About Methyldopa

Indications of Methyldopa

Methyldopa is a centrally-acting antihypertensive agent (alpha-2 adrenergic agonist) approved for the treatment of hypertension. Its evidence-based uses include:

  • Established/FDA-approved use: Management of mild to moderate hypertension, alone or in combination with other antihypertensive agents (e.g., diuretics), when blood pressure is not adequately controlled by a single agent.
  • Guideline-supported use: Methyldopa is one of the preferred agents for hypertension occurring during pregnancy (including chronic hypertension and gestational hypertension), given its long-established safety record in this population compared with many newer antihypertensives.
  • Combination therapy: When used with a diuretic or other antihypertensive, Methyldopa may allow for a lower dose of each agent and improved blood pressure control.

Methyldopa is not indicated for hypertensive emergencies requiring rapid blood pressure reduction.

Renal Impairment

Patients with reduced renal function may be more sensitive to the antihypertensive effect and may respond to lower doses; dose should be individualized with careful monitoring of blood pressure.

Frequently Asked Questions about Methyldopa

What is Methyldopa used for?

Methyldopa is used to treat high blood pressure (hypertension). It is especially well established for treating hypertension that occurs during pregnancy.

Is Methyldopa safe to take during pregnancy?

Methyldopa has a long history of use in pregnancy and is considered one of the preferred antihypertensive options in this setting, but it should still be used only if clearly needed and under the supervision of a physician, who will weigh the potential benefits against any potential risks.

Can Methyldopa cause liver problems?

Yes, rarely Methyldopa can cause liver injury, including severe hepatitis-like reactions. Your doctor may check your liver function periodically, especially in the first 6-12 weeks of treatment. Seek medical attention immediately if you develop fever, yellowing of the skin/eyes, dark urine, or unusual tiredness.

Why do I feel sleepy after starting Methyldopa?

Sedation or drowsiness is a common side effect, especially when starting Methyldopa or after a dose increase. This usually improves within a few weeks of continued treatment. Avoid driving or operating machinery until you know how it affects you.

Can I take Methyldopa with iron tablets?

Iron supplements can reduce the absorption and effectiveness of Methyldopa. Tell your doctor if you are taking iron supplements, as they may adjust the timing of doses or choose an alternative.

What should I do if I miss a dose of Methyldopa?

Take the missed dose as soon as you remember, unless it is almost time for your next dose, in which case skip the missed dose. Do not take a double dose to make up for a missed one. Contact your doctor if you have questions about your dosing schedule.
