Tearfresh Liquigel 10 ml (also known as carmellose sodium or sodium carboxymethylcellulose) is an ophthalmic demulcent/lubricant that is established, under FDA OTC ophthalmic drug monographs, for the temporary relief of burning, irritation, and discomfort due to dryness of the eye (dry eye syndrome, keratoconjunctivitis sicca) and for relief of eye discomfort caused by exposure to wind, sun, or other minor environmental irritants.

• Symptomatic relief of dry, gritty, burning, or irritated eyes
• Protection of the ocular surface from further dryness/irritation (as a demulcent/protectant)
• Use as a lubricant to reduce discomfort associated with artificial eye prostheses (some formulations are labeled for this)

Adjunct Use

Tearfresh Liquigel 10 ml eye drops are commonly used as an adjunct lubricant before or after ophthalmic procedures, contact lens wear, or alongside other prescription eye medications to counter ocular surface dryness.

Other Pharmaceutical Uses (Non-Ophthalmic Formulations)

In some markets, oral formulations containing Tearfresh Liquigel 10 ml are used as a bulk-forming agent for the management of constipation and for regulating stool consistency (e.g., after ileostomy/colostomy). This is a formulation-specific, less common use compared to the ophthalmic lubricant use, which is the form most widely available and used in Bangladesh.

Tearfresh Liquigel 10 ml is also widely used as a pharmaceutical excipient (viscosity-modifying, suspending, and binding agent) in tablets, oral suspensions, and topical/ophthalmic gel formulations; in this role it is not itself the therapeutic agent.

Oral bulk-forming formulations, where marketed, contain Tearfresh Liquigel 10 ml as tablets or granules (commonly around 500 mg per tablet) intended to be taken with adequate fluid.

Ophthalmic lubricant / tear substitute (ocular demulcent); Tearfresh Liquigel 10 ml is also classified as a bulk-forming laxative (oral use) and a pharmaceutical excipient (viscosity-modifying agent), depending on formulation.

Mechanism of action (ophthalmic use): Tearfresh Liquigel 10 ml is a hydrophilic, high-molecular-weight cellulose polymer. When instilled into the eye, it increases the viscosity and stability of the tear film, prolonging its residence time on the ocular surface. This forms a smooth, lubricating layer that reduces friction from blinking, minimizes evaporation of the natural tear film, and provides a soothing, protective barrier over the cornea and conjunctiva, relieving symptoms of dryness, burning, and irritation.

Mechanism of action (oral bulk-forming use): When taken orally in appropriate formulations, Tearfresh Liquigel 10 ml absorbs water within the gastrointestinal tract, increasing stool bulk and softness, which mechanically stimulates peristalsis and facilitates bowel movement.

Tearfresh Liquigel 10 ml is largely non-absorbed and acts locally (on the ocular surface or within the gut lumen), which accounts for its favorable systemic safety profile and minimal potential for systemic drug interactions.

Tearfresh Liquigel 10 ml ophthalmic solution/gel is for topical ocular use only - it must not be injected or taken orally unless the specific product is labeled and formulated for oral use. Remove contact lenses before instilling Tearfresh Liquigel 10 ml eye drops unless the specific product is labeled as safe for use with contact lenses in place; lenses may generally be reinserted after 10-15 minutes, or as directed on the product label. Do not touch the dropper tip to the eye, fingers, or any surface to avoid contaminating the solution. Discard multi-dose bottles per the labeled discard period after opening (commonly 4 weeks) and discard single-use (preservative-free) vials after one use.

Tearfresh Liquigel 10 ml used as an ophthalmic lubricant has minimal systemic absorption and therefore a very low potential for clinically significant systemic drug interactions.

• Other topical eye medications: If Tearfresh Liquigel 10 ml is used together with other eye drops or ointments, separate administration by at least 5-10 minutes to prevent one product from washing out or diluting the other and to allow adequate absorption of each.
• Oral bulk-forming use: When taken orally, Tearfresh Liquigel 10 ml (as a bulk fiber agent) may theoretically reduce the rate or extent of absorption of certain concurrently administered oral medications by physically binding or delaying gastric transit; as a precaution, other oral medicines should be taken at least 1-2 hours apart from oral Tearfresh Liquigel 10 ml formulations.

Tearfresh Liquigel 10 ml ophthalmic products are generally very well tolerated. Reported adverse effects are typically mild and transient:

• Temporary blurred vision immediately after instillation
• Mild stinging, burning, or a foreign-body/sticky sensation in the eye on instillation
• Eye redness or irritation (uncommon)
• Rare hypersensitivity reactions (itching, swelling, redness) - usually related to a preservative rather than Tearfresh Liquigel 10 ml itself

With oral bulk-forming formulations, gastrointestinal effects such as bloating, flatulence, or abdominal cramping may occur, particularly if fluid intake is inadequate.

There is minimal systemic absorption of Tearfresh Liquigel 10 ml when used as an ophthalmic lubricant, and it is generally regarded as having a favorable safety profile for use during pregnancy and breastfeeding. However, as with any medicine, Tearfresh Liquigel 10 ml should be used during pregnancy and lactation only if clearly needed, and a physician or pharmacist should be consulted before use, particularly for oral bulk-forming formulations, to weigh potential benefit against any theoretical risk.

• For topical ophthalmic use only; not for injection or intraocular use.
• Remove contact lenses before instillation unless the specific product is labeled safe for use with lenses in place.
• Do not touch the dropper tip to the eye or any other surface to prevent contamination of the solution and risk of eye infection.
• If eye pain, changes in vision, continued redness or irritation of the eye, or worsening of symptoms occurs, or if symptoms persist for more than 72 hours, discontinue use and seek medical evaluation.
• Do not use if the solution has changed color or becomes cloudy.
• Do not share Tearfresh Liquigel 10 ml eye drops with another person, to avoid the risk of eye infection.
• For oral bulk-forming formulations, ensure adequate fluid intake to reduce the risk of choking or intestinal/esophageal obstruction; see also Contraindications and Side Effects.

Ocular overdose (excess instillation) of Tearfresh Liquigel 10 ml is unlikely to cause serious harm; any excess solution can simply be flushed from the eye with clean water. If Tearfresh Liquigel 10 ml ophthalmic solution is accidentally ingested in small amounts, significant toxicity is unlikely given its minimal systemic absorption; however, if a large amount is swallowed, or if any concerning symptoms develop after accidental ingestion or ocular overuse, seek immediate medical attention or contact a poison control center/emergency services rather than attempting home treatment.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children. Do not freeze. Discard multi-dose bottles per the label's stated period after first opening (commonly 4 weeks), and discard single-use preservative-free vials immediately after one use.

Pediatric Use

Tearfresh Liquigel 10 ml ophthalmic lubricant is generally considered safe for use in children for symptomatic relief of dry or irritated eyes; however, a physician should be consulted before use in infants and young children, and formal pediatric dosing studies establishing efficacy in specific age groups are limited. Oral bulk-forming formulations of Tearfresh Liquigel 10 ml are not generally recommended for young children without direct medical supervision.

Elderly Use

No dose adjustment is required for elderly patients using Tearfresh Liquigel 10 ml ophthalmic lubricant; dry eye is common in this age group and Tearfresh Liquigel 10 ml is commonly and safely used.

Renal/Hepatic Impairment

Because systemic absorption of ophthalmic Tearfresh Liquigel 10 ml is minimal, no dose adjustment is required in patients with renal or hepatic impairment.

Tearfresh Liquigel 10 ml ophthalmic lubricant may be used as needed for symptomatic relief and can be continued long-term/chronically for ongoing management of chronic dry eye, since it has a favorable long-term safety profile.
