Indications of Vonocab Trio

Established / FDA-Approved Use

Vonocab Trio is indicated for the treatment (eradication) of Helicobacter pylori (H. pylori) infection in adults, given as a fixed 14-day combination course.

Important Limitations

  • Vonocab Trio is not indicated for the treatment of gastroesophageal reflux disease (GERD), peptic ulcer disease, or other acid-related conditions on its own — it is specifically an H. pylori eradication regimen.
  • Regional clarithromycin resistance rates in H. pylori strains affect the expected success of this regimen; where resistance is high, or after a prior failed eradication attempt, a physician may choose an alternative (e.g., bismuth-based quadruple) regimen instead.
  • Use of Vonocab Trio when infection is not proven or strongly suspected does not provide benefit and increases the risk of drug-resistant bacteria.

Composition

Each course of Vonoprazan + Amoxicillin + Clarithromycin combination therapy supplies, per dose:

  • Vonoprazan 20 mg (potassium-competitive acid blocker) — one tablet
  • Amoxicillin 1,000 mg (as two 500 mg capsules/tablets) — penicillin-class antibacterial
  • Clarithromycin 500 mg (macrolide antibacterial) — one tablet

All three components of Vonoprazan + Amoxicillin + Clarithromycin are taken together, twice daily, as a fixed combination regimen.

Description

Vonocab Trio is a fixed combination regimen used as triple therapy to eradicate Helicobacter pylori (H. pylori) infection in adults. It pairs a potent, potassium-competitive acid blocker (vonoprazan) with two antibacterial agents (amoxicillin and clarithromycin) so that gastric acid is suppressed strongly enough, and for long enough, to let the antibiotics act effectively against H. pylori colonizing the stomach lining.

Vonocab Trio is intended only as a complete, fixed-duration eradication course and is not intended for long-term or as-needed use for acid-related symptoms alone.

Therapeutic Class

Vonocab Trio belongs to the therapeutic class of H. pylori eradication regimens, combining:

  • A potassium-competitive acid blocker (P-CAB) — vonoprazan
  • A penicillin-class (aminopenicillin) antibacterial — amoxicillin
  • A macrolide antibacterial — clarithromycin

Pharmacology

Vonoprazan

Vonoprazan reversibly and competitively blocks potassium binding at the gastric H+/K+-ATPase (proton pump) in parietal cells, producing potent and sustained suppression of gastric acid secretion. Unlike proton pump inhibitors, its acid-suppressing effect does not depend on CYP2C19 metabolizer status and begins rapidly after the first dose.

Amoxicillin

Amoxicillin is a beta-lactam antibacterial that inhibits bacterial cell-wall synthesis by binding penicillin-binding proteins, producing a bactericidal effect against susceptible H. pylori.

Clarithromycin

Clarithromycin is a macrolide antibacterial that binds the 50S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis. Its activity against H. pylori can be reduced by 23S rRNA mutations that confer clarithromycin resistance, which vary by region.

In Vonoprazan + Amoxicillin + Clarithromycin, the strong, sustained acid suppression from vonoprazan raises intragastric pH, which improves the stability and antibacterial activity of amoxicillin and clarithromycin against H. pylori.

Dosage & Administration of Vonocab Trio

Standard Adult Regimen (H. pylori eradication)

TimeVonoprazanAmoxicillinClarithromycin
Morning20 mg1,000 mg500 mg
Evening20 mg1,000 mg500 mg

The full course of Vonocab Trio is taken twice daily for a fixed duration (typically 14 days, per physician's prescription), with or without food. All three components must be taken together at each dosing time.

Missed Dose

If a dose of Vonocab Trio is missed, take it as soon as possible within 4 hours of the scheduled time. If more than 4 hours have passed, skip the missed dose and resume the normal dosing schedule — do not double the next dose.

Renal and Hepatic Impairment

  • Renal impairment: Use of Vonocab Trio is not recommended in patients with severe renal impairment (creatinine clearance below 30 mL/min).
  • Hepatic impairment: Use is not recommended in patients with moderate to severe hepatic impairment (Child-Pugh Class B or C).

Antibiotic Stewardship

Take Vonocab Trio exactly as prescribed by your physician; do not stop, extend, skip doses, or share this medicine with others without medical advice. Completing the full prescribed course is essential for successful H. pylori eradication and to limit the development of antibiotic resistance.

Administration of Vonocab Trio

Vonocab Trio is administered orally. All tablets/capsules for a given dosing time should be swallowed together with water, with or without food, at approximately the same times each day (e.g., morning and evening) to maintain even drug levels and treatment adherence.

Interaction of Vonocab Trio

Contraindicated Combinations

Certain drugs must not be used with Vonocab Trio because clarithromycin is a strong CYP3A4 inhibitor and can raise their levels to dangerous concentrations — see Contraindications for the full list (e.g., pimozide, lomitapide, lovastatin, simvastatin, ergot alkaloids, lurasidone, colchicine in renal/hepatic impairment, rilpivirine-containing products).

Major Interactions Requiring Caution or Dose Adjustment

  • Other statins (atorvastatin, pravastatin): Increased risk of myopathy/rhabdomyolysis; if concurrent use is necessary, the lowest statin dose should be used and patients monitored for muscle symptoms.
  • Warfarin and other oral anticoagulants: Clarithromycin can increase INR and bleeding risk; monitor coagulation closely during and after therapy.
  • Benzodiazepines metabolized by CYP3A4 (e.g., midazolam, triazolam): Increased sedation and respiratory depression risk.
  • Antiarrhythmics (disopyramide, amiodarone, dofetilide, procainamide, quinidine, sotalol) and other QT-prolonging drugs: Additive risk of QT prolongation and arrhythmia; avoid combined use where possible.
  • Clopidogrel: Vonoprazan may reduce conversion of clopidogrel to its active metabolite via CYP2C19 inhibition, potentially reducing its antiplatelet effect.
  • Hypoglycemic agents/insulin: Clarithromycin has been associated with significant hypoglycemia; monitor blood glucose.
  • Hormonal contraceptives: Amoxicillin may rarely reduce contraceptive efficacy; an additional non-hormonal method is advisable during and briefly after the course.
  • Probenecid: Increases and prolongs amoxicillin blood levels.
  • Allopurinol: Concurrent use with amoxicillin increases the incidence of skin rash.
  • Omeprazole and other proton pump inhibitors: Should not be combined with this regimen, as they can alter clarithromycin gastric tissue concentrations and duplicate acid-suppressive therapy unnecessarily.

Always inform your physician or pharmacist of all other medicines, supplements, and herbal products before starting Vonocab Trio.

Contraindications

Vonoprazan + Amoxicillin + Clarithromycin is contraindicated in patients with:

  • Known hypersensitivity to vonoprazan, or to any component of the regimen
  • Known hypersensitivity to amoxicillin, other penicillins, or other beta-lactam antibacterials
  • Known hypersensitivity to clarithromycin, erythromycin, or other macrolide antibacterials
  • A prior history of cholestatic jaundice or hepatic dysfunction associated with previous clarithromycin use
  • Concurrent use of pimozide, lomitapide, lovastatin, or simvastatin (risk of serious cardiac arrhythmia or rhabdomyolysis due to CYP3A4 inhibition by clarithromycin)
  • Concurrent use of ergot alkaloids (e.g., ergotamine, dihydroergotamine) (risk of acute ergot toxicity)
  • Concurrent use of lurasidone
  • Concurrent use of colchicine in patients with renal or hepatic impairment
  • Concurrent use of rilpivirine-containing products

Side Effects of Vonocab Trio

The most commonly reported side effects of Vonocab Trio (occurring in ≥2% of patients) include:

  • Altered or metallic taste (dysgeusia)
  • Diarrhea
  • Vulvovaginal candidiasis (yeast infection)
  • Headache
  • Abdominal pain
  • Increased blood pressure

Diarrhea and elevated blood pressure are the most common reasons treatment is stopped early. Less common but serious effects can include severe allergic reactions, severe skin reactions, liver problems, and Clostridioides difficile-associated diarrhea — see Precautions and Warnings for details. Contact your physician promptly if you notice a rash, yellowing of the skin/eyes, severe or persistent diarrhea, irregular heartbeat, or signs of a serious allergic reaction while taking Vonocab Trio.

Pregnancy & Lactation

Pregnancy

Vonocab Trio is not recommended during pregnancy except in situations where no alternative therapy is appropriate and the potential benefit is judged by a physician to justify the potential risk to the fetus. Clarithromycin, one of its components, has been associated with an increased risk of miscarriage and adverse fetal outcomes in some observational studies. Use only if clearly needed, under direct medical supervision, and after discussing risks and alternatives with a physician.

Lactation

Amoxicillin and clarithromycin pass into breast milk in small amounts and could affect a nursing infant's gut flora (e.g., diarrhea, oral thrush); data on vonoprazan in human milk are limited. Breastfeeding mothers should use Vonocab Trio only if clearly needed and advised by a physician, with the infant monitored for GI disturbance or rash.

Precautions & Warnings

Antibiotic Stewardship and Resistance

Take Vonocab Trio exactly as prescribed; do not stop, extend, skip doses, or share this medicine with others without medical advice. This is a fixed-duration eradication course and should not be used to treat other acid-related conditions alone. Regional clarithromycin resistance rates in H. pylori can reduce the effectiveness of this regimen; physicians may choose an alternative regimen where local resistance is high or after a prior failed eradication attempt.

QT Prolongation

Clarithromycin can prolong the QT interval and, rarely, cause serious arrhythmias including torsades de pointes. Avoid this regimen in patients with known QT prolongation, ventricular arrhythmias, uncorrected hypokalemia or hypomagnesemia, clinically significant bradycardia, or concurrent use of other QT-prolonging drugs or Class IA/III antiarrhythmics; elderly patients are at higher risk.

Hepatotoxicity

Hepatic dysfunction, usually reversible, has been reported with clarithromycin; rare cases of fatal hepatic failure have occurred. Stop treatment and seek medical attention if signs of hepatitis (e.g., anorexia, jaundice, dark urine, pruritus, tender abdomen) appear.

Clostridioides difficile-Associated Diarrhea

As with virtually all antibacterial agents, C. difficile-associated diarrhea can occur, ranging from mild diarrhea to fatal colitis, and may appear during or up to several weeks after treatment. Seek medical attention for severe, persistent, or bloody diarrhea.

Serious Hypersensitivity and Skin Reactions

Serious and occasionally fatal hypersensitivity reactions (including anaphylaxis) and severe cutaneous reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis, DRESS, and acute generalized exanthematous pustulosis) have been reported with the antibiotic components. Discontinue immediately and seek urgent medical care if a rash, blistering, mucosal lesions, or facial/throat swelling develops.

Other Precautions

  • Acute tubulointerstitial nephritis has been reported with vonoprazan; discontinue and evaluate if suspected.
  • Avoid amoxicillin-containing regimens in patients with infectious mononucleosis due to a high rate of associated skin rash.
  • Clarithromycin may worsen symptoms of myasthenia gravis; monitor patients with this condition.
  • Use with caution in patients on drugs that are sensitive CYP3A4 substrates not listed under Contraindications; dose adjustment of the interacting drug may be needed (see Interactions).

Overdose Effects of Vonocab Trio

There is limited specific experience with overdose of the combined Vonocab Trio regimen. Overdose of the individual components may cause gastrointestinal symptoms (nausea, vomiting, diarrhea, abdominal pain), and very large amoxicillin overdoses can rarely cause crystalluria or kidney effects. In case of a suspected overdose, seek immediate medical attention or contact your local emergency/poison control services; do not attempt home treatment. Management is supportive; amoxicillin is partially removed by hemodialysis if needed.

Storage Conditions

Store at room temperature (below 30°C), away from light and moisture, in the original packaging. Keep out of reach of children.

Use In Special Populations

Renal Impairment

Hepatic Impairment

Not recommended in patients with moderate to severe hepatic impairment (Child-Pugh Class B or C); see Dosage and Administration.

Elderly

Elderly patients may be at higher risk of clarithromycin-associated QT prolongation and torsades de pointes (see Precautions and Warnings); use with caution and monitor cardiac and renal status.

Pregnancy and Lactation

See the dedicated Pregnancy and Lactation section.

Pediatric Patients

See Pediatric Uses.

Duration Of Treatment

The standard course of Vonocab Trio is a fixed duration, typically 14 days, as prescribed by the physician. The full course must be completed even if symptoms improve earlier, to maximize H. pylori eradication rates and reduce the risk of antibiotic resistance. Do not restart or repeat a course without medical reassessment, particularly if a prior course failed.

Drug Classes

Vonoprazan + Amoxicillin + Clarithromycin combines three drug classes: a potassium-competitive acid blocker (P-CAB), a penicillin-class (aminopenicillin) antibacterial, and a macrolide antibacterial, used together as an H. pylori eradication regimen.

Mode Of Action

See Pharmacology for the detailed mechanism of each component of Vonoprazan + Amoxicillin + Clarithromycin: vonoprazan blocks gastric acid secretion by reversibly inhibiting the H+/K+-ATPase proton pump; amoxicillin kills H. pylori by inhibiting bacterial cell-wall synthesis; and clarithromycin inhibits bacterial protein synthesis at the 50S ribosome. Together, sustained acid suppression enhances antibiotic activity against H. pylori.

Pediatric Uses

The safety and efficacy of Vonocab Trio have not been established in pediatric patients, and this regimen is not recommended for use in children. H. pylori infection in children, when treatment is indicated, should be managed by a pediatric specialist using age-appropriate regimens.
