About Dopamine Hydrochloride

Indications of Dopamine Hydrochloride

Dopamine Hydrochloride is an endogenous catecholamine used as a vasopressor and inotropic agent for the hemodynamic management of shock and severe hypotension. Indications are classified by strength of evidence below.

Established / FDA-approved uses

  • Correction of hemodynamic imbalances (hypotension, low cardiac output, poor organ perfusion) present in the shock syndrome due to myocardial infarction, trauma, endotoxic septicemia, open-heart surgery, renal failure, and chronic cardiac decompensation (e.g. congestive heart failure).
  • Improvement of blood pressure, cardiac output, and urine flow in patients who remain hypotensive after adequate fluid volume replacement (correction of hypovolemia).

Adjunct / combination-therapy use

  • Dopamine Hydrochloride is used as an adjunct to, not a substitute for, volume replacement; in hypovolemic shock, whole blood, plasma, or appropriate fluids must be given first or concurrently.
  • Often combined with other vasoactive or inotropic agents (e.g. norepinephrine, dobutamine) in refractory shock under intensive care supervision.

Not routinely recommended

  • So-called "low-dose" or "renal-dose" Dopamine Hydrochloride for renal protection or prevention of acute kidney injury is not supported by current evidence and is not a recommended indication.

Dosage & Administration of Dopamine Hydrochloride

Dosage and Administration of Dopamine Hydrochloride

Dopamine Hydrochloride is given exclusively by continuous intravenous infusion after dilution, in a monitored hospital setting. See the Dosage and Administration sections above for full per-indication and per-population details.

Key points

  • Dilute before infusion; administer via infusion pump into a secure, preferably central, vein.
  • Start low (2–5 mcg/kg/min) and titrate to hemodynamic response; higher doses (>10 mcg/kg/min) predominantly cause vasoconstriction.
  • Ensure adequate fluid volume replacement has been given or is given concurrently.
  • Continuous cardiac and blood pressure monitoring is required throughout therapy.

Side Effects of Dopamine Hydrochloride

Adverse effects of Dopamine Hydrochloride are generally dose-related and occur most often at higher infusion rates.

Common

  • Ectopic heartbeats, tachycardia, palpitations
  • Anginal chest pain, dyspnea
  • Headache, nausea, vomiting
  • Hypotension (at low doses) or hypertension (at high doses)

Serious / less common

  • Ventricular arrhythmias, marked increase in myocardial oxygen demand
  • Peripheral ischemia/gangrene of the extremities with prolonged high-dose use
  • Local tissue necrosis and sloughing if extravasation occurs at the infusion site (see Precautions and Warnings)
  • Azotemia (rare, reversible on discontinuation)

Contraindications of Dopamine Hydrochloride

Dopamine Hydrochloride is contraindicated in patients with:

  • Pheochromocytoma.
  • Uncorrected tachyarrhythmias or ventricular fibrillation.
  • Known hypersensitivity to Dopamine Hydrochloride or, for sulfite-containing formulations, known sulfite hypersensitivity.

Frequently Asked Questions about Dopamine Hydrochloride

What is Dopamine Hydrochloride used for?

Dopamine Hydrochloride is used in hospitalized, critically ill patients to raise blood pressure and improve heart function and blood flow to organs during shock (for example due to heart attack, severe infection, major surgery, or trauma) when fluids alone are not enough.

How is Dopamine Hydrochloride given?

Dopamine Hydrochloride is given only as a continuous intravenous infusion, after dilution, through a pump in an intensive care unit, with constant monitoring of heart rate, blood pressure, and heart rhythm.

Can Dopamine Hydrochloride be taken as a tablet at home?

No. Dopamine Hydrochloride is only available as an injectable concentrate for intravenous infusion in a hospital critical-care setting; it is not available as an oral or home-use formulation.

Who should not receive Dopamine Hydrochloride?

Dopamine Hydrochloride should not be given to patients with a tumor called pheochromocytoma, uncorrected fast/irregular heart rhythms (tachyarrhythmias) or ventricular fibrillation, or known allergy to Dopamine Hydrochloride or, in sulfite-containing products, to sulfites.

Is Dopamine Hydrochloride safe during pregnancy?

There is limited human data. Because Dopamine Hydrochloride is used only in life-threatening shock, it should be given during pregnancy only if the potential benefit to the mother clearly outweighs the potential risk to the fetus, under close physician supervision.

What are the main risks of Dopamine Hydrochloride treatment?

The main risks include irregular heartbeats, chest pain, very high or low blood pressure, and severe tissue damage if the drug leaks out of the vein (extravasation) at the infusion site. Because of these risks, Dopamine Hydrochloride is given only with continuous monitoring by trained hospital staff.
