatin pharmacokinetics are significantly affected by renal function; however, patients on hemodialysis should be monitored as clinically appropriate.
Divastin Plus should be used with caution in mild-to-moderate hepatic impairment and is contraindicated in active liver disease (see Contraindications). Lower atorvastatin starting doses and close liver enzyme monitoring are advised when hepatic impairment is present.
Divastin Plus is taken orally, once daily, with or without food, at approximately the same time each day to maintain steady blood levels. Tablets should be swallowed whole and not crushed or chewed unless a physician has advised otherwise.
Divastin Plus carries a combined interaction profile from both its amlodipine and atorvastatin components. Report all prescription, over-the-counter, and herbal products to your physician before starting Divastin Plus.
Amlodipine + Atorvastatin is contraindicated in:
Most patients tolerate Divastin Plus well, but side effects can occur from either component.
Contact your physician promptly if you notice any serious or persistent side effects while taking Divastin Plus.
Divastin Plus is contraindicated in pregnancy. The atorvastatin component may cause fetal harm, as cholesterol and cholesterol biosynthesis products are essential for fetal development; statins provide no known benefit during pregnancy. If a patient becomes pregnant while taking Divastin Plus, the medicine should be discontinued immediately and the physician informed.
Women of reproductive potential should use effective contraception while taking Divastin Plus. If pregnancy is planned, Divastin Plus should be stopped in advance and alternative management discussed with a physician.
Divastin Plus is contraindicated during breastfeeding. It is not known whether amlodipine passes into breast milk in clinically significant amounts, but the atorvastatin component is contraindicated during lactation due to the potential for serious adverse effects in the nursing infant. Women who require Divastin Plus should not breastfeed, and physicians should help identify alternative feeding or treatment strategies.
Divastin Plus contains atorvastatin, which like other statins can rarely cause myopathy and, very rarely, life-threatening rhabdomyolysis with acute kidney injury. Risk is increased with advanced age, renal impairment, hypothyroidism, and concomitant use of interacting drugs (see Interactions). Patients should report unexplained muscle pain, tenderness, or weakness immediately, especially if accompanied by fever or dark urine. Divastin Plus should be discontinued if myopathy is diagnosed or suspected, or if creatine kinase levels are markedly elevated.
Liver enzyme abnormalities have occurred with statin use. Liver function tests are recommended before starting Divastin Plus and as clinically indicated thereafter. Divastin Plus should be used with caution in patients with a history of liver disease or substantial alcohol use, and is contraindicated in active liver disease (see Contraindications).
The amlodipine component of Divastin Plus can cause symptomatic hypotension, particularly in patients with severe aortic stenosis, and should be used cautiously in patients with severe hypotension, unstable angina, or heart failure, as it may occasionally increase the frequency, duration, or severity of angina or precipitate a myocardial infarction, particularly at treatment initiation or dose increase.
See Pregnancy and Lactation — Divastin Plus is contraindicated in pregnancy due to the atorvastatin component.
Do not stop Divastin Plus abruptly without consulting your physician. Regular follow-up, including blood pressure checks and periodic lipid and liver function monitoring, is recommended during treatment with Divastin Plus.
Overdose of Divastin Plus may cause excessive peripheral vasodilation with marked and prolonged systemic hypotension, reflex tachycardia, and possible shock, related to the amlodipine component; specific effects of acute atorvastatin overdose are not well characterized but are not expected to be life-threatening based on available data.
If an overdose of Divastin Plus is suspected, seek immediate medical attention or contact a poison control center/emergency services right away. Do not attempt to treat an overdose at home. Supportive care, including cardiovascular monitoring and treatment, will be provided by medical professionals; there is no specific antidote for either component of Divastin Plus.
Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.
No overall dosage adjustment of Divastin Plus is required based on age alone, but elderly patients may be more sensitive to the blood-pressure-lowering effects of amlodipine and are at higher risk of statin-associated myopathy; use with routine caution and monitoring.
No dose adjustment of Divastin Plus is generally required, though patients with significant renal impairment should be monitored (see Dosage and Administration).
Divastin Plus should be used cautiously in hepatic impairment and is contraindicated in active liver disease (see Contraindications and Dosage and Administration).
See Pediatric Uses — safety and effectiveness of Divastin Plus as a fixed-dose combination have not been established in pediatric patients.
See Pregnancy and Lactation — Divastin Plus is contraindicated in both pregnancy and breastfeeding.
Divastin Plus is generally used as a long-term, ongoing treatment for chronic hypertension and dyslipidemia, since both conditions typically require continuous management to sustain blood pressure control and cardiovascular risk reduction. Duration of therapy should be determined by a physician based on periodic reassessment of blood pressure, lipid profile, and tolerability, and Divastin Plus should not be stopped without medical advice.
Calcium channel blocker (dihydropyridine) combined with HMG-CoA reductase inhibitor (statin); antihypertensive–antihyperlipidemic fixed-dose combination.
Amlodipine + Atorvastatin works through two independent, complementary mechanisms. Amlodipine blocks L-type calcium channels in vascular smooth muscle, reducing calcium influx and causing vasodilation, which lowers systemic vascular resistance and blood pressure. Atorvastatin inhibits HMG-CoA reductase, reducing hepatic cholesterol synthesis and upregulating LDL receptors, which increases clearance of LDL-cholesterol from the blood and improves the overall lipid profile.

The safety and effectiveness of Divastin Plus as a fixed-dose combination tablet have not been established in pediatric patients. Divastin Plus should generally not be used in children.
When individually appropriate and prescribed by a specialist, amlodipine alone has been studied for hypertension in children aged 6 to 17 years, and atorvastatin alone is approved for certain pediatric patients aged 10 years and older with specific inherited high-cholesterol conditions; however, this does not extend to the combined Divastin Plus product, which should not be used in pediatric patients outside of specific specialist guidance.
Q: What is Divastin Plus 5 mg+10 mg Tablet used for?
