OTO-4 is a fixed-dose combination otic (ear) preparation indicated for the topical treatment of ear infections associated with pain and inflammation caused by susceptible bacteria and fungi.

Established uses

• Acute otitis externa (infection of the outer ear canal, "swimmer's ear") with an intact tympanic membrane, due to susceptible bacterial and/or fungal (e.g. Candida) organisms, particularly when mixed infection and associated inflammation/pain are present.
• Otomycosis (fungal infection of the ear canal) with secondary bacterial involvement.
• Chronic suppurative otitis media (discharging middle-ear infection with a perforated tympanic membrane), in patients specifically indicated for this use per product labeling (typically adolescents and adults).

Adjunct role

The corticosteroid (beclomethasone) and local anesthetic (lidocaine) components are adjuncts within this combination that reduce inflammation and provide symptomatic pain relief while the antimicrobial components treat the underlying infection; they are not used as stand-alone therapy for ear infection.

OTO-4 should only be used for infections in which mixed bacterial-fungal etiology with inflammation is suspected or confirmed, as directed by a physician.

Each mL of Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine ear drop solution/suspension typically contains:

• Ofloxacin BP ................ 3 mg (0.3% w/v)
• Clotrimazole BP ............. 10 mg (1% w/v)
• Beclomethasone Dipropionate BP ... 0.25 mg (0.025% w/v)
• Lidocaine Hydrochloride BP .. 20 mg (2% w/v)

Formulated as sterile otic drops for topical use in the ear only.

OTO-4 is a topical, multi-component ear drop combining a fluoroquinolone antibiotic (ofloxacin), an imidazole antifungal (clotrimazole), a corticosteroid (beclomethasone dipropionate), and an amide-type local anesthetic (lidocaine).

This combination is designed to provide broad antibacterial and antifungal coverage against the organisms most commonly responsible for otitis externa and related ear infections, while the corticosteroid reduces local inflammation/swelling and the local anesthetic provides rapid relief of ear pain (otalgia) and itching associated with the infection.

OTO-4 is intended for short-term topical use in the ear canal only and is not for oral, ophthalmic, or injectable use.

OTO-4 belongs to the therapeutic class of topical otic (ear) anti-infective and anti-inflammatory combinations — combining a fluoroquinolone antibacterial, an imidazole antifungal, a topical corticosteroid, and a local anesthetic in a single ear-drop formulation.

Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine works through the complementary actions of its four components:

Ofloxacin

A fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication and leading to bactericidal activity against a broad range of gram-positive and gram-negative organisms commonly implicated in ear infections, including Pseudomonas aeruginosa and Staphylococcus aureus.

Clotrimazole

An imidazole antifungal that inhibits ergosterol synthesis in the fungal cell membrane, increasing membrane permeability and causing leakage of cell contents; active against Candida species and dermatophytes commonly responsible for otomycosis.

Beclomethasone Dipropionate

A synthetic corticosteroid that reduces local inflammation, redness, and swelling by suppressing the release of inflammatory mediators (prostaglandins, leukotrienes) and inhibiting phospholipase A2 activity.

Lidocaine

An amide-type local anesthetic that blocks voltage-gated sodium channels in sensory nerve endings of the ear canal, providing rapid, temporary relief of pain (otalgia) and pruritus.

Together, these actions allow Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine to treat the infection, reduce inflammation, and relieve associated symptoms simultaneously.

To administer OTO-4 ear drops:

• Warm the bottle slightly by holding it in the hand for a few minutes to reduce dizziness from cold drops; shake well if the product is a suspension.
• Tilt the head sideways or lie down with the affected ear facing upward.
• Gently pull the earlobe up and back (adults) or down and back (young children) to straighten the ear canal.
• Instill the prescribed number of drops into the ear canal without letting the dropper tip touch the ear.
• Keep the head tilted or remain lying down for 3-5 minutes to allow the drops to reach the affected area; gently press the small flap (tragus) in front of the ear a few times to help the drops move in.
• Avoid inserting cotton or other objects into the ear immediately after application unless advised by the physician.
• Wash hands before and after use, and avoid contaminating the dropper tip.

Because OTO-4 is applied topically to the ear with minimal systemic absorption, clinically significant drug interactions are uncommon. Nonetheless, the following should be noted:

• Other ototopical or ototoxic agents: Concurrent use with other ear preparations (especially aminoglycoside-containing otic products) should be avoided unless directed by a physician, to minimize the risk of additive local irritation or ototoxicity.
• Other topical ear/skin products: Simultaneous use of other topical antibiotics, antifungals, or corticosteroids in the same ear is not recommended without medical advice, as it may increase the risk of local irritation or mask signs of worsening infection.
• Systemic fluoroquinolone interactions (e.g., with antacids, QT-prolonging drugs) are not expected to be clinically relevant with topical/otic use, but should still be mentioned to the physician if the patient is on multiple systemic medications, particularly with prolonged use over broken skin/perforated eardrum where absorption may be greater.

Always inform the physician or pharmacist about all other medicines (including other ear drops) being used before starting OTO-4.

Most side effects of OTO-4 are local and mild, related to application in the ear canal. Reported side effects include:

• Local burning, stinging, or itching sensation in the ear at the time of application
• Ear irritation, dryness, or redness of the ear canal
• Earache or discomfort
• Transient taste disturbance (bitter taste) if drops reach the throat via a perforated eardrum
• Headache, dizziness, or vertigo (uncommon)
• Folliculitis or local skin reactions with prolonged use
• Hypersensitivity/allergic contact dermatitis (uncommon but can occur with prolonged use)

If severe itching, rash, swelling, or worsening symptoms occur, discontinue use and consult a physician promptly.

The safety of OTO-4 during pregnancy and breastfeeding has not been well established through controlled studies. Although systemic absorption from topical ear application is expected to be minimal, OTO-4 should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus, and only under medical supervision.

It is not known whether the components of OTO-4 are excreted in human breast milk in clinically significant amounts after topical otic use. Caution is advised, and a physician should be consulted before use during breastfeeding.

Before and during use of OTO-4, the following precautions should be observed:

• Use OTO-4 exactly as prescribed; do not stop, extend, skip doses, or share this medicine with others without medical advice, and use it only for as long as directed to reduce the risk of antimicrobial resistance and local side effects.
• Discontinue and consult a physician if there is no improvement within 5-7 days of treatment, or if symptoms worsen.
• Prolonged or repeated use can lead to local sensitization or allergic contact dermatitis, and may also permit overgrowth of non-susceptible organisms (bacterial or fungal superinfection); the corticosteroid component may mask signs of a worsening or secondary infection with prolonged use.
• Use with caution in patients with a perforated tympanic membrane, particularly young children, since safety and the potential for absorption/ototoxicity in this setting is not fully established outside the specific approved age group; use only under physician guidance in such cases.
• OTO-4 is for use in the ear only — it must not be swallowed, injected, or used in the eyes.
• Avoid contaminating the dropper tip; do not touch it to the ear or any surface.
• Inform the physician of any known allergy to fluoroquinolones, imidazole antifungals, corticosteroids, or local anesthetics before starting treatment.

Topical overdose of OTO-4 in the ear is unlikely to cause serious systemic toxicity due to minimal systemic absorption; excessive local use may increase the risk of local irritation or sensitization.

If OTO-4 is accidentally swallowed or a significantly larger amount than prescribed is used, or if signs of a serious reaction (such as severe dizziness, allergic reaction, or difficulty breathing) occur, seek immediate medical attention or contact a poison control center/emergency services. Do not attempt unsupervised home treatment for a suspected overdose.

Store OTO-4 below 25°C, protected from light and moisture. Do not freeze. Keep the container tightly closed and out of the reach and sight of children. Discard any unused portion after the period recommended on the label or by the pharmacist once opened.

Renal impairment: No dose adjustment of OTO-4 is generally required for topical otic use, as systemic absorption is minimal; however, caution is advised in patients with significant renal impairment if used extensively or with a perforated eardrum.

Hepatic impairment: No specific dose adjustment is established; use with caution as with renal impairment.

Elderly: No specific dose adjustment is required; use as directed by the physician.

Pediatric and geriatric use: See Pediatric Uses section below for age-specific guidance.

Treatment with OTO-4 is typically continued for 7 to 14 days, depending on the indication and clinical response, as directed by the physician. If symptoms do not improve within 5-7 days, the physician should be consulted to reassess the diagnosis and treatment. Treatment should not be extended beyond the prescribed duration without medical advice.

Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine is a fixed-dose combination of: a fluoroquinolone antibiotic (ofloxacin), an imidazole antifungal (clotrimazole), a topical corticosteroid (beclomethasone dipropionate), and an amide-type local anesthetic (lidocaine).

Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine acts through four simultaneous mechanisms: ofloxacin inhibits bacterial DNA gyrase/topoisomerase IV to kill susceptible bacteria; clotrimazole inhibits fungal ergosterol synthesis to kill or inhibit fungi; beclomethasone dipropionate suppresses local inflammatory mediator release to reduce swelling and redness; and lidocaine blocks sodium channels in sensory nerves to relieve ear pain and itching. This combined mechanism allows Ofloxacin + Clotrimazole + Beclomethasone + Lidocaine to address infection, inflammation, and pain together in a single topical preparation.

Use of OTO-4 in children should be guided by a pediatrician or physician:

• Acute otitis externa with intact eardrum: May be used in children 6 months of age and older, at the standard dose, under medical supervision.
• Chronic suppurative otitis media (perforated eardrum): Generally reserved for adolescents (commonly 12 years and older) and adults per product labeling; safety and efficacy in younger children with a perforated eardrum have not been well established.
• Infants under 6 months: Safety and efficacy of OTO-4 have not been established; use is generally not recommended in this age group.

Parents/caregivers should strictly follow the physician's instructions regarding dose, frequency, and duration in children.
