Risperdal Consta 50 mg/vial is an atypical antipsychotic medicine prescribed for the management of a range of psychiatric conditions. It works by restoring the balance of natural chemical messengers (neurotransmitters) in the brain, helping to reduce disturbed thoughts, mood swings, and behavioural symptoms. Risperdal Consta 50 mg/vial is indicated for the treatment of:

• Acute and chronic psychoses — conditions marked by a loss of contact with reality, including hallucinations and delusions.
• Mania — episodes of elevated mood, overactivity, and irritability, commonly seen in bipolar disorder.
• Schizophrenia — a long-term mental health disorder affecting thoughts, emotions, and behaviour, including both positive symptoms (such as hallucinations) and negative symptoms (such as social withdrawal).

Risperdal Consta 50 mg/vial belongs to the class of atypical neuroleptic (antipsychotic) drugs.

Risperdal Consta 50 mg/vial is a selective monoaminergic antagonist with a high affinity for serotonergic 5-HT₂ and dopaminergic D₂ receptors. It also binds to alpha₁-adrenergic receptors and, with lower affinity, to H₁ histaminergic and alpha₂-adrenergic receptors. Risperdal Consta 50 mg/vial has no affinity for cholinergic receptors.

As a potent D₂ antagonist, Risperdal Consta 50 mg/vial improves the positive symptoms of schizophrenia (such as hallucinations and delusions) while causing comparatively less suppression of motor activity than conventional antipsychotics. Its balanced antagonism of central serotonin and dopamine is thought to reduce the likelihood of extrapyramidal side effects and to extend therapeutic benefit to the negative and affective symptoms of schizophrenia, such as blunted emotion, social withdrawal, and depressed mood.

Risperdal Consta 50 mg/vial may interact with several other medicines, altering their effects or its own. Notable interactions include:

• Levodopa and dopamine agonists: Risperdal Consta 50 mg/vial may antagonise (reduce) their effects.
• Carbamazepine: Long-term (chronic) co-administration reduces the plasma clearance of Risperdal Consta 50 mg/vial, which may affect its levels.
• Clozapine: Chronic co-administration may decrease the clearance of Risperdal Consta 50 mg/vial.
• Antihypertensives: Risperdal Consta 50 mg/vial may enhance the blood-pressure-lowering effects of certain antihypertensive medicines.

Inform your doctor about all prescription medicines, over-the-counter drugs, and supplements you are taking before starting Risperdal Consta 50 mg/vial.

Like all medicines, Risperdal Consta 50 mg/vial can cause side effects, although not everyone experiences them. Commonly reported side effects include insomnia, agitation, anxiety, and headache.

Less common side effects that have been reported include:

• Drowsiness, impaired concentration, and fatigue
• Blurred vision
• Constipation, nausea, vomiting, dyspepsia (indigestion), and abdominal pain
• Hyperprolactinaemia (raised prolactin levels) and urinary incontinence
• Tachycardia (rapid heartbeat), hypertension (raised blood pressure), and oedema (swelling)
• Skin rash and rhinitis (runny or blocked nose)
• Cerebrovascular accident (stroke)
• Neutropenia and thrombocytopenia (reductions in certain blood cells)

Consult your doctor if any side effect becomes severe, persistent, or troublesome.

Pregnancy: In experimental animals, Risperdal Consta 50 mg/vial did not show direct reproductive toxicity, though some indirect, prolactin- and CNS-mediated effects were observed. No teratogenic (birth-defect-causing) effect was noted in any study. However, the safety of Risperdal Consta 50 mg/vial during human pregnancy has not been established, so it should be used only if the potential benefit justifies the potential risk to the foetus.

Lactation: In animal studies, Risperdal Consta 50 mg/vial and its active metabolite 9-hydroxyRisperdal Consta 50 mg/vial are excreted in milk, and both have also been demonstrated to be excreted in human breast milk. Therefore, women receiving Risperdal Consta 50 mg/vial should not breastfeed.

Special caution is required when using Risperdal Consta 50 mg/vial in the following situations:

• Pre-existing cardiovascular (heart and blood vessel) diseases.
• Discontinue use if signs and symptoms of tardive dyskinesia (involuntary, repetitive body movements) occur.
• Renal (kidney) and hepatic (liver) impairment.
• Elderly patients.
• Epilepsy.
• Parkinson's disease.
• Pregnancy.

Patients and caregivers should be advised to report any unusual movements, mood changes, or other concerning symptoms to a physician promptly.

Drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms may occur. In case of severe extrapyramidal symptoms, anticholinergic medication should be administered. Close medical supervision and monitoring should continue until the patient recovers. There is no specific antidote to Risperdal Consta 50 mg/vial.

Do not store above 30°C. Keep Risperdal Consta 50 mg/vial away from light and out of the reach of children. Do not use the medicine after its expiry date.
