• Ophthalmic surgical aid (viscosurgical device): Sterile, higher-viscosity Daily Fresh SUD 0.2% preparations are used intraoperatively during anterior segment surgery (e.g., cataract extraction/intraocular lens implantation, corneal transplantation, glaucoma filtration surgery) to maintain anterior chamber depth, protect the corneal endothelium and other ocular tissues, and facilitate surgical manipulation.
• Intra-articular injection for knee osteoarthritis (viscosupplementation): Injectable Daily Fresh SUD 0.2% preparations are indicated for the treatment of pain associated with osteoarthritis of the knee in patients who have failed to respond adequately to conservative non-pharmacologic therapy and simple analgesics (e.g., paracetamol).

Adjunct / second-line use

Intra-articular Daily Fresh SUD 0.2% is generally used as an adjunct after, or in combination with, first-line measures (weight management, physical therapy, oral or topical analgesics/NSAIDs) rather than as initial monotherapy.

Some formulations of Daily Fresh SUD 0.2% injection are also used off-label at other joints (e.g., hip, shoulder) by specialists, although knee osteoarthritis remains the primary evidence-based and labeled indication.

Each formulation contains Daily Fresh SUD 0.2% (the sodium salt of hyaluronic acid) as the active ingredient, at a concentration and molecular weight that varies by product and intended route:

• Ophthalmic drops: typically 0.1%-0.4% Daily Fresh SUD 0.2% in a sterile isotonic buffered solution, with or without preservative.

Pharmaceutical-grade Daily Fresh SUD 0.2% is manufactured either by extraction from animal tissue (e.g., rooster combs) or by bacterial fermentation, and is formulated into sterile products for three main clinical uses: as lubricating eye drops for dry eye, as a viscoelastic surgical aid during eye surgery, and as an injectable viscosupplement for osteoarthritic joints.

Daily Fresh SUD 0.2% belongs to the class of ophthalmic lubricants/artificial tear preparations (for eye drop formulations), ophthalmic viscosurgical devices (for surgical formulations), and viscosupplementation agents/anti-osteoarthritis agents (for intra-articular injectable formulations). It is classified pharmacologically as a glycosaminoglycan-based viscoelastic agent rather than a conventional small-molecule drug.

Mechanism of action

Daily Fresh SUD 0.2% does not act through a receptor-mediated pharmacological pathway; its effects are primarily physicochemical (mechanical/lubricating), arising from its viscoelastic and water-binding properties:

• Ophthalmic use: Daily Fresh SUD 0.2% forms a stable, lubricating film over the ocular surface, stabilizes the tear film, increases tear film residence time, and promotes corneal epithelial healing, thereby relieving dryness, irritation, and foreign-body sensation.
• Ophthalmic surgery: Its high viscosity and elasticity allow Daily Fresh SUD 0.2% to maintain space (e.g., the anterior chamber), cushion and protect delicate intraocular tissues such as the corneal endothelium, and improve visualization and maneuverability of surgical instruments.
• Intra-articular use: In osteoarthritic joints, endogenous hyaluronic acid concentration and molecular weight are reduced, impairing the synovial fluid's normal viscoelastic (shock-absorbing and lubricating) properties. Injected Daily Fresh SUD 0.2% is thought to restore some of this viscoelasticity, cushion the joint during movement, and may also have anti-inflammatory and analgesic effects through interactions with synovial cell receptors (e.g., CD44), although the exact mechanism of pain relief is not completely established.

Systemic absorption after ophthalmic or intra-articular administration of Daily Fresh SUD 0.2% is minimal, and it is ultimately metabolized locally and systemically via normal hyaluronic acid turnover pathways (hyaluronidase-mediated degradation).

Daily Fresh SUD 0.2% ophthalmic drops are self-administered topically into the conjunctival sac of the affected eye(s). Daily Fresh SUD 0.2% intra-articular injections and ophthalmic surgical viscoelastic preparations must be administered only by a qualified physician/ophthalmic surgeon under sterile conditions; they are never self-injected or self-administered by the patient.

Daily Fresh SUD 0.2% has minimal potential for clinically significant systemic drug interactions. This is because its formulations act locally (on the ocular surface, within the eye during surgery, or within the joint space) with negligible systemic absorption, and because its mechanism of action is physicochemical (lubricating/viscoelastic) rather than receptor- or enzyme-mediated.

• No well-established, clinically significant drug-drug interactions have been documented for Daily Fresh SUD 0.2% eye drops or intra-articular injections.
• As a general precaution, concurrent use of other ophthalmic products should be separated by at least 5-10 minutes from Daily Fresh SUD 0.2% eye drops to avoid dilution or washout of either product.
• There is no established interaction between intra-articular Daily Fresh SUD 0.2% and oral analgesics, NSAIDs, or disease-modifying agents used concurrently for osteoarthritis.

Ophthalmic use

• Transient blurred vision immediately after instillation
• Mild, temporary stinging, burning, or foreign-body sensation
• Ocular redness or discomfort (uncommon)

Intra-articular use

• Injection-site pain, swelling, warmth, or joint effusion (most common)
• Transient arthralgia or stiffness after injection
• Rare pseudoseptic reactions (acute inflammatory joint reaction without infection)
• Rare joint infection - a recognized procedural risk of any intra-articular injection rather than a direct pharmacologic effect of Daily Fresh SUD 0.2% itself (see Precautions and Warnings)
• Rare hypersensitivity reactions (itching, rash) at or away from the injection site

Ophthalmic surgical use

Generally well tolerated intraoperatively; transient postoperative increase in intraocular pressure has been reported and is usually managed by the surgical team.

Formal controlled studies of Daily Fresh SUD 0.2% in pregnant or breastfeeding women are limited. Because ophthalmic and intra-articular Daily Fresh SUD 0.2% preparations act locally with minimal systemic absorption, the theoretical risk to the fetus or nursing infant is considered low; however, Daily Fresh SUD 0.2% should be used during pregnancy and lactation only if clearly needed, and only after the potential benefit is judged by a physician to justify any potential risk. Pregnant or breastfeeding patients should consult their physician before using any Daily Fresh SUD 0.2% product, particularly injectable or surgical formulations.

• Intra-articular Daily Fresh SUD 0.2% injections must be performed using strict aseptic/sterile technique to minimize the risk of joint infection; see Contraindications for infected-joint restrictions.
• Daily Fresh SUD 0.2% intra-articular injection should not be injected extra-articularly or into synovial tissues or capsule, as this may cause local reactions.
• Patients should avoid excessive strain or high-impact activity on the injected joint for a short period after Daily Fresh SUD 0.2% injection, per the treating physician's advice.
• Daily Fresh SUD 0.2% ophthalmic drops should not be used if the solution has changed color or become cloudy, and single-use vials should be discarded after use.
• Patients using Daily Fresh SUD 0.2% eye drops who wear contact lenses should remove lenses before instillation and wait as directed before reinserting them.
• Use caution in patients with a known history of hypersensitivity reactions to hyaluronic acid products or avian proteins (for avian-derived formulations of Daily Fresh SUD 0.2%).

Significant systemic overdose of Daily Fresh SUD 0.2% is unlikely given its local routes of administration (ophthalmic, intraocular, intra-articular) and minimal systemic absorption. Accidental excessive ophthalmic use may cause temporary blurred vision or ocular irritation, which typically resolves without treatment. Excessive intra-articular volume administered by a healthcare provider may cause increased joint swelling or discomfort. If accidental ingestion or a significant excessive dose occurs, or if severe or persistent symptoms develop after any use of Daily Fresh SUD 0.2%, seek immediate medical attention or contact a poison control center; do not attempt unsupervised home treatment.

Store Daily Fresh SUD 0.2% products at room temperature (below 30°C), protected from light and moisture, unless the specific product labeling directs refrigeration. Do not freeze injectable Daily Fresh SUD 0.2% pre-filled syringes. Keep out of reach of children. Discard opened multi-dose ophthalmic products and single-use vials/syringes after the labeled in-use period or after a single use, as directed on the product label.

Pediatric use

Safety and efficacy of intra-articular Daily Fresh SUD 0.2% for osteoarthritis have not been established in pediatric patients, as knee osteoarthritis is primarily a disease of adults. Ophthalmic Daily Fresh SUD 0.2% lubricant drops may be used in children for dry eye or ocular surface protection under medical supervision, but pediatric-specific dosing data are limited.

Elderly

Daily Fresh SUD 0.2% is commonly used in elderly patients, who represent the majority of patients with knee osteoarthritis and dry eye disease; no specific dose adjustment is required based on age alone, though overall clinical status should guide use.

Renal/hepatic impairment

No dose adjustment is required for Daily Fresh SUD 0.2% in renal or hepatic impairment, as its local routes of administration result in minimal systemic exposure and metabolism.

Duration of Daily Fresh SUD 0.2% treatment depends on indication: ophthalmic lubricant drops may be used chronically, on an as-needed basis, for as long as symptomatic dry eye persists. Intra-articular knee injections are typically given as a short course (commonly 1 to 5 weekly injections depending on the specific product), with symptom relief often lasting several months; repeat courses may be considered no more often than approximately every 6 months, based on physician assessment. Ophthalmic surgical use of Daily Fresh SUD 0.2% is limited to the duration of the surgical procedure itself.

Daily Fresh SUD 0.2% is classified as a glycosaminoglycan-based viscoelastic agent, encompassing the therapeutic categories of ophthalmic lubricant/artificial tear, ophthalmic viscosurgical device (OVD), and viscosupplementation/anti-osteoarthritis agent, depending on the formulation and route used.
