Indications

OMG-3 is FDA-approved as an adjunct to diet for the reduction of very high triglyceride (TG) levels in adult patients with severe hypertriglyceridemia (fasting TG ≥ 500 mg/dL).

• Established/approved use: Adjunct to dietary triglyceride and fat restriction in adults with severe hypertriglyceridemia, to reduce triglyceride levels and reduce the risk of pancreatitis associated with very high triglycerides.
• Guideline-supported adjunct use: Used alongside statin therapy in mixed dyslipidemia when triglycerides remain markedly elevated despite dietary measures and LDL-directed therapy.
• Not established: OMG-3 has not been shown to reduce the risk of major cardiovascular events (myocardial infarction, stroke) in the general population; it should not be used solely as cardioprotective therapy.

Before starting OMG-3, secondary causes of hypertriglyceridemia (poorly controlled diabetes mellitus, hypothyroidism, nephrotic syndrome, dysproteinemia, obstructive liver disease, alcohol use, or certain medications) should be ruled out or treated.

Composition

Each capsule of Omega-3 Acid Ethyl Esters contains approximately 1 gram of omega-3 fatty acids as ethyl esters, principally:

• Eicosapentaenoic acid (EPA) ethyl ester — approximately 465 mg
• Docosahexaenoic acid (DHA) ethyl ester — approximately 375 mg

The active ingredients are derived and purified from fish oil sources.

Description

OMG-3 is a prescription-strength, purified and concentrated formulation of omega-3 polyunsaturated fatty acids (EPA and DHA) presented as ethyl esters, supplied as an oral soft-gelatin capsule. Unlike over-the-counter fish oil supplements, OMG-3 is manufactured to pharmaceutical purity and potency standards and has been evaluated in controlled clinical trials for the treatment of severe hypertriglyceridemia.

OMG-3 works primarily in the liver to reduce the production and increase the clearance of triglyceride-rich lipoproteins, lowering circulating triglyceride levels.

Therapeutic Class

OMG-3 belongs to the antihyperlipidemic (lipid-regulating) class of agents, specifically the omega-3 fatty acid derivatives used for triglyceride reduction.

Pharmacology / Mechanism of Action

The precise mechanism by which Omega-3 Acid Ethyl Esters lowers triglyceride levels has not been fully elucidated. Proposed mechanisms, based on animal and in-vitro data, include:

• Inhibition of acyl-CoA:1,2-diacylglycerol acyltransferase and reduced hepatic triglyceride synthesis.
• Increased hepatic mitochondrial and peroxisomal beta-oxidation of fatty acids, reducing available substrate for triglyceride synthesis.
• Reduced hepatic secretion of very-low-density lipoprotein (VLDL) triglycerides.
• Increased plasma lipoprotein lipase activity, enhancing clearance of triglyceride-rich lipoproteins.

Omega-3 Acid Ethyl Esters may increase low-density lipoprotein cholesterol (LDL-C) in some patients while lowering triglycerides; the effect on LDL-C is variable and dose-related.

Administration

OMG-3 capsules should be swallowed whole with water and should not be broken open, crushed, dissolved, or chewed. OMG-3 may be taken with or without food; taking it with meals may help minimize burping and gastrointestinal upset. Doses should be spaced evenly if given twice daily, and patients should continue prescribed dietary modification alongside treatment.

Drug Interactions

• Anticoagulants (e.g., warfarin) and antiplatelet agents (e.g., aspirin, clopidogrel): OMG-3 has mild antiplatelet activity and may modestly prolong bleeding time. Concurrent use with anticoagulant or antiplatelet therapy may increase the risk of bleeding; patients on such combinations should be monitored periodically for signs of bleeding, and anticoagulated patients may need periodic INR monitoring.

No other well-established, clinically significant drug interactions have been documented for OMG-3.

Side Effects

The most commonly reported adverse effects with OMG-3 include:

• Eructation (burping) and fishy aftertaste
• Dyspepsia (indigestion) and taste perversion
• Nausea
• Diarrhea
• Back pain
• Skin reactions such as rash or eczema (uncommon)

Less common but clinically important effects include elevated liver enzymes (ALT), increased LDL cholesterol, and an increased risk of atrial fibrillation or atrial flutter, particularly in patients with pre-existing cardiovascular disease (see Precautions and Warnings). Patients should seek prompt medical attention for palpitations, irregular heartbeat, unusual bleeding or bruising, or signs of an allergic reaction (rash, swelling, difficulty breathing).

Pregnancy and Lactation

Data on the use of OMG-3 in pregnant women are limited. OMG-3 should be used during pregnancy only if clearly needed and if the potential benefit to the mother justifies the potential risk to the fetus; a physician should always be consulted before use in pregnancy.

It is not known whether the ethyl ester components of OMG-3 are excreted in human milk in clinically significant amounts. Caution is advised when OMG-3 is administered to a breastfeeding woman, and a physician should be consulted before use during breastfeeding.

Precautions and Warnings

• Liver function: OMG-3 can increase levels of ALT (alanine aminotransferase) without a concurrent increase in AST. Periodic monitoring of liver function tests is recommended, particularly in patients with hepatic impairment.
• LDL-cholesterol increase: OMG-3 may raise LDL-C in some patients even as it lowers triglycerides. A lipid panel, including LDL-C, should be checked periodically during treatment.
• Bleeding risk: OMG-3 has mild antiplatelet effects; patients on anticoagulant or antiplatelet therapy should be monitored for bleeding (see Drug Interactions).
• Atrial fibrillation/flutter: An increased risk of atrial fibrillation or atrial flutter has been observed with omega-3 fatty acid therapy, particularly in patients with a history of cardiovascular disease or prior arrhythmia. Patients should be advised to report palpitations, irregular heartbeat, dizziness, or chest discomfort promptly.
• Fish/shellfish sensitivity: Because OMG-3 is derived from fish sources, it should be used with caution in patients with known or suspected hypersensitivity to fish or shellfish (see Contraindications).
• OMG-3 is not a substitute for dietary modification, weight management, exercise, or treatment of underlying/secondary causes of hypertriglyceridemia (e.g., poorly controlled diabetes, hypothyroidism).

Overdose

There is limited clinical experience with overdosage of OMG-3. No specific antidote exists. In case of a suspected overdose, seek immediate medical attention or contact a poison control center or emergency services; treatment should be supportive and symptomatic, with monitoring as clinically indicated.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Use in Special Populations

• Hepatic impairment: OMG-3 should be used with caution; periodic monitoring of liver enzymes (particularly ALT) is recommended.
• Renal impairment: No specific dose adjustment has been established; use with routine clinical monitoring.
• Elderly: No overall differences in safety or effectiveness have been observed between elderly and younger adult patients; no dose adjustment is required based on age alone.
• Pediatric patients: Safety and efficacy of OMG-3 have not been established in pediatric patients.
• Pregnancy and lactation: See Pregnancy and Lactation section.

Duration of Treatment

Treatment with OMG-3 is typically long-term, as hypertriglyceridemia is a chronic condition requiring ongoing management. Response to therapy (triglyceride and LDL-C levels) should be reassessed periodically, generally within 2–3 months of starting or adjusting treatment, and treatment continued or modified based on lipid response, tolerability, and continued clinical need, together with dietary and lifestyle measures.

Mode of Action

The exact mode of action of Omega-3 Acid Ethyl Esters is not completely understood. It is believed to reduce hepatic triglyceride synthesis, increase fatty acid beta-oxidation, reduce VLDL secretion, and increase lipoprotein lipase activity, together lowering circulating triglyceride concentrations.

Pediatric Use

The safety and efficacy of OMG-3 have not been established in pediatric patients. Use in children and adolescents is not routinely recommended outside of specialist guidance, and secondary causes of hypertriglyceridemia should be thoroughly investigated in this age group before considering pharmacologic therapy.
