Nyclobate is an ultra-high-potency (super-potent, Class I) topical corticosteroid formulated specifically for application to the scalp (as a solution, lotion, shampoo, or foam, depending on the marketed product).

Established / approved use

• Short-term topical treatment of moderate to severe scalp psoriasis in adults.
• Other corticosteroid-responsive dermatoses of the scalp where a super-potent topical steroid is indicated.

Adjunct / commonly used off-label

• As an adjunct in severe seborrheic dermatitis of the scalp unresponsive to lower-potency agents, generally for short courses only.
• Sometimes combined with keratolytic (e.g., salicylic acid) or antifungal shampoos as part of a combination regimen for scaly scalp plaques, under physician direction.

Nyclobate is not intended for widespread, prolonged, or unsupervised use, and is not a first-line agent for mild scalp conditions.

Each formulation of Clobetasol Propionate [Scalp Preparation] contains Clobetasol Propionate USP 0.05% w/w or w/v (approximately 0.5 mg per gram or mL) as the active ingredient, in a vehicle base (solution, lotion, shampoo, or foam) designed for application to hair-bearing scalp skin.

Nyclobate belongs to the super-potent (Group I) class of topical corticosteroids, the highest potency category available. It is specially formulated (as a topical solution, lotion, shampoo, or foam) to spread easily through hair and reach the scalp skin without leaving a greasy residue.

It works by reducing inflammation, redness, scaling, and itching associated with corticosteroid-responsive scalp conditions such as psoriasis. Because of its high potency, its use is restricted to short courses and to the scalp only, under medical supervision.

Super-potent (Class I) Topical Corticosteroid - Scalp Preparation (Nyclobate)

Clobetasol Propionate [Scalp Preparation] contains clobetasol propionate, a synthetic fluorinated corticosteroid with marked anti-inflammatory, antipruritic, and vasoconstrictive activity.

It diffuses across cell membranes and binds to cytoplasmic glucocorticoid receptors. The activated receptor-steroid complex translocates to the nucleus, altering gene transcription and inducing synthesis of lipocortins (annexin A1), which inhibit phospholipase A2. This blocks release of arachidonic acid, reducing downstream production of prostaglandins and leukotrienes — the key inflammatory mediators responsible for erythema, scaling, and pruritus in scalp dermatoses. Clobetasol propionate is classified as a super-potent (Class I) topical steroid, with minimal systemic absorption expected when used as directed on intact scalp skin for short durations.

Nyclobate is for external (topical) use on the scalp only. It must not be applied to the face, eyes, groin, axillae (armpits), or mucous membranes, and must not be swallowed.

Part the hair to apply directly to the affected scalp skin rather than only to the hair shaft. Avoid contact with the eyes; if accidental contact occurs, rinse thoroughly with water. Do not bandage, wrap, or otherwise occlude the treated area unless a physician specifically instructs this. Some formulations (e.g., foam or solution vehicles) may be flammable due to alcohol content — keep away from open flame, lit cigarettes, and other sources of ignition during and immediately after application.

When used correctly on limited scalp areas for short durations, systemic absorption of Nyclobate is minimal and clinically significant drug interactions are uncommon. Recognized concerns include:

• Other topical or systemic corticosteroids: concurrent use increases the combined corticosteroid load and the risk of HPA-axis suppression and Cushingoid effects; avoid combining unless directed by a physician.
• Potent CYP3A4 inhibitors (e.g., ritonavir, itraconazole, ketoconazole): if significant systemic absorption occurs (e.g., with extensive or prolonged use, or use on broken skin), these agents could theoretically raise systemic corticosteroid exposure; use with caution in patients on such therapy.

Most adverse effects of Nyclobate are local and related to its high potency and application site:

• Common: burning, stinging, itching, dryness, or irritation of the scalp; folliculitis.
• Less common: hypertrichosis (increased hair growth), acneiform eruptions, skin atrophy, striae (stretch marks), telangiectasia (visible small blood vessels), hypopigmentation, secondary skin infection, allergic contact dermatitis.
• Rare (with extensive/prolonged use): reversible hypothalamic-pituitary-adrenal (HPA) axis suppression, Cushingoid features, hyperglycemia, and glucosuria — see Precautions and Warnings.

Patients should stop use and consult a physician if irritation, signs of infection, or worsening of the scalp condition occurs.

Pregnancy: Topical corticosteroids such as Nyclobate can be absorbed systemically, and there are no adequate, well-controlled studies in pregnant women. It should be used during pregnancy only if the potential benefit clearly justifies the potential risk to the fetus, and only for the shortest duration and smallest area necessary, under medical supervision. Extensive use or use over large areas during pregnancy should be avoided.

Lactation: It is not known whether topically applied clobetasol propionate is excreted in human breast milk. Caution should be exercised when used by a breastfeeding woman; avoid application to or near the breast/nipple area, and use only if clearly needed and directed by a physician.

Nyclobate is a super-potent topical corticosteroid and requires careful, physician-supervised use:

• HPA-axis suppression: prolonged use, use over large scalp areas, use under occlusion, or use in children can suppress the hypothalamic-pituitary-adrenal axis, potentially producing Cushing syndrome, hyperglycemia, and glucosuria. Treatment should be limited to 2 consecutive weeks unless a physician reassesses and extends it, and total dosage should not exceed 50 g/50 mL per week.
• Local skin changes: prolonged or repeated use can cause skin atrophy, striae, telangiectasia, and delayed wound healing, particularly with occlusion.
• Restricted sites: not for use on the face, groin, axillae, or mucous membranes (see Contraindications); avoid contact with the eyes.
• Infection risk: if a scalp infection is present or develops during treatment, appropriate antimicrobial therapy should be started and the corticosteroid discontinued if response is not prompt.
• Children: pediatric patients are more susceptible to HPA-axis suppression due to a larger skin-surface-to-body-weight ratio; use the smallest effective amount for the shortest possible duration and avoid occlusive dressings.
• Flammability: alcohol-containing vehicles (some foams/solutions) are flammable; avoid heat, flame, and smoking during and immediately after application.
• Avoid abrupt discontinuation after prolonged use; taper under medical guidance if long-term therapy has been used, to reduce risk of rebound flare or adrenal insufficiency.

Excessive or prolonged use of Nyclobate beyond recommended amounts and duration can lead to significant systemic corticosteroid absorption, resulting in HPA-axis suppression, Cushingoid features, hyperglycemia, and glucosuria. Accidental ingestion may cause similar systemic corticosteroid effects.

If overdose, accidental ingestion, or signs of systemic corticosteroid toxicity (e.g., unusual weight gain, moon face, weakness) are suspected, discontinue use and seek immediate medical attention or contact a poison control center. Do not attempt home treatment of a suspected overdose.

Store at room temperature (below 30°C), away from direct light, heat, and moisture. Keep the container tightly closed and out of reach of children. If the formulation is alcohol-based (e.g., foam or solution), keep away from open flame or lit cigarettes.

• Elderly: no specific dose adjustment is established, but the lowest effective amount and shortest duration should be used, as skin may be thinner and more prone to atrophy.
• Renal/hepatic impairment: no formal dose adjustment established for Nyclobate, since systemic exposure from correctly-used scalp application is expected to be minimal; use cautiously if impairment is significant.
• Children: more susceptible to HPA-axis suppression and local skin atrophy; use is generally not recommended in children under 12 years, and only under close physician supervision with the shortest effective course in older children/adolescents.
• Immunocompromised patients: increased risk of masked or worsened skin infection; monitor closely.

Treatment with Nyclobate should generally be limited to a maximum of 2 consecutive weeks. Total weekly use should not exceed 50 g (or 50 mL). If there is no improvement within 2 weeks, the diagnosis should be reassessed by a physician. Continuous, long-term, or unsupervised use should be avoided due to the risk of HPA-axis suppression and skin atrophy; if longer-term intermittent therapy is required, it must be under direct physician supervision.

Topical Corticosteroids; Super-potent (Class I / Group I) Glucocorticoids

Clobetasol Propionate [Scalp Preparation] acts by binding to intracellular glucocorticoid receptors in scalp skin cells. The steroid-receptor complex enters the nucleus and modulates gene expression, inducing synthesis of lipocortin-1 (annexin A1), which inhibits phospholipase A2 and thereby reduces the release of arachidonic acid and downstream inflammatory mediators (prostaglandins, leukotrienes). The net effect is potent anti-inflammatory, antipruritic, and vasoconstrictive activity at the site of application, reducing the redness, scaling, and itching characteristic of scalp psoriasis and other corticosteroid-responsive scalp dermatoses.

Safety and efficacy of Nyclobate in children under 12 years of age have not been established, and its use in this age group is generally not recommended. Children have a larger skin-surface-area-to-body-weight ratio than adults, making them more susceptible to HPA-axis suppression, Cushing syndrome, and growth suppression with topical corticosteroid use.

If used in adolescents (12-17 years) under physician direction, treatment should be limited to the smallest effective area, the lowest effective frequency, and the shortest possible duration, with monitoring for signs of local and systemic corticosteroid effects.
