Prodigest capsules should be swallowed whole with water. Sachets/powder formulations should be mixed with a small amount of cool or lukewarm (not hot) water, milk, or food immediately before consumption, as heat can reduce viability of the live organisms. Prodigest may be taken with or without food, though taking it with a meal may improve tolerability. If used with antibiotics, separate dosing by about 2 hours.

Antibiotics/antimicrobials: Concomitant antibiotic therapy can reduce the viability and effectiveness of the live organisms in Prodigest; space administration of Prodigest apart from antibiotic doses by approximately 2 hours.

Immunosuppressants/antifungal agents: No major pharmacokinetic drug interactions are well established for Prodigest; however, caution is advised in immunosuppressed patients (see Contraindications and Precautions).

No other clinically significant, well-verified drug interactions have been established for Prodigest. Theoretical interactions have not been consistently demonstrated in clinical practice.

Prodigest is generally well tolerated. Reported side effects include:

• Common: mild gas, bloating, and flatulence, particularly during the first few days of use; these typically resolve with continued use.
• Less common: mild abdominal discomfort or changes in stool consistency.
• Rare but serious: in immunocompromised individuals or those with central venous catheters, systemic infection (bacteremia/fungemia) has been reported (see Contraindications).

Discontinue Prodigest and seek medical attention if fever, persistent vomiting, or signs of systemic infection develop while using it.

Prodigest is generally considered low risk for use during pregnancy and breastfeeding, as the constituent organisms are not systemically absorbed to a clinically significant degree. However, formal controlled safety data specific to Prodigest in pregnancy and lactation are limited. As with any supplement, Prodigest should be used during pregnancy and breastfeeding only if clearly needed, and a physician should be consulted before use to weigh potential benefit against any theoretical risk.

Use Prodigest with caution in patients with a compromised immune system, structural heart valve disease, indwelling medical devices (e.g., central venous catheters), or those who are critically ill, given a rare but reported risk of systemic infection with probiotic organisms in these groups (see Contraindications).

Discontinue Prodigest and seek prompt medical attention if fever, chills, or other signs of infection develop during use.

Prodigest is not an antibiotic and should not be used as a substitute for prescribed antimicrobial therapy in serious bacterial infections. If used concurrently with antibiotics, separate dosing by approximately 2 hours (see Interactions).

Sachet/powder forms of Prodigest should not be mixed with hot liquids or food, as heat can kill the live organisms and reduce efficacy.

Prodigest has a wide margin of safety, and significant toxicity from overdose is unlikely given it is a supplement of live microorganisms rather than a pharmacologic agent. Excessive intake may cause increased gastrointestinal symptoms such as bloating, gas, or loose stools. If a large overdose is taken, particularly in an immunocompromised individual, or if any concerning symptoms (such as fever, persistent vomiting, or signs of infection) develop, seek immediate medical attention or contact a poison control center/emergency services rather than attempting home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children. Some formulations of probiotic products may require refrigeration to preserve organism viability; check the specific product label for storage instructions, as this can vary by brand and formulation.

Elderly: Prodigest can generally be used in elderly patients at standard adult doses; no specific dose adjustment is established, though age-related immune decline warrants awareness of the rare infection risk described in Contraindications.

Renal impairment: No dose adjustment is established as necessary for Prodigest in renal impairment, as it is not systemically absorbed or renally cleared in a clinically significant manner.

Hepatic impairment: No dose adjustment is established as necessary for Prodigest in hepatic impairment.

Immunocompromised patients: Prodigest is contraindicated in severe immunocompromise (see Contraindications).

Duration of Prodigest use depends on the indication. For general digestive health support, Prodigest may be used on an ongoing basis as directed. For prevention of antibiotic-associated diarrhea, Prodigest is typically continued throughout the antibiotic course and for several days (commonly up to 1-2 weeks) after completion. For acute infectious diarrhea, Prodigest is typically used for the duration of the acute episode (usually 5-7 days) unless otherwise advised by a physician.

Safety and efficacy of specific strain combinations and CFU strengths in Prodigest have not been formally established for all pediatric age groups, particularly in infants under a few months of age or in preterm/very low birth weight infants, in whom rare cases of probiotic-related sepsis have been reported. Prodigest should be used in children only under medical or pharmacist guidance, using age-appropriate formulations and doses.
