About Acyclovir + Hydrocortisone

Indications of Acyclovir + Hydrocortisone

Acyclovir + Hydrocortisone topical cream is a combination antiviral-corticosteroid preparation with an established indication:

  • Early treatment of recurrent herpes labialis (cold sores) in adults and children (age 6 years and above): Acyclovir + Hydrocortisone is indicated for the early treatment of cold sores at the first sign of an outbreak (tingling, itching, burning, redness), to reduce the likelihood of progression to a classic cold sore lesion.

Acyclovir + Hydrocortisone has not been shown to be effective when initiated after the cold sore has developed to the papule, vesicle, or ulcerative stage, and it has not been shown to prevent recurrences or transmission of herpes simplex virus. It is not indicated for use inside the mouth, in the eyes, on the genitals, or for other forms of herpes infection.

Dosage & Administration of Acyclovir + Hydrocortisone

IndicationPopulationDose
Herpes labialis (cold sores), early treatmentAdults and children ≥ 6 yearsApply a thin layer of Acyclovir + Hydrocortisone cream to the affected area 5 times daily for 5 days

Treatment with Acyclovir + Hydrocortisone should be started as early as possible, ideally during the prodromal stage (tingling, itching, burning, or redness) before any visible lesion appears, for the best chance of reducing lesion progression. If a course of Acyclovir + Hydrocortisone is not started until after the cold sore has already developed into a blister or ulcer, benefit is less certain.

Safety and efficacy of Acyclovir + Hydrocortisone for repeated, intermittent, or chronic use beyond a single 5-day course have not been established; see Precautions and Warnings regarding prolonged corticosteroid exposure.

Side Effects of Acyclovir + Hydrocortisone

Most reported side effects with Acyclovir + Hydrocortisone are limited to the application site and are usually mild:

  • Application-site dryness, flaking, or exfoliation
  • Local burning, stinging, or itching
  • Redness or mild swelling at the application site
  • Application-site pain

Systemic side effects are rare with correctly used topical Acyclovir + Hydrocortisone because absorption through skin is minimal. Discontinue and seek medical advice if signs of an allergic reaction (rash, swelling of the face/lips/tongue, difficulty breathing) occur.

Contraindications of Acyclovir + Hydrocortisone

Acyclovir + Hydrocortisone is contraindicated in patients with known hypersensitivity to acyclovir, valacyclovir, hydrocortisone, or any other component of the cream formulation.

Frequently Asked Questions about Acyclovir + Hydrocortisone

What is Acyclovir + Hydrocortisone cream used for?

Acyclovir + Hydrocortisone is used for the early treatment of cold sores (herpes labialis) in adults and children 6 years and older, applied at the very first sign of an outbreak such as tingling, itching, or redness.

When should I start applying Acyclovir + Hydrocortisone?

You should start applying Acyclovir + Hydrocortisone as soon as you notice the earliest warning signs of a cold sore (tingling, itching, burning, or redness), before a visible blister forms. Starting later, once the sore has already blistered or ulcerated, is less likely to be effective.

How long can I use Acyclovir + Hydrocortisone?

Acyclovir + Hydrocortisone should be applied 5 times a day for 5 days only. It should not be used for longer than this recommended course because the corticosteroid component can cause skin thinning or local immune suppression with prolonged use.

Can I use Acyclovir + Hydrocortisone during pregnancy or breastfeeding?

Acyclovir + Hydrocortisone should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus; consult a physician before use. If breastfeeding, avoid applying Acyclovir + Hydrocortisone near the nipple/areola and consult a physician first.

What are the side effects of Acyclovir + Hydrocortisone?

The most common side effects of Acyclovir + Hydrocortisone are mild application-site reactions such as dryness, flaking, burning, stinging, or redness. Stop use and seek medical advice if signs of an allergic reaction occur.

Is Acyclovir + Hydrocortisone safe for children?

Acyclovir + Hydrocortisone is approved for children 6 years of age and older at the same dosing as adults. Safety and efficacy in children under 6 have not been established, so it should only be used in younger children under medical supervision.
