Bolt is a topical combination preparation (heparinoid-type mucopolysaccharide polysulphate plus the keratolytic agent salicylic acid) used for the local management of superficial venous and soft-tissue inflammatory conditions.

Established/commonly accepted topical uses

• Superficial thrombophlebitis (inflammation of a superficial vein, with or without a small clot)
• Resolution support for hematomas, bruises (contusions), and localized soft-tissue swelling following minor trauma or injections/infusions
• Localized inflammation, pain, and edema associated with sprains, strains, and soft-tissue injuries, as an adjunct to standard care (rest, compression, elevation)

Adjunct / supportive use

• As an adjunct alongside standard first-aid measures (rest, ice/compression, elevation) for minor sports injuries and localized swelling; not a substitute for evaluation of significant trauma.

Bolt is intended for local, external use on intact skin only; it is not indicated for deep vein thrombosis, systemic anticoagulation, or for use on open wounds or infected skin.

Mucopolysaccharide Polysulphate + Salicylic Acid is formulated as a topical gel, cream, or ointment. Each preparation combines Mucopolysaccharide Polysulphate (a semi-synthetic, heparin-like glycosaminoglycan/heparinoid) with Salicylic Acid (a keratolytic agent that promotes penetration of the active ingredient through the skin), in a base suitable for external application. Exact strengths and excipients vary by brand/manufacturer; refer to the specific product label for the precise concentration of each component.

Bolt is a topical anti-inflammatory preparation that combines a heparinoid (mucopolysaccharide polysulphate) with salicylic acid. Mucopolysaccharide polysulphate is chemically related to heparin and, when applied to the skin, exerts local anti-inflammatory, anti-exudative, and mild antithrombotic effects that help reduce swelling and support resorption of superficial clots and bruises. Salicylic acid is a keratolytic agent that softens the outer skin layer, improving penetration of the heparinoid into the affected tissue and contributing a mild local anti-inflammatory action of its own. Together, these components are used to relieve pain, swelling, and discoloration associated with superficial thrombophlebitis, hematomas, bruises, and localized soft-tissue inflammation.

Bolt belongs to the therapeutic class of Topical Anti-inflammatory / Antithrombotic (Heparinoid) Preparations combined with a Keratolytic, used for external treatment of superficial venous and soft-tissue inflammatory conditions.

Mechanism

Mucopolysaccharide Polysulphate + Salicylic Acid acts locally at the site of application. Mucopolysaccharide polysulphate is a heparin-like glycosaminoglycan that, after penetrating the skin, exhibits local anticoagulant, fibrinolytic-supportive, and anti-inflammatory activity; it inhibits local pro-inflammatory mediators, reduces capillary permeability and edema, and promotes resorption of extravasated blood (hematoma) and superficial clots. Salicylic acid is a keratolytic that loosens the intercellular matrix of the stratum corneum, enhancing cutaneous penetration of the heparinoid component, and also contributes a mild local anti-inflammatory and analgesic effect through local inhibition of inflammatory mediators.

Pharmacokinetics

With normal topical application to intact skin over limited body surface area, systemic absorption of both components is minimal. However, salicylic acid can be absorbed systemically to a clinically relevant degree if applied over large body surface areas, under occlusion, or on broken/inflamed skin, particularly in children; this is the basis for the precaution against extensive-area application (see Precautions and Warnings).

Bolt is for external, topical use only. Apply a thin film to clean, intact skin over the affected area and massage gently; do not apply to open wounds, mucous membranes, or under occlusive dressings.

Bolt has limited systemic absorption when used as directed on limited skin areas, so clinically significant systemic drug interactions are uncommon. Relevant considerations include:

• Systemic anticoagulants/antiplatelets: Applying Bolt over large areas or for prolonged periods in a patient on systemic anticoagulant or antiplatelet therapy could theoretically add to local antithrombotic effect; use conservatively (small area, limited duration) in such patients.
• Other topical salicylates/keratolytics: Concurrent use of other topical products containing salicylic acid or similar keratolytic agents on the same skin area may increase local irritation and salicylate systemic absorption; avoid combining on the same site.
• Systemic salicylate/NSAID therapy: In patients on high-dose oral salicylates or NSAIDs, extensive topical salicylic acid use adds to overall salicylate exposure and should be minimized.

Bolt is generally well tolerated when used as directed. Reported side effects are typically local and mild:

• Local skin irritation, burning, or stinging at the application site
• Erythema (redness) or mild rash
• Itching (pruritus)
• Contact dermatitis with prolonged or repeated use
• Rarely, allergic hypersensitivity reactions (localized swelling, urticaria)
• With excessive application over large areas, signs of systemic salicylate absorption (see Overdose Effects)

Discontinue use and consult a physician if severe irritation, blistering, or signs of an allergic reaction develop.

There are no well-controlled studies of Bolt in pregnant or breastfeeding women. Because the salicylic acid component can be absorbed systemically, particularly with use over large areas or for prolonged periods, Bolt should be used during pregnancy and lactation only if clearly needed and only over small, limited areas, and only after consulting a physician, who can weigh the potential benefit against potential risk to the fetus or nursing infant. Application to the breast area should be avoided during breastfeeding.

• Bolt is for external use only; avoid contact with the eyes, mouth, and other mucous membranes.
• Do not apply to open wounds, broken skin, or actively bleeding areas (see Contraindications).
• Avoid application over an extensive body surface area or for prolonged periods, especially in children, due to the risk of systemic salicylate absorption and salicylate toxicity.
• Use with caution in children; limit the area treated and the duration of use, and consult a physician before use in young children.
• Do not apply under occlusive (airtight) dressings unless specifically directed by a physician, as this increases systemic absorption.
• Discontinue use and seek medical advice if excessive local irritation, rash, blistering, or signs of allergy occur.
• Use cautiously, and only under medical supervision, in patients with known bleeding disorders or those taking systemic anticoagulant/antiplatelet medication, particularly if larger areas require treatment.
• Not intended for treatment of deep vein thrombosis or other conditions requiring systemic anticoagulation; seek prompt medical evaluation for suspected deep vein thrombosis, signs of infection, or significant/worsening swelling.

Overdose from normal, limited topical use of Bolt is unlikely. However, application over an extensive body surface area, prolonged use, use under occlusion, or accidental ingestion can lead to systemic salicylate absorption and salicylate toxicity, with features such as tinnitus (ringing in the ears), nausea, vomiting, rapid or deep breathing (hyperventilation), dizziness, or confusion; this risk is higher in children. If Bolt is accidentally swallowed, or if signs of salicylate toxicity appear after topical use, seek immediate medical attention or contact a poison control center/emergency services promptly. Do not attempt specific home treatment beyond washing off the product from the skin and seeking professional medical advice.

Store at room temperature (below 30°C), away from light and moisture. Keep the container tightly closed and out of reach of children.

Pediatric patients

Use Bolt with caution in children; treat only a small, limited area for the shortest effective duration, due to a higher risk of systemic salicylate absorption and toxicity in this population. See Pediatric Uses.

Elderly

No specific dose adjustment is established for elderly patients; standard application guidance applies, with attention to skin fragility and any concurrent anticoagulant therapy.

Renal/Hepatic impairment

Formal dosing studies in renal or hepatic impairment are not available. Because systemic absorption is normally minimal with limited-area topical use, significant accumulation is not expected, but extensive-area or prolonged application should be avoided in these patients as a precaution.

Pregnancy and lactation

Bolt is typically used until local symptoms (pain, swelling, bruising, or induration) resolve, commonly over a period of about 1-2 weeks. If there is no improvement within this period, or if symptoms worsen, treatment should be reassessed and a physician consulted, since prolonged use increases the risk of local irritation and, with extensive application, systemic salicylate absorption.

Heparinoid (Mucopolysaccharide Polysulphate) + Salicylic Acid; Topical Anti-inflammatory/Antithrombotic Preparation with Keratolytic Enhancer

Mucopolysaccharide Polysulphate + Salicylic Acid works through two complementary local actions: the heparinoid component (mucopolysaccharide polysulphate) reduces local inflammation, capillary permeability, and edema, and supports resorption of superficial clots and extravasated blood, while the salicylic acid component acts as a keratolytic that enhances skin penetration of the heparinoid and adds a mild local anti-inflammatory/analgesic effect of its own.

Safety and efficacy of Bolt in children have not been formally established through large controlled trials, and salicylic acid absorbed through the skin poses a greater risk of systemic toxicity in children than in adults, especially with use over large body areas or for prolonged periods. If use in a child is considered necessary, it should be limited to a small area, for the shortest effective duration, under the guidance of a physician; consult a pediatrician before use in infants and young children.
