Novirax HC5%+1%

Novirax HC topical cream is a combination antiviral-corticosteroid preparation with an established indication:

• Early treatment of recurrent herpes labialis (cold sores) in adults and children (age 6 years and above): Novirax HC is indicated for the early treatment of cold sores at the first sign of an outbreak (tingling, itching, burning, redness), to reduce the likelihood of progression to a classic cold sore lesion.

Novirax HC has not been shown to be effective when initiated after the cold sore has developed to the papule, vesicle, or ulcerative stage, and it has not been shown to prevent recurrences or transmission of herpes simplex virus. It is not indicated for use inside the mouth, in the eyes, on the genitals, or for other forms of herpes infection.

Each gram of Acyclovir + Hydrocortisone cream typically contains Acyclovir 50 mg (5% w/w) as the antiviral component and Hydrocortisone 10 mg (1% w/w) as the topical corticosteroid component, in a cream base.

Novirax HC is a fixed-dose combination topical cream that pairs a synthetic nucleoside analogue antiviral agent (acyclovir) with a mild-to-moderate potency topical corticosteroid (hydrocortisone). It is applied externally to the lips and surrounding skin at the earliest sign of a cold sore. The combination is designed to target both the viral replication that drives the lesion and the local inflammatory response that contributes to lesion size, pain, and duration.

Novirax HC belongs to the therapeutic class of topical antiviral-corticosteroid combinations, used specifically for herpes labialis (cold sores).

Acyclovir + Hydrocortisone combines two complementary mechanisms of action:

• Acyclovir is a guanosine nucleoside analogue. Inside herpes simplex virus (HSV)-infected cells, it is phosphorylated by viral thymidine kinase and subsequently by host cellular kinases to acyclovir triphosphate, which competitively inhibits viral DNA polymerase and is incorporated into viral DNA, causing chain termination. This selectively suppresses HSV replication in infected cells with minimal effect on uninfected cells.
• Hydrocortisone is a low-potency corticosteroid that reduces local inflammation, erythema, edema, and pruritus by suppressing the release of inflammatory mediators and reducing local immune cell activity in the skin.

Together, the components in Acyclovir + Hydrocortisone aim to reduce both the viral load driving the lesion and the inflammatory component that contributes to lesion progression when treatment is started early (during the prodromal/tingling stage).

Novirax HC is for external (topical) use only, applied to the lips and immediate surrounding skin. Wash and dry hands before and after application. Apply a thin film sufficient to cover the affected area; rubbing in is not required. Avoid application inside the mouth, in or near the eyes, or on mucous membranes. Do not apply occlusive dressings over Novirax HC unless directed by a physician.

Clinically significant drug interactions with topical Novirax HC are unlikely because systemic absorption through intact skin is minimal. However:

• Concomitant use of other topical products (including other corticosteroids or irritant/exfoliating agents such as retinoids) on the same area may increase local irritation or unpredictably alter corticosteroid absorption; such combinations should be avoided unless a physician advises otherwise.
• There is no established clinically significant interaction between topical Novirax HC and systemic medications when used as labeled for cold sores.

Most reported side effects with Novirax HC are limited to the application site and are usually mild:

• Application-site dryness, flaking, or exfoliation
• Local burning, stinging, or itching
• Redness or mild swelling at the application site
• Application-site pain

Systemic side effects are rare with correctly used topical Novirax HC because absorption through skin is minimal. Discontinue and seek medical advice if signs of an allergic reaction (rash, swelling of the face/lips/tongue, difficulty breathing) occur.

Data on the use of topical Novirax HC in pregnancy are limited, although systemic absorption after topical application is minimal. As with any medicine in pregnancy, Novirax HC should be used only if clearly needed and if the potential benefit justifies the potential risk to the fetus; a physician should be consulted before use during pregnancy.

It is not known whether topically applied Novirax HC passes into breast milk in clinically significant amounts. Caution is advised, and breastfeeding mothers should avoid applying Novirax HC directly to or near the nipple/areola, and should consult a physician before use while breastfeeding.

Novirax HC is for external use on the lips and surrounding skin only. Observe the following precautions:

• Avoid contact with the eyes, inside of the mouth, nose, and other mucous membranes; if accidental contact occurs, rinse thoroughly with water.
• For best results, begin applying Novirax HC at the earliest prodromal symptoms (tingling, itching, burning, redness); benefit is reduced once the lesion has progressed to blister or ulcer stage.
• Do not use Novirax HC for longer than the recommended 5-day course. Because it contains a corticosteroid, prolonged or excessive use may lead to local skin thinning (atrophy), striae, telangiectasia, or local immunosuppression that could worsen an underlying viral infection.
• Novirax HC is not recommended for use on active, widespread, or severe herpetic lesions, or in immunocompromised patients, without direct medical supervision.
• As an antiviral product, Novirax HC should be used strictly for its labeled indication (cold sores) and not applied for general skin inflammation or unrelated rashes; use exactly as directed and do not share this medicine with others without medical advice.
• If the cold sore does not improve within 10 days, or worsens, or if fever, widespread rash, or eye involvement occurs, stop use and consult a physician.

Topical overdose with Novirax HC is unlikely given minimal systemic absorption, but excessive or prolonged application may increase the risk of local skin irritation or corticosteroid-related skin changes. If Novirax HC is accidentally ingested or applied to a large area in excessive amounts, seek immediate medical attention or contact a poison control center; do not attempt unsupervised home treatment for suspected overdose.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Renal/Hepatic impairment: Because systemic absorption of topical Novirax HC is minimal, specific dose adjustment for renal or hepatic impairment is generally not required for topical use; however, caution and physician consultation are advised in patients with significant organ impairment.

Elderly: No specific dose adjustment is established for elderly patients using Novirax HC topically; skin fragility in elderly patients may increase sensitivity to the corticosteroid component, so use should be monitored.

Immunocompromised patients: Use of Novirax HC in immunocompromised individuals should occur only under medical supervision, as corticosteroid-related local immunosuppression could theoretically affect viral clearance.

The standard course of Novirax HC is 5 days (applied 5 times daily). Treatment beyond 5 days is not recommended without medical advice, primarily due to the corticosteroid component's risk of local skin changes with prolonged use. If the cold sore has not resolved after 10 days, a physician should be consulted.

Acyclovir + Hydrocortisone is classified as a topical antiviral (nucleoside analogue) combined with a topical corticosteroid.

Acyclovir + Hydrocortisone works through two mechanisms: the acyclovir component is phosphorylated inside HSV-infected cells and inhibits viral DNA polymerase, halting viral replication; the hydrocortisone component suppresses local inflammatory mediators, reducing redness, swelling, and irritation at the lesion site. Combined, this addresses both the underlying viral cause and the visible inflammatory response of a cold sore.

Novirax HC is approved for early treatment of cold sores in children 6 years of age and older, using the same regimen as adults (applied 5 times daily for 5 days). Safety and efficacy of Novirax HC in children under 6 years of age have not been established, and use in this age group should only occur under medical supervision.
