Menthol Crystal is a topical counter-irritant and cooling agent used in a variety ofover-the-counter external preparations. Its indications, classified by strength of evidence, are:

Established / OTC-monograph approved uses

• Temporary relief of minor muscle aches, joint pain, sprains, and strains: appliedtopically as a cream, gel, ointment, lotion, or patch (often combined with other counter-irritantssuch as methyl salicylate or camphor).
• Temporary relief of minor itching (pruritus): including insect bites and minorskin irritation, in topical anti-itch preparations.
• Temporary relief of cold/cough symptoms: as an inhalant, chest rub, or lozengeingredient, Menthol Crystal produces a cooling sensation in the nose and throat that gives a subjective feelingof easier breathing and helps suppress minor cough irritation; it does not treat the underlyinginfection.
• Oral care / throat preparations: as a flavoring and mild local anesthetic inlozenges, mouthwashes, and oral sprays for temporary relief of minor mouth or throat irritation.

Adjunct / combination use

Menthol Crystal is frequently formulated together with other agents (e.g., camphor, methyl salicylate,eucalyptus oil, benzocaine) for combined counter-irritant, anesthetic, or decongestant-sensationeffects; in such products Menthol Crystal is one contributing ingredient rather than the sole active agent.

Not established / off-label

Menthol Crystal is not an antibiotic, does not treat infection or inflammation at its source, and is notestablished as effective for deep or chronic pain, nerve pain, or as a substitute for prescribedrespiratory therapy. Use for any purpose beyond temporary symptomatic relief should be discussed witha physician.

Topical products contain Menthol as an active ingredient at concentrations thattypically range from about 0.1% to 16% or higher depending on the product type (e.g., lowconcentrations in anti-itch lotions, higher concentrations in muscle-rub ointments, gels, andpatches). Menthol is often combined with other actives such as camphor, methyl salicylate, oreucalyptus oil. Oral lozenges and oral-care products contain Menthol in much smaller amounts as aflavoring/mild anesthetic agent. Exact strength and formulation vary by brand; always check thespecific product label.

Menthol Crystal is a naturally occurring or synthetically produced cyclic monoterpenealcohol, most commonly obtained from peppermint or corn mint oil. It is used topically as acounter-irritant, cooling agent, and mild local anesthetic in over-the-counter preparations formuscle aches, minor itching, and cold/cough symptom relief, and in small amounts as a flavoringagent in oral-care and confectionery products.

Menthol Crystal is not a prescription medicine for systemic disease and is not an antibiotic; it providestemporary symptomatic relief only.

Menthol Crystal belongs to the topical counter-irritant / rubefacient class of agents, also classified as a topical analgesic-cooling agent and, in oral-care products, a mild local anesthetic and flavoring agent.

Mechanism of Action

Menthol activates the cold-sensing TRPM8 (transient receptor potential melastatin 8)ion channel on sensory nerve endings in the skin and mucous membranes. Activation of TRPM8 produces asensation of coolness without an actual significant drop in tissue temperature. This coolingsensation, along with mild activation of pain-modulating pathways, is thought to counter-irritate anddistract from the perception of underlying minor aches, itching, and nasal/throat discomfort. Athigher concentrations Menthol also has mild local anesthetic and antipruritic properties by a weak,transient effect on peripheral nerve conduction.

Pharmacokinetics

Menthol is absorbed to a variable but generally limited extent through intact skin and mucousmembranes; absorption increases with concentration, area of application, and if the skin barrier isbroken. Absorbed Menthol is metabolized in the liver (mainly by glucuronide/sulfate conjugation) andexcreted primarily in urine. At the low systemic exposure produced by labeled topical or lozenge use,clinically significant systemic pharmacologic effects are not expected; toxicity is a concern mainlywith ingestion of concentrated products or excessive/occlusive topical application, especially inyoung children.

Menthol Crystal topical products are applied externally to intact skin as directed on the product label; do not apply to the eyes, mucous membranes, or broken/irritated skin, and do not use with a heating pad or tight bandage. Menthol Crystal lozenges or oral sprays are used in the mouth/throat exactly as directed and are not swallowed whole. Always follow the specific product instructions or a physician's/pharmacist's advice.

Clinically Significant Interactions

• Other topical counter-irritants or anesthetics (e.g., methyl salicylate, camphor,benzocaine, capsaicin): concurrent application with other topical Menthol Crystal-containing orcounter-irritant products on the same skin area can increase local irritation, absorption, and riskof skin reaction; avoid layering multiple counter-irritant products on the same site.
• Heat sources (heating pads, hot water, sun exposure, tight bandaging): not a druginteraction but a well-documented physical interaction — heat significantly increases percutaneousabsorption of Menthol Crystal and the risk of chemical burns; avoid combining.
• High-dose oral salicylates (e.g., in combination products containing methyl salicylate): topical Menthol Crystal-salicylate combination products applied over large areas or under occlusion canincrease systemic salicylate absorption, which may be clinically relevant in patients also taking oralaspirin/salicylates or anticoagulants; use combination products cautiously and avoid large-area orocclusive use.

Inform a physician or pharmacist of all other topical or oral medicines being used before startinga Menthol Crystal-containing product.

When used as directed, Menthol Crystal is generally well tolerated. Reported side effectsinclude:

• Local burning, stinging, or tingling sensation at the application site (common, usually mild andtransient)
• Skin redness (erythema) or irritation
• Contact dermatitis or allergic skin reaction (uncommon)
• Cold or "icy-hot" sensation more intense than expected, especially with excessive application
• Rarely, chemical burns if used with heat sources or under occlusive dressings
• With concentrated inhalant/rub products used near an infant's or young child's face: throattightness, breathing difficulty, or laryngospasm (rare but serious — see Contraindications andPrecautions)

Stop use and consult a physician if excessive irritation, blistering, or an allergic reactionoccurs.

Pregnancy

There are no well-controlled human studies specifically establishing the safety ofMenthol Crystal in pregnancy. When used topically as directed, in the small areas andconcentrations found in typical OTC products, systemic absorption is generally low. Nonetheless,Menthol Crystal should be used during pregnancy only if clearly needed, at the lowest effective amount, andafter consulting a physician, particularly for products combined with other actives (e.g., methylsalicylate) or for large-area/prolonged use.

Lactation

It is not well established whether topically applied Menthol Crystal is excreted in clinically significantamounts in breast milk. Topical use in small areas away from the breast/nipple is generallyconsidered low risk, but a physician should be consulted before use during breastfeeding, especiallyfor combination products or use over large body areas.

External Use Only

Menthol Crystal topical products are for external use only. Avoid contact with the eyes,inner nose, mouth, and mucous membranes; if accidental contact occurs, rinse thoroughly with water.Do not apply to broken, irritated, or infected skin.

Heat and Occlusion

Do not use a heating pad, hot water bottle, or tight bandage over skin treated with Menthol Crystal, andavoid sun exposure or hot showers/baths immediately after application, as this significantlyincreases absorption and the risk of chemical burns.

Use in Infants and Young Children

Concentrated Menthol Crystal-containing rubs, inhalants, or vapor products should not be applied to or nearthe nostrils/face of infants and children under 2 years of age, and should be used with caution inolder young children, because of reports of throat/airway tightening (laryngospasm) and breathingdifficulty. Follow the specific product's age restrictions and a pediatrician's advice.

Excessive or Prolonged Use

Avoid applying Menthol Crystal to large areas of the body, using it more often than directed, or forprolonged periods, as this increases the risk of systemic absorption and toxicity. If symptomspersist for more than 7 days, worsen, or are accompanied by fever, discontinue use and consult aphysician.

Menthol Crystal is not an antibiotic and does not require antimicrobial stewardship counseling; however, itshould always be used exactly as directed on the product label or by a physician, and not shared ina way that departs from labeled use.

Excessive topical application of Menthol Crystal, use under occlusion or with heat, oraccidental ingestion of concentrated Menthol Crystal products (e.g., pure Menthol Crystal crystals, high-concentrationrubs, or essential oils) can cause systemic toxicity. Symptoms may include nausea, vomiting,abdominal pain, drowsiness, ataxia (unsteady gait), confusion, and in severe cases (particularly inyoung children) seizures, loss of consciousness, or respiratory depression.

If overdose or accidental ingestion of a Menthol Crystal-containing product is suspected, seek immediatemedical attention or contact emergency services / a poison control center right away. Do not attemptto induce vomiting or give any home remedy unless specifically instructed by emergency medicalpersonnel or a poison control center.

Store at room temperature (below 30°C), away from light, heat, and moisture. Keep the container tightly closed. Keep Menthol Crystal-containing products, especially concentrated rubs, inhalants, or lozenges, out of reach of children, as accidental ingestion can be harmful.

Infants and Young Children

Concentrated Menthol Crystal rub/inhalant products should not be used on or near the face ornostrils of infants and children under 2 years of age; some products restrict use further (seePediatric Uses and Precautions). Oral lozenges are generally not recommended for young children dueto choking risk.

Elderly

No specific dose adjustment is established for elderly patients using topical Menthol Crystal as directed,but thinner, more fragile skin in some elderly patients may increase local irritation risk; applycautiously and monitor the skin.

Hepatic/Renal Impairment

Specific dose adjustment data are not established, as systemic absorption from labeled topical/lozenge use is normally low; nonetheless, patients with significant hepatic or renal impairmentshould use Menthol Crystal-containing products cautiously and avoid large-area or prolonged use, given thatabsorbed Menthol Crystal is hepatically metabolized and renally excreted.

Menthol Crystal is intended for short-term, as-needed symptomatic relief. If minor aches,itching, or cold/throat symptoms do not improve within about 7 days, or worsen, or are accompanied byfever, rash, or signs of infection, use should be stopped and a physician consulted. Menthol Crystal is notintended for continuous long-term use without medical supervision.

Menthol belongs to the topical counter-irritant/rubefacient and cooling-agent class, with secondary classification as a mild local anesthetic and antipruritic agent used in over-the-counter external and oral-care preparations.
