About Sorafenib Tosylate

Indications of Sorafenib Tosylate

Sorafenib Tosylate is an oral multi-targeted tyrosine kinase inhibitor with the following evidence-based indications:

Established / Approved uses

  • Unresectable hepatocellular carcinoma (HCC): Sorafenib Tosylate is indicated for the treatment of patients with liver cancer that cannot be surgically removed.
  • Advanced renal cell carcinoma (RCC): Sorafenib Tosylate is indicated for the treatment of advanced kidney cancer, typically after failure of prior cytokine-based therapy or as an alternative targeted agent.
  • Differentiated thyroid carcinoma (DTC): Sorafenib Tosylate is indicated for locally recurrent or metastatic, progressive differentiated thyroid cancer that is refractory to radioactive iodine (RAI) treatment.

Sorafenib Tosylate is not indicated for medullary or anaplastic thyroid cancer, and is not established for use in children.

Dosage & Administration of Sorafenib Tosylate

Recommended dosing by indication

IndicationAdult dose
Unresectable hepatocellular carcinoma400 mg (two 200 mg tablets) orally twice daily, continued until lack of clinical benefit or unacceptable toxicity
Advanced renal cell carcinoma400 mg (two 200 mg tablets) orally twice daily, continued until lack of clinical benefit or unacceptable toxicity
Radioactive-iodine-refractory differentiated thyroid carcinoma400 mg (two 200 mg tablets) orally twice daily, continued until lack of clinical benefit or unacceptable toxicity

Administration

Take Sorafenib Tosylate tablets on an empty stomach, at least 1 hour before or 2 hours after a meal, and swallow whole with a full glass of water. Avoid taking with a high-fat meal, as this reduces absorption.

Dose modification for toxicity

For adverse reactions such as hand-foot skin reaction, hypertension, or other significant toxicity, the physician may interrupt therapy and/or reduce the dose in a stepwise manner, typically first to 400 mg once daily, and further to 400 mg every other day, according to severity.

Hepatic impairment

No dose adjustment is required in patients with mild to moderate hepatic impairment (Child-Pugh A or B). Data in severe hepatic impairment (Child-Pugh C) are limited; use with caution and close monitoring.

Renal impairment

No dose adjustment is generally required in mild-to-moderate renal impairment. Data in severe renal impairment or patients on dialysis are limited; use with caution.
