Composition

Acyclovir is available in Bangladesh in the following strengths and formulations: oral tablets containing Acyclovir 200 mg or 400 mg; oral suspension containing Acyclovir 200 mg/5 mL; topical cream containing Acyclovir 5% w/w; and Acyclovir sodium powder for IV infusion available in 250 mg, 500 mg, and 1 g vials.

Description

Xovir is an antiviral medicine used to treat infections caused by herpes simplex virus and varicella-zoster virus. Xovir works by slowing the growth and spread of the virus so the body's immune system can help control the infection, though Xovir does not cure or permanently eliminate the virus from the body. Xovir is available as tablets and oral suspension for genital herpes, cold sores, chickenpox, and shingles; as a topical cream for cold sores on the lips; and as an intravenous infusion for severe infections requiring hospital care. Starting Xovir as early as possible after symptoms begin usually gives the best results. Xovir should always be taken exactly as prescribed by a qualified healthcare professional.

Therapeutic Class

Antiviral (Nucleoside Analogue)

Pharmacology

Acyclovir is a synthetic purine nucleoside analogue. Inside cells infected with herpes simplex virus (HSV-1, HSV-2) or varicella-zoster virus (VZV), the viral enzyme thymidine kinase converts Acyclovir into Acyclovir monophosphate, which host cell enzymes further convert to Acyclovir triphosphate. This active form competitively inhibits viral DNA polymerase, is incorporated into the growing viral DNA chain causing chain termination, and inactivates the viral DNA polymerase, thereby halting viral replication. Because this activation step depends on a virus-specific enzyme, the antiviral action of Acyclovir is highly selective for virus-infected cells, with much less effect on uninfected host cells. After oral dosing, Acyclovir has variable, dose-dependent bioavailability of roughly 10-20%; food does not significantly affect absorption. Administration of Xovir

  • Tablets/suspension: Xovir may be taken with or without food; swallow tablets with a full glass of water; shake the oral suspension well and measure each dose with an accurate oral syringe or measuring cup.
  • Maintain adequate hydration while taking oral or IV Xovir, especially at higher doses, to help protect the kidneys (see Precautions & Warnings for the full explanation).

Interaction of Xovir

  • Probenecid: reduces renal excretion of Xovir, increasing Xovir plasma concentration and half-life, which may raise the risk of Xovir-related side effects.
  • Other nephrotoxic drugs (e.g., aminoglycosides, cyclosporine, tacrolimus, NSAIDs): coadministration with Xovir, especially IV or high-dose oral Xovir, may increase the risk of kidney dysfunction.
  • Mycophenolate mofetil: Xovir and the active metabolite of mycophenolate mofetil compete for renal tubular excretion, which can raise blood levels of both drugs when combined with Xovir, most relevant in transplant patients.
  • Zidovudine: concurrent use with Xovir has been associated with increased drowsiness and lethargy in some patients.
  • Theophylline: limited evidence suggests Xovir may affect theophylline levels; monitoring may be advised if combined with Xovir.

Contraindications

Pregnancy & Lactation

Pregnancy

Xovir crosses the placenta. Xovir has been used in pregnancy for many years, and a prospective pregnancy registry following 749 pregnancies with first-trimester exposure to Xovir found a rate of birth defects similar to that in the general population; however, the registry was not large enough to rule out risk for less common defects, and no formal, adequately controlled human studies of Xovir exist. Xovir should be used during pregnancy only if the potential benefit to the mother justifies the potential risk to the fetus, and only under the direction of a qualified healthcare professional — this applies to oral, topical, and IV Xovir alike.

Breastfeeding

Xovir passes into breast milk, at concentrations reported to range from about 0.6 to 4.1 times the corresponding maternal plasma level. This can cause acute kidney injury or, rarely, kidney failure, which has been fatal in some reported cases. Overdose Effects of Xovir

Reported symptoms of Xovir overdose include agitation, confusion, drowsiness/lethargy, and seizures. Excess Xovir can crystallize in the kidney tubules, which may lead to acute kidney injury. If an overdose of Xovir is suspected, do not induce vomiting unless specifically directed to do so by a healthcare professional or poison control service, and seek emergency medical attention or contact a poison control center immediately. Storage Conditions

Children

IV Xovir is used in infants and children, including neonates, for serious infections such as neonatal herpes and encephalitis, with doses calculated by body weight and administered only under specialist hospital care. For chickenpox, routine Xovir treatment is not recommended for otherwise healthy children; oral Xovir is generally reserved for children older than 12 years or those with specific risk factors, as determined by a healthcare professional.

Elderly

Older adults may have reduced kidney function even when standard lab tests appear normal, and are more likely to experience neurological side effects from Xovir and require dose adjustment. See the renal/hydration precaution above.

Renal Impairment

Because Xovir is cleared mainly by the kidneys, patients with reduced kidney function require dose and/or interval adjustment for both oral and IV Xovir, as outlined in the Dosage table. Adequate hydration is essential (see Precautions & Warnings).

Reconstitution

Xovir sodium for IV infusion is supplied as a powder in vials and must be reconstituted and further diluted by a qualified healthcare professional before use — for example, a 500 mg vial of Xovir is reconstituted with Sterile Water for Injection to a concentration of approximately 50 mg/mL, then further diluted in an IV fluid to a concentration of 7 mg/mL or less before slow infusion over at least 1 hour.
