Biocid Plus(200 mg+400 mg+30 mg)/5 ml

Biocid Plus is an over-the-counter (OTC) combination antacid and antiflatulent used for the short-term, symptomatic relief of:

• Heartburn and acid indigestion
• Sour stomach
• Hyperacidity associated with peptic ulcer disease, gastritis, and peptic oesophagitis (reflux)
• Excess gas, bloating, and abdominal discomfort due to trapped gas (flatulence)

These are established OTC uses supported by FDA antacid/antigas monograph data. Biocid Plus is not intended for the diagnosis or long-term management of underlying gastrointestinal disease.

Aluminium Hydroxide + Magnesium Hydroxide + Simethicone is available as tablets/chewable tablets and as an oral suspension. Representative strengths include:

• Tablet: Dried Aluminium Hydroxide Gel BP ~425 mg + Magnesium Hydroxide BP 400 mg + Simethicone USP 30 mg
• Oral suspension (per 5 mL): Aluminium Hydroxide (as Aluminium Oxide equivalent) 200 mg + Magnesium Hydroxide 400 mg + Simethicone USP 30 mg

Biocid Plus is a fixed-dose combination OTC medicine that pairs two acid-neutralizing antacid salts (aluminium hydroxide and magnesium hydroxide) with an antiflatulent (simethicone). The antacid components neutralize excess stomach acid to relieve heartburn and indigestion, while simethicone works locally in the gut to break up trapped gas bubbles and ease bloating. The two antacid salts are combined because aluminium hydroxide tends to be constipating and magnesium hydroxide tends to be laxative, so the combination is formulated to help balance bowel effects. It is widely available in Bangladesh under numerous brand names as tablets and suspensions.

Antacid and Antiflatulent combination (non-systemic gastric acid neutralizer with anti-gas agent).

Aluminium hydroxide and magnesium hydroxide are non-systemic antacids that react chemically with hydrochloric acid in the stomach to raise intragastric pH, thereby neutralizing gastric acid and relieving acid-related symptoms; they do not inhibit acid secretion. Simethicone is a physiologically inert silicone-based agent that reduces the surface tension of gas bubbles in the gastrointestinal tract, causing small bubbles to coalesce so trapped gas can be expelled more easily through belching or flatus. Simethicone is not absorbed systemically and is excreted unchanged in the stool. Onset of antacid effect is rapid (minutes) but duration is relatively short, which is why frequent dosing (after meals and at bedtime) is used.

Chew tablets thoroughly before swallowing; do not swallow whole. Shake the oral suspension well before each use and measure the dose with an accurate measuring device (not a household spoon). Take doses after meals and at bedtime for best acid-neutralizing effect, or as symptoms of gas/bloating occur. Take with a small amount of water if desired. If prescription or other oral medicines are also being taken, separate their dosing from Biocid Plus by at least 2 hours (see Interaction).

Biocid Plus can reduce the absorption and effectiveness of several oral medicines by binding to them in the gut or by raising gastric pH. Clinically significant interactions include:

• Tetracycline and fluoroquinolone antibiotics (e.g., doxycycline, ciprofloxacin) - absorption significantly reduced; separate dosing by at least 2 hours.
• Iron supplements - absorption reduced; separate dosing by at least 2 hours.
• Digoxin - absorption may be reduced; separate dosing by at least 2 hours and monitor as advised by a physician.
• Antifungal azoles (e.g., ketoconazole, itraconazole) which require gastric acid for absorption - effectiveness may be reduced; separate dosing by at least 2 hours.
• Other drugs whose absorption may be altered include theophylline, penicillamine, and some sulfonamides.

As a general rule, separate Biocid Plus from other oral medicines by at least 2 hours, and consult a physician or pharmacist if regularly taking other prescription drugs.

When used as directed, side effects are generally uncommon and mild. Possible effects include:

• Constipation - more associated with the aluminium hydroxide component
• Diarrhea - more associated with the magnesium hydroxide component
• Stomach cramps, chalky taste, or mild nausea
• With excessive or prolonged use, or in patients with impaired kidney function: hypermagnesemia (symptoms such as fatigue, muscle weakness, low blood pressure) or aluminium accumulation/toxicity

Because the aluminium and magnesium components have opposing effects on bowel habit, the combination is formulated to reduce the likelihood of either constipation or diarrhea, though either can still occur.

Biocid Plus is generally considered a low-risk OTC antacid option in pregnancy and breastfeeding when used occasionally at recommended doses, as the components act largely within the gut with minimal systemic absorption. However, some guidance advises caution during the first trimester, and regular or prolonged use in pregnancy has not been rigorously studied. Pregnant and breastfeeding women should consult a physician or pharmacist before use, particularly for anything beyond occasional, short-term use.

Use with caution in patients with renal (kidney) impairment - reduced ability to excrete aluminium and magnesium may lead to accumulation and toxicity (hypermagnesemia, aluminium toxicity) with regular or prolonged use; such patients should avoid this product unless directed by a physician. Use with caution in the elderly, in patients on magnesium-restricted diets, and in those with hypophosphataemia. Do not use continuously for more than 2 weeks without consulting a physician - persistent or worsening symptoms (e.g., ongoing indigestion, black or tarry stools, unexplained weight loss) may indicate a more serious underlying condition and warrant medical evaluation rather than continued self-treatment. Keep out of reach of children.

Deliberate or accidental overdose may cause pronounced gastrointestinal effects (severe diarrhea or constipation, abdominal cramping) and, particularly in those with reduced kidney function, may lead to hypermagnesemia (drowsiness, muscle weakness, low blood pressure, slowed heart rate) or aluminium toxicity with chronic excessive intake. If overdose is suspected, stop the medicine and seek prompt medical attention or contact a poison control center; treatment is generally supportive and directed at the presenting symptoms.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.

Renal impairment: Avoid or use only under medical supervision due to risk of aluminium/magnesium accumulation (see Contraindications and Precautions and Warnings).

Elderly: Use with caution, particularly with pre-existing kidney disease; monitor for bowel effects and electrolyte disturbance with regular use.

For self-medication, Biocid Plus should not be used continuously for more than 2 weeks. If symptoms of heartburn, indigestion, or gas persist, recur frequently, or worsen despite use, discontinue and consult a physician, since ongoing symptoms may signal a condition needing medical diagnosis and treatment beyond simple antacid therapy.

The aluminium and magnesium hydroxide components act by direct chemical neutralization of hydrochloric acid in gastric juice, raising intragastric pH and inactivating pepsin, which relieves symptoms of hyperacidity without altering acid secretion. Simethicone acts physically rather than chemically: it lowers the surface tension of gas bubbles trapped in mucus within the gastrointestinal tract, allowing small bubbles to merge into larger ones that are more easily passed or belched away, thereby relieving bloating and flatulence.

Biocid Plus is generally not recommended for children under 12 years of age for self-medication; use in young children should only occur under the guidance and supervision of a physician, who can advise on an appropriate product and dose. For children 12 years and older, adult OTC dosing may apply as directed on the product label, but a physician or pharmacist should be consulted, especially for any use beyond occasional, short-term relief.
