Neogen Plus(400 IU+3.5 mg+5000 IU)/gm

Neogen Plus is a topical antibacterial combination used for the prevention and treatment of infection in minor skin injuries. Its indications can be classified by strength of evidence as follows:

Established / Well-Recognized Use

• First-aid prevention of bacterial infection in minor cuts, scrapes, and abrasions.
• Prevention of infection in minor thermal (first-degree) burns.
• Adjunctive topical care of superficial, uninfected minor wounds where the skin surface is broken but the wound is small and superficial.

Not Established / Inappropriate Uses

• Neogen Plus is not intended for deep puncture wounds, animal or human bites, serious or extensive burns, or large areas of damaged skin.
• It is not a substitute for medical evaluation of wounds that are deep, heavily contaminated, or showing signs of spreading infection (increasing redness, warmth, swelling, pus, fever).

Neogen Plus does not treat viral or fungal skin infections and has no role in the management of internal or systemic infections.

Neomycin Sulfate + Bacitracin Zinc + Polymyxin B Sulfate is formulated as a topical ointment combining three antibacterial agents that together provide broad antibacterial coverage against common skin pathogens. A typical formulation provides (per gram of ointment):

• Bacitracin Zinc - approximately 400-500 units
• Neomycin Sulfate - equivalent to approximately 3.5 mg neomycin base
• Polymyxin B Sulfate - approximately 5,000 units

The ointment is typically dispensed in a soft paraffin/white petrolatum-type base. Exact strengths may vary slightly by manufacturer; always check the product label of the specific brand dispensed.

Neogen Plus is a fixed-dose topical combination antibiotic ointment. Neogen Plus belongs to the therapeutic class of topical antibacterial combinations (multiple antibiotic, topical), used in first-aid and minor wound care.

Neomycin Sulfate + Bacitracin Zinc + Polymyxin B Sulfate combines three antibacterial agents with complementary and distinct mechanisms of action, giving broader coverage than any single agent alone:

• Neomycin sulfate - an aminoglycoside antibiotic that binds irreversibly to the bacterial 30S ribosomal subunit, causing misreading of mRNA and inhibition of bacterial protein synthesis; primarily bactericidal against many aerobic Gram-negative bacteria and some Gram-positive organisms.
• Bacitracin zinc - a polypeptide antibiotic that inhibits bacterial cell wall synthesis by interfering with the dephosphorylation of the lipid carrier that transports peptidoglycan precursors across the cell membrane; primarily active against Gram-positive organisms such as staphylococci and streptococci.
• Polymyxin B sulfate - a cationic cyclic polypeptide that binds to and disrupts the phospholipid structure of the bacterial cell membrane, increasing permeability and causing cell death; active mainly against Gram-negative organisms including Pseudomonas aeruginosa.

Because topical application results in minimal systemic absorption through intact or minimally broken skin, Neomycin Sulfate + Bacitracin Zinc + Polymyxin B Sulfate acts locally at the site of application rather than systemically.

• Wash hands before and after application.
• Gently clean the affected area with mild soap and water and pat dry before applying Neogen Plus.
• Apply a thin layer of Neogen Plus directly to the wound; a sterile bandage or gauze dressing may be applied over it if desired.
• Do not apply over large areas of skin or to deep or heavily draining wounds.
• If a dose/application is missed, apply as soon as remembered as part of the regular schedule; do not double up.

Because Neogen Plus is applied topically and systemic absorption is normally minimal through intact or minor superficial wounds, clinically significant drug interactions are uncommon. However:

• Concurrent use of other topical antibiotic, antiseptic, or corticosteroid products on the same area should be avoided unless directed by a physician, as this may mask signs of worsening infection or increase the risk of irritation/sensitization.
• When Neogen Plus is applied to large, denuded, or extensively broken skin areas, increased systemic absorption of neomycin may occur; in this situation, concurrent use of other ototoxic or nephrotoxic drugs (e.g., aminoglycoside injections, certain diuretics) could theoretically add to the risk of toxicity - such extensive use should only occur under medical supervision.

The most common adverse effect associated with Neogen Plus is a local allergic or irritant skin reaction. Reported side effects include:

• Allergic contact dermatitis - redness, itching, rash, burning, or swelling at the application site (most frequently attributed to the neomycin component); this can occur even after previously uneventful use.
• Mild, transient stinging or irritation on application.
• Delayed wound healing with prolonged or excessive use.
• Rarely, severe allergic reactions (including generalized rash, hives, or anaphylaxis) have been reported.
• Rarely, with application to large areas of broken skin, sufficient systemic absorption of neomycin may occur to cause ototoxicity (hearing/balance disturbance) or nephrotoxicity (kidney injury) - see Precautions and Warnings.
• Prolonged use may occasionally lead to overgrowth of non-susceptible organisms, including fungi.

Discontinue use immediately and seek medical advice if a rash, itching, or worsening irritation develops.

Pregnancy: There are no well-controlled studies of Neogen Plus in pregnant women. Systemic absorption from normal topical use on small, superficial wounds is expected to be minimal, but Neogen Plus should be used during pregnancy only if clearly needed, in the smallest effective amount and for the shortest duration, and only after consulting a physician, particularly if application to larger or more extensive skin areas is being considered.

Lactation: It is not known whether topically applied Neogen Plus is absorbed in amounts sufficient to appear in breast milk. As a precaution, breastfeeding mothers should consult a physician before use, avoid application to the breast/nipple area unless advised, and use only the amount needed to treat the affected area.

For external use only. Observe the following precautions when using Neogen Plus:

• Use only for minor, superficial wounds and for short durations (generally no more than 7 days) unless a physician directs otherwise. Neogen Plus is not a substitute for medical evaluation of deep, large, puncture, or animal/human bite wounds, or wounds showing signs of spreading infection (fever, increasing redness, warmth, or swelling) - seek prompt medical care for these.
• Do not use in the eyes, as this may cause irritation; a specific ophthalmic formulation, if directed by a physician, should be used for eye infections instead.
• Avoid use on deep puncture wounds, serious or extensive burns, or wounds/areas covering a large portion of the body, since this increases systemic absorption of neomycin and the associated risk of nephrotoxicity or ototoxicity.
• Discontinue use and consult a physician if a rash, itching, redness, or irritation develops, as this may indicate allergic contact sensitization (see Contraindications and Side Effects).
• Do not bandage tightly or use occlusive dressings unless directed by a physician, as this may increase systemic absorption.
• Take/use Neogen Plus exactly as prescribed or as directed by a physician or pharmacist; do not stop, extend, skip applications, or share this medicine with others without medical advice.
• If the condition worsens, does not improve within a few days, or symptoms recur, stop use and consult a physician.

Topical overdose with Neogen Plus is unlikely with normal first-aid use, but applying excessive amounts over large or extensively broken skin areas for prolonged periods may increase the risk of systemic absorption and associated toxicity (such as hearing disturbance or kidney injury). If Neogen Plus is accidentally swallowed, or if signs of an allergic or toxic reaction (such as difficulty breathing, facial swelling, ringing in the ears, dizziness, or decreased urination) occur, seek immediate medical attention or contact a poison control center/emergency services rather than attempting home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep the container tightly closed and out of the reach and sight of children. Do not use after the expiry date printed on the pack.

Pediatric Use

Neogen Plus may generally be used in children 2 years of age and older for minor cuts, scrapes, and burns, following the same application guidance as for adults. Use in children under 2 years should only occur on the advice of a physician (see Pediatric Uses).

Elderly

No specific dose adjustment is generally required in elderly patients for standard topical first-aid use; the same precautions regarding limited area and duration of use apply.

Renal or Hepatic Impairment

Because systemic absorption from normal topical use on small areas is minimal, formal dose adjustment for renal or hepatic impairment is not established. However, patients with renal impairment may be at theoretically higher risk of neomycin-related toxicity if Neogen Plus is applied to large or extensively broken skin areas, and should use it only under medical supervision in that situation.

Neogen Plus is intended for short-term, first-aid use. Treatment is generally limited to a maximum of about 7 days for a given wound unless a physician directs a longer course. If the wound has not improved, or if symptoms worsen or recur within this period, use should be stopped and medical advice sought rather than continuing or repeating treatment on the same area indefinitely.

Parents/caregivers should ensure only a thin layer is applied, avoid use near the eyes or mouth, and watch for any signs of skin irritation or allergic reaction, discontinuing and consulting a physician if these occur.
