About Sparfloxacin

Indications of Sparfloxacin

Sparfloxacin is a broad-spectrum fluoroquinolone antibacterial used to treat certain bacterial infections caused by susceptible organisms in adults. Because of its phototoxicity and QT-prolongation risk, it is generally reserved for situations where more established antibiotics are unsuitable, and treatment should be guided by culture and susceptibility testing where possible.

Established / guideline-supported uses

  • Community-acquired pneumonia (CAP) caused by susceptible bacteria.
  • Acute bacterial exacerbation of chronic bronchitis (AECB) in patients with underlying chronic obstructive pulmonary disease.
  • Acute bacterial maxillary sinusitis caused by susceptible organisms.

Adjunct / combination-therapy use

  • As part of multidrug regimens for tuberculosis or leprosy in select cases where first-line agents cannot be used; Sparfloxacin is not used as monotherapy for these conditions and must be combined with other anti-mycobacterial agents under specialist supervision.

Off-label / less-established use

  • Complicated urinary tract infections and bacterial prostatitis, where local resistance patterns support its use — considered off-label/second-line in most current guidance given the availability of safer fluoroquinolones.

Sparfloxacin should only be used for infections proven or strongly suspected to be caused by susceptible bacteria, in line with antibiotic stewardship principles.

Dosage & Administration of Sparfloxacin

Sparfloxacin is for oral use only and is intended for adults. Take Sparfloxacin exactly as prescribed by your physician; do not stop, extend, skip doses, or share this medicine with others without medical advice, as inappropriate use of antibiotics can promote resistance and treatment failure.

IndicationAdult dosage regimen
Community-acquired pneumonia400 mg (loading dose) on day 1, then 200 mg once daily, usually for a total of 10 days
Acute exacerbation of chronic bronchitis400 mg (loading dose) on day 1, then 200 mg once daily, usually for a total of 10 days
Acute bacterial maxillary sinusitis400 mg (loading dose) on day 1, then 200 mg once daily for the duration advised by the physician

Renal impairment

For creatinine clearance below 50 mL/min: 400 mg loading dose on day 1, then 200 mg every 48 hours for the remainder of therapy. See Use in Special Populations for further detail.

Administration instructions

  • Swallow the tablet whole with a full glass of water; may be taken with or without food.
  • Maintain good hydration during treatment.
  • Separate dosing from antacids, and products containing iron, zinc, calcium, or magnesium by at least 4 hours, as these markedly reduce absorption.
  • Avoid direct and prolonged sun/UV light exposure, including sunlamps, during treatment and for several days after the last dose (see Precautions).

Side Effects of Sparfloxacin

Most side effects of Sparfloxacin are mild to moderate and transient, but some are serious and require prompt medical attention.

Common

  • Photosensitivity/phototoxicity: notably more frequent and more severe with Sparfloxacin than with many other fluoroquinolones — presents as exaggerated sunburn-like reaction; sun/UV exposure should be avoided.
  • Nausea, diarrhea, dyspepsia, abdominal pain, vomiting, flatulence.
  • Headache, dizziness, insomnia.
  • Taste disturbance, itching (pruritus).

Less common but serious

  • QT interval prolongation and, rarely, serious cardiac arrhythmia.
  • Tendinitis and tendon rupture (see Precautions).
  • Peripheral neuropathy.
  • CNS effects: confusion, hallucinations, seizures (rare).
  • Hypersensitivity reactions, including severe skin reactions.

Contraindications of Sparfloxacin

Sparfloxacin is contraindicated in:

  • Known hypersensitivity to Sparfloxacin or any other fluoroquinolone antibacterial.
  • A prior history of tendinitis or tendon rupture associated with fluoroquinolone use.
  • Known QT interval prolongation, or uncorrected hypokalemia.
  • Concurrent use with other drugs known to prolong the QT interval.
  • Myasthenia gravis, due to risk of exacerbating muscle weakness.
  • Glucose-6-phosphate dehydrogenase (G6PD) deficiency, due to risk of hemolysis.
  • Pregnancy and breastfeeding (see Pregnancy and Lactation).

Frequently Asked Questions about Sparfloxacin

What is Sparfloxacin used for?

Sparfloxacin is a fluoroquinolone antibiotic used to treat certain bacterial infections in adults, mainly community-acquired pneumonia, acute exacerbation of chronic bronchitis, and acute bacterial sinusitis, when the infection is caused by susceptible bacteria.

How should I take Sparfloxacin?

Take Sparfloxacin exactly as prescribed — typically a 400 mg loading dose on the first day followed by 200 mg once daily. Swallow the tablet with water, with or without food, and complete the full course even if you feel better early. Do not stop, extend, skip doses, or share this medicine with others without medical advice.

Can I take Sparfloxacin with antacids or supplements?

No. Antacids and supplements containing aluminum, magnesium, calcium, iron, or zinc significantly reduce absorption of Sparfloxacin. If you need to take these, separate them from your Sparfloxacin dose by at least 4 hours.

Why do I need to avoid sunlight while taking Sparfloxacin?

Sparfloxacin can cause moderate to severe phototoxic skin reactions — an exaggerated sunburn-like response — that is more pronounced with this medicine than with many other fluoroquinolones. Avoid direct sunlight, tanning beds, and sunlamps during treatment and for several days after your last dose, and use protective clothing/sunscreen if exposure is unavoidable.

Is Sparfloxacin safe during pregnancy or breastfeeding?

Sparfloxacin is generally contraindicated in pregnancy and breastfeeding because fluoroquinolones may affect the developing fetus's cartilage and pass into breast milk. It should only be used in pregnancy if a physician determines there is no suitable alternative and the benefit clearly outweighs the risk. Always consult your physician if you are pregnant, planning pregnancy, or breastfeeding.

What should I do if I notice tendon pain while taking Sparfloxacin?

Stop taking Sparfloxacin, rest the affected limb, and contact your physician immediately. Fluoroquinolones, including Sparfloxacin, carry a risk of tendinitis and tendon rupture, especially in people over 60, those on corticosteroids, and organ transplant recipients.
