Sertal 25 mg is a selective serotonin reuptake inhibitor (SSRI) approved for the treatment of several psychiatric conditions in adults, and for one indication in pediatric patients. Evidence and approval status vary by indication:

FDA-approved / established indications (adults)

• Major depressive disorder (MDD) - first-line treatment for moderate to severe depressive episodes.
• Obsessive-compulsive disorder (OCD).
• Panic disorder, with or without agoraphobia.
• Post-traumatic stress disorder (PTSD).
• Social anxiety disorder (social phobia).
• Premenstrual dysphoric disorder (PMDD), given continuously or during the luteal phase only.

FDA-approved pediatric indication

• Obsessive-compulsive disorder in children and adolescents aged 6-17 years. Sertal 25 mg is not approved for pediatric major depressive disorder.

Guideline-supported / commonly used off-label uses

• Generalized anxiety disorder (GAD) - widely used based on clinical guidelines though not a formal FDA indication.
• Other anxiety-spectrum and depressive presentations at a clinician's discretion, based on individual risk-benefit assessment.

Use of Sertal 25 mg should always be guided and monitored by a qualified physician.

Each tablet/capsule contains Sertal 25 mg (as Sertal 25 mg hydrochloride) as the active ingredient, in strengths typically of 25 mg, 50 mg, or 100 mg. Inactive ingredients vary by manufacturer and formulation; consult the specific product label for excipient details.

Sertal 25 mg is a selective serotonin reuptake inhibitor (SSRI), a class of antidepressant medicines that work by increasing the availability of serotonin (a neurotransmitter) in the brain. Sertal 25 mg is used to treat depression and a range of anxiety-related disorders, and is one of the most commonly prescribed antidepressants worldwide because of its relatively favorable side-effect and interaction profile compared with older antidepressant classes. It is taken orally, usually once daily, and full therapeutic benefit typically develops over several weeks of continuous use.

Sertal 25 mg belongs to the therapeutic class of Selective Serotonin Reuptake Inhibitors (SSRIs), a subclass of antidepressant medications.

Sertal 25 mg selectively and potently inhibits the neuronal reuptake of serotonin (5-hydroxytryptamine, 5-HT) in the central nervous system, with only minimal effect on norepinephrine and dopamine reuptake at therapeutic doses. By blocking the serotonin transporter (SERT), Sertal 25 mg increases the concentration of serotonin available at the synapse, which over time leads to downstream adaptive changes in serotonergic receptor sensitivity that are believed to underlie its antidepressant and anti-anxiety effects.

Sertal 25 mg is well absorbed after oral administration, undergoes extensive first-pass hepatic metabolism primarily via CYP3A4 and CYP2C19 (with contributions from other CYP enzymes), and is largely inactivated to N-desmethylSertal 25 mg. It has a plasma elimination half-life of approximately 26 hours (somewhat longer in the weakly active metabolite), supporting once-daily dosing. It is highly bound to plasma proteins (~98%).

Take Sertal 25 mg by mouth once daily, at approximately the same time each day, with or without food. Swallow tablets/capsules whole with water; do not crush or chew unless the specific product formulation permits it. If a dose is missed, take it as soon as remembered unless it is close to the time of the next dose, in which case skip the missed dose - do not double the dose.

Sertal 25 mg has clinically significant interactions with several drug classes, primarily due to serotonergic and bleeding-risk mechanisms:

• Monoamine oxidase inhibitors (MAOIs): Concomitant use is contraindicated. Combining Sertal 25 mg with an MAOI (or starting Sertal 25 mg within 14 days of stopping an MAOI, or starting an MAOI within 14 days of stopping Sertal 25 mg) can cause serotonin syndrome, which may be life-threatening.
• Other serotonergic drugs (triptans, tramadol, other SSRIs/SNRIs, tricyclic antidepressants, lithium, buspirone, St. John's Wort, fentanyl): increased risk of serotonin syndrome when combined with Sertal 25 mg. Use with caution and monitor closely if co-administration is necessary.
• Pimozide: Contraindicated due to risk of QT prolongation; Sertal 25 mg increases pimozide plasma levels.
• NSAIDs, aspirin, warfarin, and other anticoagulants/antiplatelets: Sertal 25 mg increases the risk of bleeding (including gastrointestinal bleeding) when combined with these agents, due to impaired platelet serotonin function combined with the anticoagulant/antiplatelet effect.
• CYP2D6 substrates (e.g., certain tricyclic antidepressants, Type 1C antiarrhythmics such as flecainide and propafenone): Sertal 25 mg is a moderate CYP2D6 inhibitor and can raise the plasma levels of these drugs.
• CNS depressants and alcohol: may enhance sedative effects; caution advised.
• Highly protein-bound drugs (e.g., warfarin): may displace or be displaced from protein binding sites, altering free drug concentrations.

Always inform your physician of all medicines, supplements, and herbal products being used before starting Sertal 25 mg.

Most side effects of Sertal 25 mg are dose-related and tend to diminish with continued use. Report persistent or severe symptoms to a physician.

Common side effects

• Nausea, diarrhea, dyspepsia, dry mouth, decreased appetite
• Insomnia, somnolence (drowsiness), dizziness, headache, tremor
• Increased sweating
• Sexual dysfunction, including delayed ejaculation and decreased libido
• Fatigue

Less common but serious side effects (seek prompt medical attention)

• Signs of serotonin syndrome: agitation, hallucinations, rapid heartbeat, fever, muscle stiffness/twitching, loss of coordination, nausea, vomiting, diarrhea
• Unusual bleeding or bruising
• Signs of low sodium (hyponatremia): headache, confusion, weakness, unsteadiness (more common in elderly patients)
• Activation of mania or hypomania in patients with underlying bipolar disorder
• New or worsening suicidal thoughts or behavior, especially in children, adolescents, and young adults (see Precautions and Warnings)
• Seizures
• Allergic reactions: rash, swelling, difficulty breathing

Pregnancy: Sertal 25 mg should be used during pregnancy only if the potential benefit to the mother clearly justifies the potential risk to the fetus. Data have not shown Sertal 25 mg to be well-established as safe in pregnancy; some studies suggest a possible small increased risk of certain birth defects with first-trimester SSRI exposure, and use late in the third trimester has been associated with neonatal complications (respiratory distress, feeding difficulty, jitteriness) and, rarely, persistent pulmonary hypertension of the newborn. Abrupt discontinuation during pregnancy is not recommended without medical guidance, as untreated depression itself carries risk. Consult a physician before starting, stopping, or continuing Sertal 25 mg during pregnancy.

Lactation: Sertal 25 mg is excreted into human breast milk in small amounts. It should be used during breastfeeding only if clearly needed, with the infant monitored for possible effects (irritability, poor feeding, sedation). Discuss the risks and benefits with a physician before use while breastfeeding.

Boxed warning: suicidality risk

Sertal 25 mg and other antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults (up to age 24), compared with placebo, particularly during the first few months of treatment and following dose changes. Patients of all ages started on Sertal 25 mg should be monitored closely for clinical worsening, unusual changes in behavior, and emergence of suicidal thoughts, especially early in treatment. Sertal 25 mg is not approved for use in pediatric patients with major depressive disorder.

Serotonin syndrome

Serotonin syndrome is a potentially life-threatening risk with Sertal 25 mg, particularly when combined with other serotonergic agents (see Interactions). Discontinue Sertal 25 mg immediately if serotonin syndrome is suspected and seek emergency medical care.

Bleeding risk

Sertal 25 mg may increase the risk of bleeding events, particularly when combined with NSAIDs, aspirin, warfarin, or other drugs affecting coagulation. Use with caution in patients at increased bleeding risk.

Discontinuation symptoms

Abrupt discontinuation of Sertal 25 mg can cause discontinuation symptoms such as dizziness, irritability, anxiety, sensory disturbances (e.g., electric shock sensations), sleep disturbance, and mood changes. Sertal 25 mg should be tapered gradually under medical supervision rather than stopped suddenly.

Activation of mania/hypomania

Use with caution in patients with a personal or family history of bipolar disorder; Sertal 25 mg may precipitate a manic or hypomanic episode.

Other precautions

• Use with caution in patients with hepatic or renal impairment (dose adjustment may be needed).
• Use with caution in patients with a seizure disorder.
• May cause hyponatremia, especially in elderly patients and those on diuretics.
• May impair mental/physical abilities required for driving or operating machinery; use caution until individual response is known.
• Angle-closure glaucoma: SSRIs may cause pupillary dilation and could precipitate an angle-closure attack in patients with anatomically narrow angles.

Overdose of Sertal 25 mg, particularly when combined with other serotonergic drugs or alcohol, can cause serotonin syndrome, drowsiness, vomiting, tachycardia, tremor, agitation, dizziness, and in severe cases seizures, coma, or cardiac conduction disturbances. There is no specific antidote for Sertal 25 mg overdose.

If an overdose is suspected, seek immediate medical attention or contact emergency services / a poison control center right away. Do not attempt to manage a suspected overdose at home. Treatment in a medical facility is supportive and may include monitoring of cardiac and vital signs and general symptomatic measures as determined by medical staff.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Elderly

Use Sertal 25 mg with caution in elderly patients, who may be more susceptible to hyponatremia and other adverse effects; no specific dose adjustment is generally required based on age alone, but individualized titration is recommended.

Hepatic impairment

Use a lower dose or reduced frequency in patients with hepatic impairment; caution advised, and use is not recommended in severe hepatic impairment.

Renal impairment

No dosage adjustment is generally required in mild to moderate renal impairment; Sertal 25 mg has not been extensively studied in severe renal impairment, so caution is advised.

Pediatric patients

Sertal 25 mg is approved only for obsessive-compulsive disorder in children and adolescents aged 6-17 years. Safety and efficacy for major depressive disorder in this age group have not been established, and use for depression in pediatric patients is not recommended given the boxed suicidality warning.

Duration of treatment with Sertal 25 mg is individualized. For an initial depressive or anxiety episode, treatment is generally continued for at least 6 months after symptom improvement to reduce relapse risk; for OCD and recurrent or chronic conditions, longer-term maintenance therapy (often a year or more) may be recommended by the treating physician. Any decision to stop Sertal 25 mg should be made with a physician and involve a gradual taper rather than abrupt discontinuation.

Selective Serotonin Reuptake Inhibitor (SSRI); Antidepressant

Sertal 25 mg works by selectively inhibiting the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby increasing the amount of serotonin available at the synapse. This enhanced serotonergic neurotransmission, along with resulting adaptive changes in serotonin receptor sensitivity over time, is believed to underlie the antidepressant and anti-anxiety effects of Sertal 25 mg.

Sertal 25 mg is approved by the FDA for the treatment of obsessive-compulsive disorder (OCD) in children and adolescents aged 6 to 17 years. It is not approved for the treatment of major depressive disorder in pediatric patients.

Pediatric OCD dosing: children 6-12 years may start at 25 mg once daily; adolescents 13-17 years may start at 50 mg once daily. Dose may be titrated gradually, under close physician supervision, to a maximum of 200 mg/day based on response and tolerability.

All pediatric and young adult patients started on Sertal 25 mg require close monitoring for worsening depression, unusual behavior changes, and emergence or worsening of suicidal thoughts, particularly during the first weeks of treatment and after dose changes (see boxed warning under Precautions and Warnings). Use in this age group should always be supervised by a qualified physician.
