About Clobetasol Propionate + Salicylic Acid

Indications of Clobetasol Propionate + Salicylic Acid

Clobetasol Propionate + Salicylic Acid ointment is a topical combination of a super-potent (Class I) corticosteroid and a keratolytic agent, indicated for:

Established use

  • Short-term topical treatment of moderate-to-severe, thick, or hyperkeratotic (scaly) plaque psoriasis in adults, particularly localized plaques on the trunk, limbs, and scalp that are resistant to standard corticosteroid-only therapy.

Guideline-supported / adjunct use

  • As an adjunct in scalp psoriasis, where the keratolytic action of salicylic acid helps soften and remove thick scale, improving penetration of clobetasol propionate into the affected skin.
  • For other chronic, recalcitrant, corticosteroid-responsive hyperkeratotic dermatoses (e.g., lichen simplex chronicus, discoid lupus erythematosus lesions with thick scale) at the discretion of the prescribing physician, when standard therapy is inadequate.

Clobetasol Propionate + Salicylic Acid is intended for short-term, localized use only and is not a first-line treatment for mild psoriasis or for widespread/extensive disease.

Dosage & Administration of Clobetasol Propionate + Salicylic Acid

Adult dosage

IndicationDose/FrequencyMaximum duration
Moderate-to-severe thick plaque psoriasis (including scalp)Apply a thin film to the affected area(s) once or twice dailyNot more than 2 consecutive weeks; total weekly dose not to exceed 50 g

Pediatric dosage

Not recommended for use in children under 12 years of age; safety and efficacy in this age group have not been established. Use in adolescents (12-17 years) only under close physician supervision, for the shortest duration and smallest area necessary, due to greater risk of systemic absorption of both components relative to body surface area.

Renal/hepatic impairment

No specific dose adjustment is established for topical use; however, caution is advised because reduced clearance of systemically absorbed corticosteroid or salicylate could theoretically increase risk of systemic effects in significant hepatic or renal impairment.

General instructions

  • Apply only to the affected psoriatic plaques; avoid normal surrounding skin where possible.
  • Do not use on the face, groin, axillae, or other skin folds unless specifically directed by the physician, as these thin-skin areas are more prone to atrophy.
  • Do not apply under occlusive dressings/bandages unless specifically instructed.
  • Do not exceed the maximum body surface area, dose, or duration prescribed, to minimize risk of HPA-axis suppression and salicylate absorption.

Side Effects of Clobetasol Propionate + Salicylic Acid

Local skin reactions are most common; systemic effects are uncommon with correct short-term, limited-area use but risk rises with excessive quantity, duration, or body surface area.

Common

  • Burning, stinging, or irritation at the application site (more often due to the salicylic acid component)
  • Dryness, itching, or mild redness

Less common but significant

  • Skin atrophy, striae (stretch marks), and telangiectasia (visible small blood vessels) with prolonged/excessive use
  • Folliculitis, hypertrichosis, perioral dermatitis, and hypopigmentation at the treated site
  • Allergic contact dermatitis

Rare but serious

  • Hypothalamic-pituitary-adrenal (HPA) axis suppression, Cushingoid features, or hyperglycemia, with excessive/prolonged use or use over large body surface areas
  • Salicylate toxicity (salicylism) - tinnitus, dizziness, nausea, vomiting, hyperventilation, or metabolic disturbance - with excessive systemic absorption of the salicylic acid component (see Overdose)

Contraindications of Clobetasol Propionate + Salicylic Acid

Clobetasol Propionate + Salicylic Acid is contraindicated in:

  • Patients with known hypersensitivity to clobetasol propionate, salicylic acid, or any excipient in the formulation.
  • Primary viral skin infections at the application site, including herpes simplex, varicella (chickenpox), and herpes zoster, due to risk of local spread/worsening under corticosteroid immunosuppression.

Frequently Asked Questions about Clobetasol Propionate + Salicylic Acid

What is Clobetasol Propionate + Salicylic Acid ointment used for?

Clobetasol Propionate + Salicylic Acid is used for the short-term topical treatment of thick, scaly plaque psoriasis, especially resistant plaques on the body or scalp, where the salicylic acid helps remove scale and improve penetration of the corticosteroid.

How long can I use Clobetasol Propionate + Salicylic Acid?

Treatment is generally limited to a maximum of 2 consecutive weeks and no more than 50 g per week, because Clobetasol Propionate + Salicylic Acid contains a super-potent (Class I) corticosteroid. Do not use longer than your physician has prescribed without follow-up.

Can I use Clobetasol Propionate + Salicylic Acid on my face or in skin folds?

No, Clobetasol Propionate + Salicylic Acid should not be applied to the face, groin, armpits, or other skin-fold areas unless your physician specifically instructs you to, because these areas are thinner and more prone to skin thinning (atrophy) and irritation.

Is Clobetasol Propionate + Salicylic Acid safe to use in children?

Clobetasol Propionate + Salicylic Acid is not recommended for children under 12 years old, as safety and effectiveness have not been established and children absorb more of the medicine relative to their body size, raising the risk of side effects. Use in adolescents should only be under close physician supervision.

What side effects should I watch for with Clobetasol Propionate + Salicylic Acid?

Common effects include burning, stinging, dryness, or irritation at the application site. With excessive or prolonged use, more serious effects such as skin thinning, stretch marks, or signs of hormone/adrenal suppression (unusual weight gain, high blood sugar) or salicylate toxicity (ringing in ears, dizziness, nausea) can occur - contact your physician if these develop.

Can I use Clobetasol Propionate + Salicylic Acid while pregnant or breastfeeding?

Clobetasol Propionate + Salicylic Acid should be used during pregnancy or breastfeeding only if your physician determines it is clearly needed, using the smallest amount for the shortest time, since topical corticosteroids and salicylic acid can be absorbed into the bloodstream and their safety in pregnancy/lactation has not been firmly established.
