About Milk of Magnesia

Indications of Milk of Magnesia

Milk of Magnesia is an over-the-counter magnesium hydroxide suspension with two dose-dependent, established uses:

  • Antacid (low dose): relief of heartburn, acid indigestion, and upset stomach associated with these symptoms.
  • Saline laxative (higher dose): relief of occasional constipation; also used for bowel evacuation before certain procedures under medical supervision.

These are established, monograph-supported OTC uses. Milk of Magnesia is not indicated for chronic or long-term management of constipation or dyspepsia without medical evaluation, and it is not an antibiotic or a treatment for any infection.

Side Effects of Milk of Magnesia

When used as directed, Milk of Magnesia is generally well tolerated. Reported side effects include:

  • Diarrhea and abdominal cramping (more common with laxative-dose use)
  • Nausea
  • Electrolyte disturbances with excessive or prolonged use

Rare but serious: hypermagnesemia (particularly in patients with renal impairment or with excessive/prolonged use), presenting as weakness, low blood pressure, irregular heartbeat, or confusion - see Overdose Effects. Stop use and consult a physician if rectal bleeding occurs or if there is no bowel movement after use (possible sign of a bowel obstruction).

Contraindications of Milk of Magnesia

Milk of Magnesia is contraindicated in patients with:

  • Known hypersensitivity to magnesium hydroxide or any component of the formulation.
  • Appendicitis or symptoms suggestive of appendicitis (e.g., abdominal pain, nausea, vomiting) - laxative use may risk perforation of an inflamed appendix.
  • Severe renal impairment/renal failure, in which magnesium cannot be adequately excreted, risking hypermagnesemia.
