About Lidocaine + Adrenaline

Indications of Lidocaine + Adrenaline

Lidocaine + Adrenaline injection is indicated for the production of local or regional anesthesia by infiltration, peripheral nerve block, and other techniques where a prolonged duration of anesthesia with reduced bleeding at the site is desirable.

Established / approved uses

  • Local infiltration anesthesia for minor surgical and dermatologic procedures
  • Dental anesthesia (infiltration and nerve block) prior to dental extractions and restorative procedures
  • Peripheral nerve block anesthesia for minor limb and soft-tissue procedures

Adjunct use

  • The adrenaline (epinephrine) component is added as a vasoconstrictor adjunct to lidocaine to prolong anesthetic duration, reduce systemic absorption, and provide local hemostasis; it is not used as a stand-alone anesthetic.

Lidocaine + Adrenaline is not indicated for spinal or intravenous regional anesthesia (Bier block) or for injection into end-arterial areas.

Dosage & Administration of Lidocaine + Adrenaline

The dose of Lidocaine + Adrenaline depends on the procedure, the area to be anesthetized, vascularity of the tissue, and the physical status of the patient. The lowest dose that produces effective anesthesia should be used.

IndicationTypical adult dosing
Dental infiltration/nerve blockUsually 1-5 mL of 2% lidocaine with adrenaline (1:100,000 or 1:80,000) per injection site, as directed by the dentist
Local infiltration (minor surgery/dermatology)Volume individualized to area; typically 1% or 2% lidocaine with adrenaline 1:200,000, using the minimum volume needed
Peripheral nerve blockIndividualized by the anesthetist/surgeon according to the nerve(s) blocked and patient factors

Maximum recommended dose: The maximum dose of the lidocaine component when combined with adrenaline is generally up to 7 mg/kg body weight (not exceeding a total of approximately 500 mg in a healthy adult), and the adrenaline component should not exceed approximately 200-250 mcg (0.2-0.25 mg) in a single procedure, or lower limits in patients with cardiovascular disease, as advised by the treating clinician. Doses should be reduced in elderly, debilitated, or medically compromised patients and in children (calculated by weight).

Pediatric dosing: Calculated strictly on a mg/kg basis according to the child's weight and the procedure; should only be administered by clinicians experienced in pediatric local anesthesia.

Side Effects of Lidocaine + Adrenaline

Side effects of Lidocaine + Adrenaline may be local (related to the injection) or systemic (related to absorption of either component).

Local effects

  • Injection site pain, transient blanching of the skin, prolonged numbness, or bruising

Systemic effects - lidocaine component

  • Common: dizziness, drowsiness, perioral numbness or tingling, tinnitus, blurred vision
  • Serious (with excessive dose or inadvertent intravascular injection): tremor, muscle twitching, seizures, hypotension, bradycardia, cardiac arrhythmia, and, rarely, cardiac arrest (local anesthetic systemic toxicity, LAST)

Systemic effects - adrenaline component

  • Transient palpitations, tachycardia, anxiety, tremor, headache, and elevated blood pressure, particularly with inadvertent intravascular injection

Contraindications of Lidocaine + Adrenaline

Lidocaine + Adrenaline is contraindicated in:

  • Known hypersensitivity to lidocaine, any other amide-type local anesthetic, adrenaline, or sulfite preservatives contained in the formulation.
  • Injection into end-arterial or poorly collateralized tissue (e.g., fingers, toes, tip of the nose, ears, or penis), due to the risk of ischemic tissue necrosis from local vasoconstriction.
  • Severe, uncontrolled cardiac arrhythmias.

Frequently Asked Questions about Lidocaine + Adrenaline

What is Lidocaine + Adrenaline used for?

Lidocaine + Adrenaline is an injectable local anesthetic used to numb a specific area of the body before dental work, minor surgery, or other procedures. The adrenaline component makes the numbing effect last longer and reduces bleeding at the injection site.

Is Lidocaine + Adrenaline safe during pregnancy?

Lidocaine + Adrenaline should be used during pregnancy only if clearly needed and if the potential benefit justifies the potential risk to the fetus. Please consult your physician or dentist before receiving this injection if you are pregnant.

Why can't Lidocaine + Adrenaline be used on fingers, toes, or the nose?

Lidocaine + Adrenaline must not be injected into areas such as the fingers, toes, tip of the nose, ears, or penis, because these areas have limited alternate blood supply. The vasoconstriction caused by the adrenaline component can dangerously reduce blood flow in these areas and lead to tissue damage.

What are the signs of an adverse reaction to Lidocaine + Adrenaline?

Signs may include dizziness, ringing in the ears, numbness around the mouth, tremor, palpitations, a fast heartbeat, or in severe cases, seizures or irregular heart rhythm. If you experience any of these symptoms after receiving Lidocaine + Adrenaline, tell your healthcare provider immediately, as this may require urgent treatment.

Can Lidocaine + Adrenaline be used in patients with heart disease?

Lidocaine + Adrenaline should be used with caution in patients with cardiovascular disease, since the adrenaline component can raise heart rate and blood pressure. Your doctor will weigh the benefits and risks and may use a lower dose or an alternative if you have significant heart disease.

What should I do if too much Lidocaine + Adrenaline is given or injected into a blood vessel by accident?

This is a medical emergency. If excessive Lidocaine + Adrenaline is given or accidentally injected into a blood vessel, seek immediate medical attention or contact emergency services right away; the treating team will provide supportive care and specific emergency treatment if needed.
